"The Toxic Depot": Parenteral Insecticide Injection.

BACKGROUND: Organophosphorus (OP) pesticides are extensively used both in developed and developing countries. Organophosphorus poisoning primarily occurs through occupational, accidental, and suicidal exposures. Toxicity through parenteral injections is seldom reported and there are only very few case reports till date. CASE PRESENTATION: We report a case of parenteral injection of 10 mL of OP compound (Dichlorvos 76%) into a swelling over the left leg. The compound was injected by the patient himself as adjuvant therapy for swelling. Initial manifestations included vomiting, abdomen pain, an

Functional Chemical Groups that May Likely Become a Source for the Synthesis of Novel Central Nervous System (CNS) Acting Drugs.

INTRODUCTION: Central Nervous System (CNS) disorders are on increase perhaps due to genetic, enviromental, social and dietetic factors. Unfortunately, a large number of CNS drugs have adverse effects such as addiction, tolerance, psychological and physical dependence. In view of this, literature search was carried out with a view to identify functional chemical groups that may serve as lead molecules for synthesis of compounds that may have CNS activity. METHODS: The search revealed that heterocycles that have heteroatoms such as nitrogen (N), sulphur (S) and oxygen (O) form the largest class

Comparison of in vitro potency of oximes (pralidoxime, obidoxime, HI-6) to reactivate sarin-inhibited acetylcholinesterase in various parts of pig brain.

The potency of currently used oximes to reactivate sarin-inhibited acetylcholinesterase (AChE) in various parts of pig brain and whole pig brain was evaluated using in vitro methods. Significant differences in reactivation potency among all tested oximes were observed. At concentrations (10(-4) M) corresponding to recommended doses in vivo, the oxime HI-6 seems to be a more efficacious reactivator of sarin-inhibited AChE in whole pig brain as well as in cerebral hemispheres and cerebellum compared with the other oximes studied. Nevertheless, there are not any differences in the potency of oxim

Oximes in acute organophosphorus pesticide poisoning: a systematic review of clinical trials.

BACKGROUND: Acute organophosphorus (OP) pesticide poisoning is widespread in the developing world. Standard treatment involves the administration of intravenous atropine and an oxime to counter acetylcholinesterase inhibition at the synapse, but the usefulness of oximes is uncertain. AIM: To assess the evidence on the use of oximes in OP poisoning. DESIGN: Systematic review. METHODS: We searched Medline, Embase, and Cochrane databases (last check 01/02/02) for 'organophosphate' or 'oxime' together with 'poisoning' or 'overdose'. We cross-referenced from other articles, and contacted experts

Transdermal Delivery of 2-PAM as a Tool to Increase the Effectiveness of Traditional Treatment of Organophosphate Poisoning.

For the first time, the efficacy of post-exposure treatment of organophosphate (OP) poisoning was increased by transdermal delivery of acetylcholinesterase (AChE) reactivator pyridine-2-aldoxime methochloride (2-PAM) as a preventive countermeasure. By selecting the optimal ratio of components, classical transfersomes (based on soybean phosphatidylcholine and Tween 20) and modified transfersomes (based on soybean phosphatidylcholine, Tween 20 and pyrrolidinium cationic surfactants with different hydrocarbon tail lengths) were obtained for 2-PAM encapsulation. Transfersomes modified with tetrade

Mechanism of central hypopnoea induced by organic phosphorus poisoning.

Whether central apnoea or hypopnoea can be induced by organophosphorus poisoning remains unknown to date. By using the acute brainstem slice method and multi-electrode array system, we established a paraoxon (a typical acetylcholinesterase inhibitor) poisoning model to investigate the time-dependent changes in respiratory burst amplitudes of the pre-Bötzinger complex (respiratory rhythm generator). We then determined whether pralidoxime or atropine, which are antidotes of paraoxon, could counteract the effects of paraoxon. Herein, we showed that paraoxon significantly decreased the respiratory

[The activity of blood cholinesterase in rats exposed to dimethypo after drug intervention].

OBJECTIVE: To investigate the activity of ChE in rats poisoned by dimehypo and then treated with pralidoxime methylchloride or unithiol. METHOD: Rats were divided into control group (dimehypo); intervention groups [dimehypo plus pralidoxime methylchloride or dimehypo plus unithiol (sodium dimercaptopropanesulphonate)]. Rats were dosed with 4 different doses of dimehypo: 1/16, 1/8, 1/4 and 1/2 of LD50 respectively(the LD50 of dimehypo is 342 mg/kg). After being poisoned with dimehypo orally, rats were immediately injected intramuscularly with pralidoxime methylchloride or unithiol. The activit

A-series agent A-234: initial in vitro and in vivo characterization.

A-series agent A-234 belongs to a new generation of nerve agents. The poisoning of a former Russian spy Sergei Skripal and his daughter in Salisbury, England, in March 2018 led to the inclusion of A-234 and other A-series agents into the Chemical Weapons Convention. Even though five years have already passed, there is still very little information on its chemical properties, biological activities, and treatment options with established antidotes. In this article, we first assessed A-234 stability in neutral pH for subsequent experiments. Then, we determined its inhibitory potential towards hum

From Cholinergic Crisis to Anticholinergic Delirium: When the Antidote Mimics the Poison.

Organophosphorus pesticide poisoning remains a major public health concern due to widespread agricultural use and easy availability of these compounds. Atropine is the cornerstone of treatment for life-threatening muscarinic manifestations. However, repeated or prolonged administration can cause central anticholinergic toxicity, which may closely resemble the neurological effects of organophosphorus poisoning and delay the correct diagnosis. We report a 26-year-old woman who developed severe cholinergic toxicity after intentionally ingesting monocrotophos and was treated with mechanical ventil

Antibody-Assisted Protection from Paraoxon and Nerve Agent Model Compounds.

A key challenge in organophosphorus (OP) research is to develop efficient means to decrease OP levels after exposure. Scavenging, capturing, or degrading circulating OPs in vivo could minimize OP penetration into the brain and other organs, and decrease toxicity. OPs produce acute effects primarily through acetylcholinesterase (AChE) inhibition, leading to neurotoxic events. Current standard of care (SOC) for OP exposure includes the AChE reactivator 2-PAM as well as atropine, and midazolam or diazepam. However, SOC therapeutics only address AChE symptomatic toxicity but do not increase cleara

Hypotension in severe dimethoate self-poisoning.

INTRODUCTION: Acute self-poisoning with the organophosphorus (OP) pesticide dimethoate has a human case fatality three-fold higher than poisoning with chlorpyrifos despite similar animal toxicity. The typical clinical presentation of severe dimethoate poisoning is quite distinct from that of chlorpyrifos and other OP pesticides: many patients present with hypotension that progresses to shock and death within 12-48 h post-ingestion. The pathophysiology of this syndrome is not clear. CASE REPORTS: We present here three patients with proven severe dimethoate poisoning. Clinically, all had inappr

The lesion site of organophosphorus-induced central apnea and the effects of antidotes.

Organophosphorus poisoning kills individuals by causing central apnea; however, the underlying cause of death remains unclear. Following findings that the pre-Bötzinger complex impairment alone does not account for central apnea, we analyzed the effect of paraoxon on the brainstem-spinal cord preparation, spanning the lower medulla oblongata to phrenic nucleus. Respiratory bursts were recorded by connecting electrodes to the ventral 4th cervical nerve root of excised brainstem-spinal cord preparations obtained from 6-day-old Sprague-Dawley rats. We observed changes in respiratory bursts when p

Toxic megacolon as a rare complication following atropine therapy due to organophosphate poisoning: A case report.

The main therapeutic basis for a case of organophosphate poisoning is a combination therapy which includes atropine as an anticholinergic drug and pralidoxime. If the poisoning is severe, a high dose of this combination of medicines may be needed, but this may cause serious side effects: paralytic ileus or even megacolon; however, these gastrointestinal events are very rare. Here, we report a case of organophosphate poisoning where atropine therapy was given and led to drug-associated toxic megacolon.

Inhibition of human brain and RBC acetylcholinesterase (AChE) by heptylphysostigmine (HPTL).

Heptylphysostigmine (HPTL), a derivative of the AChE inhibitor physostigmine (PHY), is under investigation as a therapeutic agent in Alzheimer's disease. HPTL is active against human RBC AChE both in vitro and in vivo. Activity of HPTL against human brain has not been documented. We have developed an in vitro assay system using particulate membrane fractions which permits comparison of inhibition and recovery kinetics of human RBC (primarily globular dimer) and brain (primarily globular tetramer) membrane-bound forms. Under these conditions the HPTLIC50 is similar for the two forms. RBC AChE i

Pharmacokinetics of pralidoxime chloride and its correlation to therapeutic efficacy against diisopropyl fluorophosphate intoxication in rats.

A two compartment system is used to study the therapeutic efficacy of pralidoxime chloride (1) along with a suitable pretreatment against diisopropyl fluorophosphate (2) intoxication in rats. The data were analyzed by standard techniques. Though thiamine hydrochloride pretreatment prolonged the biological half-life of 1 but it failed to increase the protective efficacy of 1, on the other hand sodium hydrogen carbonate pretreatment augmented the protective action of 1 without altering its biological half-life appreciably, through increased distribution of 1 into tissue compartment.

Protection by phenytoin and calcium channel blocking agents against the toxicity of diisopropylfluorophosphate.

Pretreatment of male Swiss-Webster mice with phenytoin, 25 mg/kg, verapamil, 25 to 3.0 mg/kg, nifedipine, 0.05 to 0.1 mg/kg, nitrendipine, 0.1 mg/kg, and nimodipine, 1 to 2.5 mg/kg, elevated the LD50 of diisopropylfluorophosphate (DFP) to a significant degree. In addition, these agents enhanced the protection that can be obtained from atropine and 2-pralidoxime. The protective effects of phenytoin cannot be attributed to an anticonvulsant action, per se, since carbamazepine, phenobarbital, and diphenylbarbituric acid in anticonvulsant doses did not influence DFP lethality. Conventional and nonconventional therapies for pesticide-induced cardiotoxicity: A systematic review.

Pesticides, as pervasive environmental pollutants, have been associated with cardiotoxic effects. However, evidence regarding cardioprotective strategies remains fragmented, particularly concerning the integration of conventional therapies and nonconventional approaches involving natural products. This systematic review (PROSPERO:CRD420251143191) aimed to synthesize experimental evidence on cardioprotection against pesticides, with emphasis on natural product-based interventions. The review was conducted according to PRISMA, with searches performed in Medline/PubMed, EMBASE, and BVS. Eligible
