The importance of moral sensitivity when including persons with dementia in qualitative research.

The aim of this article is to show the importance of moral sensitivity when including persons with dementia in research. The article presents and discusses ethical challenges encountered when a total of 15 persons with dementia from two nursing homes and seven proxies were included in a qualitative study. The examples show that the ethical challenges may be unpredictable. As researchers, you participate with the informants in their daily life and in the interviews, and it is not possible to plan all that may happen during the research. A procedural proposal to an ethical committee at the begin

An ethics of justice without an ethics of care: Why artificial intelligence ethics guidelines remain unimplementable in community care for older adults.

Governments and international bodies have issued a great many artificial intelligence (AI) ethics guidelines over the past decade, yet evidence that they change how AI is built and deployed remains thin. This paper argues that the implementation gap in AI ethics is not only a shortfall of tools. In caring contexts, it is a consequence of moral form. Mainstream AI ethics operates, in its prevailing form, as an ethics of justice: principlist, calculative, and oriented toward what can be formalised. The claim is not that care is absent from its vocabulary. It is that the principles amenable to te

Ethical Challenges and Considerations in Dysphagia Management: A Scoping Review.

BACKGROUND: Speech and language therapists (SLTs) working in dysphagia care regularly navigate complex ethical dilemmas involving clinical risk, patient autonomy and cultural considerations. While ethical principles are well-recognized in the field, consolidated evidence mapping how these principles and ethical reasoning components have been represented in the literature over time remains limited. This review offers a comprehensive synthesis of ethical challenges in dysphagia management across decades and contexts, uniquely structured using Rest's Four-Component Model of ethical behaviour. ME

The principle of double effect in palliative care: a systematic review of ethical, legal, and clinical perspectives.

BACKGROUND: The Principle of Double Effect (PDE) has historically been used to justify medical interventions intended to relieve suffering, even when such treatments may carry a foreseeable risk of hastening death. In palliative care, the doctrine is particularly relevant to opioid administration and palliative sedation. However, its relevance and practical importance remain debated in contemporary clinical and ethical discourse. OBJECTIVE: This systematic review aims to examine how the Principle of Double Effect is interpreted and applied in palliative care literature, with a focus on ethica

Presymptomatic genetic testing in children for neurofibromatosis 2.

Genetic testing in children, when there is a question of whether or not there is a clear medical benefit that will accrue to the child, is a controversial topic within the health care community. A convenience sample of 10 parents from nine families who had made the decision whether or not to test their children for the neurofibromatosis 2 gene mutation was asked in interviews to describe why they made their choice about presymptomatic testing for this late-onset disease. Findings from a narrative analysis revealed how the nine parents who tested or intended to test their young children saw the

[Conceptualization of Narrative Bioethics in Healthcare].

Ethical dilemmas in real-world medical settings expose the limitations of bioethics, represented by the "four-principle framework," in dynamic and personalized clinical decision-making.The development of narrative medicine provides more space for showcasing the unique context of life, alleviating the predicament of normative ethics. However, current ethical practices in narrative medicine have not responded to the theoretical problem of the lack of ontology, moral subject, and reflective process in principlism, calling for the further exploration of the moral issues inherent in the narrative process

The Ethics of Clinical Research on Diseases of the Fetus and Newborn: Balancing Benefit-Risk, Autonomy, and Maternal-Fetal Interests.

OBJECTIVE: Pregnant persons historically have been excluded from clinical trials. Recently, there has been a shift from exclusion toward inclusion of pregnant persons in research while acknowledging the complexity of reproductive ethics and the intertwined interests of the pregnant person and fetus. Our objective was to use a principle-based approach to review the ethics of clinical research concerning pregnancy-related disorders that predominantly affect fetal well-being. STUDY DESIGN: Ethical principles are applied to the design of interventional trials in two rare conditions of pregnancy,

Acknowledgment of uncertainty: a fundamental means to ensure scientific and ethical validity in clinical research.

Recognition of the importance of uncertainty in the design of randomized, controlled trials (RCT) has reached the status of a principle. The "uncertainty principle," or less ambiguously, equipoise, holds that a patient should be enrolled in an RCT only if there is substantial uncertainty about which of the trial treatments would benefit the patient most. In fact, the "uncertainty principle" addresses the most important issue of a clinical trial--the choice of an adequate comparative control. Studies in which intervention and control group are believed to be non-equivalent violate the uncertain

Beyond Principlism: A Cosmopolitan and Care Ethics Approach to One Digital Health.

This article examines the limitations of principlism in addressing the ethical challenges posed by One Digital Health. While principlism remains influential in bioethics, digital health, and global health policy, we argue that it is insufficient to capture the relational, data infrastructural, and multispecies dynamics shaping the threats to One Health. Using antimicrobial resistance as a paradigmatic case, we show that One Digital Health governance is shaped not only by distributive ethical concerns but also by structural asymmetries in data visibility, digital infrastructure, and epistemic i

The scientific nature of the linear no-threshold (LNT) model used in the system of radiological protection.

During the first half of the 20th century, it was commonly assumed that radiation-induced health effects occur only when the dose exceeds a certain threshold. This idea was discarded for stochastic effects when more knowledge was gained about the mechanisms of radiation-induced cancer. Currently, a key tenet of the international system of radiological protection is the linear no-threshold (LNT) model where the risk of radiation-induced cancer is believed to be directly proportional to the dose received, even at dose levels where the effects cannot be proven directly. The validity of the LNT ap

[Understanding/acceptance of radiotherapy: An ethical dilemma resolved by an ethics of care].

PURPOSE: Ethical questions are poorly investigated specifically in radiation oncology. The objective of the study was to identify and understand the main ethical issue in radiation oncology. MATERIALS AND METHODS: A quantitative analysis was based on the answers to a questionnaire of 200 professionals from 22 radiation oncology departments. The questionnaire mainly aimed to characterize the main ethical issue. A monocentric qualitative analysis was based on semi-structured interviews focused on the main identified ethical issue, carried out with eight technologists, and 20 patients undergoing

Living donation and cosmetic surgery: a double standard in medical ethics?

The commitment of transplant physicians to protect the physical and psychological health of potential donors is fundamental to the process of living donor organ transplantation. It is appropriate that strict regulations to govern an individual's decision to donate have been developed. Some may argue that adherence to such regulations creates a doctor-patient relationship that is rooted in paternalism, which is in drastic contrast with a doctor-patient relationship that is rooted in patients' autonomy, characteristic of most other operative interventions. In this article we analyze the similari

Ethics of managed care in renal transplantation: the principal approach and alignment of incentives create an ethical system.

PURPOSE OF REVIEW: Organ transplantation is a vital medical procedure that saves and extends lives. However, it requires a careful balance of resources, which are often limited by managed care constraints. Renal transplantation is the definitive treatment for patients with end-stage renal disease (ESRD). We share our experience with a managed care organization and how it upheld ethical principles while maintaining solvency and cost-effectiveness. RECENT FINDINGS: Managed care organizations are pervasive healthcare entities and highly regulated businesses. To remain solvent, patients must join

Treatment of anorexia nervosa in Ireland: striking an ethical balance.

Anorexia nervosa (AN) is a complex psychiatric illness with severe and life-threatening medical sequelae, including death. Existing evidence-based treatments are linked to good prognosis and full recovery in many. For a small minority of critically ill patients, treatment decisions extend beyond voluntary engagement. Severe cases may involve involuntary hospitalisation, nasogastric feeding, physical restraint, and other coercive measures. While these interventions are sometimes necessary to prevent death, they raise profound ethical concerns. This article explores the ethical tensions in treat

Ethics in Evaluating a Sociotechnical Intervention With Socially Isolated Older Adults.

The purpose of this article is to consider how ethical principles can inform the effective design and implementation of technology-based interventions that aim to promote the well-being of socially isolated older adults. We evaluated a new iPad application with small groups of older adults. In this article, we reflect on the ethical issues encountered at each stage of the research process. Drawing on the ethical principles of beneficence, research merit and integrity, justice, and respect, we identify key issues to consider in the future design and implementation of social isolation interventi

Ethical Challenges With Smartwatch-Based Screening for Atrial Fibrillation: Putting Users at Risk for Marketing Purposes?

Background: Atrial fibrillation is the most common persistent arrhythmia. It is associated with increased mortality and morbidity such as stroke. The early detection of atrial fibrillation can significantly reduce the risk of stroke through preventive anticoagulation. Smartwatches offer the opportunity to screen for atrial fibrillation in the general population. This paper aims to analyze the ethical challenges associated with screening for atrial fibrillation using smartwatches. Methods: This is an ethical analysis. The methodology is based on the principle-orientated approach of Beauchamp an
