INTRODUCTION: Fasciculoventricular (FV) fiber is a rare cause for ventricular preexcitation. It is usually described as an innocent bystander pathway. There is only limited data on FV fiber in children. Hence we evaluated the clinical and electrophysiological features of FV fiber in a group of 11 children.
METHODS AND RESULTS: Of 215 children with manifest preexcitation who had electrophysiological studies at the University of Miami, 11 (5.1%) had characteristics of FV fiber. FV fiber was not directly responsible for any arrhythmias in these children. Three children had supraventricular tachy

Preexcitation and aberrant conduction both cause a broad QRS complex. An unusual case of an atriofascicular accessory pathway effectively functioning as an ectopic right bundle branch and responsible for both physiologically normal ventricular activation and pathologic preexcited tachycardias as part of a split AV node-bundle branch system is presented.

BACKGROUND: Accessory pathways (APs) with anterograde decremental conduction properties referred to as Mahaim fibers have recently been recognized as originating from the right lateral atrium. Little information is available about their distal insertion. The purpose of this study was to determine the different kinds of APs involved and the characteristics of their distal insertion site.
METHODS AND RESULTS: Twenty-one patients (mean age, 28 +/- 13 years) with reciprocating tachycardia or atrial fibrillation were studied. Right-sided atrial and/or ventricular endocardial mapping during tachyca

There is debate concerning the distal insertion of Mahaim fibers. Some findings favor an atriofascicular fiber connected with the distal right bundle branch. Other findings favor a long atrioventricular (AV) structure inserting into the myocardium. A patient having a decrementally conducting accessory pathway is reported. Proximal and distal Mahaim potentials were recorded during sinus rhythm, atrial pacing, and antidromic tachycardia. Both proximal and distal M potentials always preceded the QRS complex during sinus rhythm and antidromic tachycardia earlier than the right bundle branch potent

INTRODUCTION: Several modalities of catheter ablation have been proposed to eliminate Mahaim pathway conduction. However, limited research has been reported on the electrophysiologic nature of this pathway in its entity.
METHODS AND RESULTS: In seven patients, electrophysiologic study was performed, and radiofrequency energy was applied to investigate the electrophysiologic clues for successful ablation. In all seven patients, the Mahaim pathway was diagnosed as a right-sided atriofascicular or atrioventricular pathway with decremental properties. In two patients, two different kinds of elect

Mahaim fibres are rare, right sided accessory pathways comparable with respect to certain properties (slow, decremential conduction) with "accessory atrioventricular node" located on the lateral tricuspid annulus at a distance from the Aschoff-Tawara node. Atriofascicular and atrioventricular fibres may be distinguished, both responsible for wide complex tachycardia (left bundle branch block pattern with left axis deviation). The authors report a series of 8 patients (6 women, 2 men: age: 27 +/- 11 years) without underlying cardiac disease, incapacitated by episodes of antidromic reciprocating

In some cases undergoing radiofrequency catheter ablation (RFCA) of accessory pathway (AP), a Mahaim-like right-sided atrioventricular AP (M-AP) showing slow and decremental conduction is observed. Among 200 consecutive patients referred to our Institution up to September 1993 for arrhythmias related to an AP and undergoing RFCA, 8 patients (6 males, 2 females; mean age 24 +/- 8 years, range 8-35) showed a M-AP. Seven out of 8 patients have been complaining episodes of palpitation for 13 +/- 7 years (range 1-20), while 1 subject was an asymptomatic young athlete. In 2/8 patients an Ebstein dis

Ebstein's malformation (EM) is characterized by dysplasia and displacement of the tricuspid inferior and septal leaflets from the true atrioventricular (AV) junction. Left ventricular hypertrabeculation/non-compaction (LVHT) including the 'atrialized' portion in EM has not been described. A 42-year-old man with a history of radiofrequency ablation of a Mahaim-like bundle suffered from chest pain. Coronary angiography was normal, but echocardiography showed a septal tricuspid leaflet inserting 3.5-cm apically beyond the AV junction, deep recesses of the atrialized interventricular septum, and a

OBJECTIVES: The aim of this study was to prospectively evaluate the sensitivity, specificity, and positive and negative predictive values of previously described ECG criteria to identify preexcited tachycardia due to decrementally conducting accessory pathways (QRS axis between 0 and -75 degrees , QRS width < or = 0.15 seconds, an R wave in lead I, an rS pattern in lead V(1), RS > 1 QRS transition > V(4), and cycle length between 220 and 450 ms).
BACKGROUND: Preexcited tachycardia associated with decrementally conducting right-sided accessory pathways usually shows a rather "narrow" QRS compl

We present a case of a 34-year-old female with Ebstein's anomaly and symptomatic wide complex tachyarrhythmia. Electrophysiologic evaluation demonstrated antidromic tachycardia utilizing a right-sided decremental, slowly conducting atrioventricular pathway. Distinct accessory pathway potentials along the length of the bypass tract were sequentially recorded to define the anatomic course of the pathway, as uniquely represented on a three-dimensional electroanatomic map, and to successfully guide catheter ablation.

Antidromic atrioventricular re-entrant tachycardia (AVRT) is a rare clinically wide-QRS tachycardia that can often be misdiagnosed as ventricular tachycardia (VT). If the accessory pathway (AP) is slower than the normal atrioventricular (AV) junction, evidence of preexcitation will not appear on the surface electrocardiogram (ECG), which increases the difficulty of diagnosis. This report describes a 13-year-old male patient who was diagnosed with VT at a local hospital at the time of onset of palpitation. ECG revealed wide-QRS-complex tachycardia, and the ECG after the termination of palpitati

BACKGROUND: Recognition of the presence and role of decremental fibers during wide QRS tachycardia requires carefully executed intracardiac studies.
OBJECTIVE: This study sought to determine the value of the atrioventricular (AV) conduction time during pre-excited tachycardia to differentiate a fast from a decrementally conducting accessory pathway (AP).
METHODS: Fifty-one patients with 56 pre-excited tachycardias were included in the study: Group I: 27 patients with 31 antidromic tachycardia (ADT) using an atriofascicular pathway, Group II: 2 patients with pre-excited tachycardia due to bys

A case with total intra-His bundle block and intermittent pre-excitation syndrome is presented. During A-V conduction there was a P-delta interval of 130 msec. with a P-A interval of 20 msec., an A-H interval of 60 msec. and an H-V interval of 50 msec. During rapid atrial pacing the P-delta interval increased primarily due to an A-H1 prolongation and a Mobitz type 2 block and total A-V block occured at increasing rates showing H1 following every A spike. The escape beats showed a normal width of the QRS complexes with preceding H2 spikes. After administration of Ajmaline the bypass tract was b

We report the case of a patient with both a left bundle branch block-preexcited tachycardia using a Mahaim bundle and a tachycardia exhibiting a left bundle branch pattern without preexcited complexes related to an AV nodal reentrant tachycardia. Successful ablation of the nodoventricular tract was obtained in the posteroseptal location in the AV node region. The pattern of Wolff-Parkinson-White (WPW) syndrome and both forms of tachycardia disappeared.

OBJECTIVES: The purpose of the study was to identify the electrocardiographic (ECG) characteristics of the Mahaim fiber.
BACKGROUND: Mahaim fibers are slowly conducting accessory pathways reaching into the right ventricle. They often play a role in tachycardias.
METHODS: We retrospectively analyzed 40 patients with Mahaim fibers. Five patients had associated Wolff-Parkinson-White syndrome and were excluded from the study. Two patients had a short atrioventricular decremental accessory pathway and were also excluded. The remaining 33 patients had a tachycardia with anterograde conduction over

BACKGROUND: Pseudo-Mahaim (AP-M) fibers are a rare variant of atrioventricular (AV) accessory pathways. Atriofascicular and atrioventricular accessory connections are characterized by slow conduction and decremental properties. Dual physiological AV node pathways, slow and fast, are observed in a large number of patients with AP-M. Therefore, there is substrate for AV nodal reentrant tachycardia (AVNRT) in addition to antidromic AV reentrant tachycardia (AVRT) with left bundle branch block (LBBB)-like morphology. Other arrhythmia such as atrial fibrillation (AF) or atrial flutter (AFL) and ven
