Heart Rhythm Society and Heart Rhythm advocates position statement: Prior authorization in cardiac electrophysiology: Evidence, policy, and reform.

Prior authorization (PA) requirements imposed by Medicare Advantage (MA) and commercial insurers have evolved into a systematic barrier to guideline-indicated cardiac electrophysiology (EP) care, producing measurable patient harm, substantial administrative burden on physicians and allied professionals, and widening health care disparities. This paper examines the financial architecture of the MA capitation model and the structural incentives it creates for denial, characterizes the root causes of the denial system, including third-party denial organizations, artificial intelligence-based algo

A pharmacist-overseen, artificial intelligence-enabled model for provider-side prior authorization: From burden to opportunity (PAVE-1).

Prior authorization (PA) imposes substantial administrative burdens on clinicians, contributing to burnout, delayed care, and excess health care spending. Artificial intelligence (AI) is emerging as a tool to automate PA tasks in managed care pharmacy, yet concerns persist regarding transparency, bias, and overreliance on autonomous systems. This is particularly true in payer-deployed AI systems that may deny claims without adequate clinical review. This viewpoint proposes a pharmacist-overseen, AI-enabled system designed for provider organizations and health systems as an optimal model, integ

Regional Disparities in Qualified Health Plans' Prior Authorization Requirements for HIV Pre-exposure Prophylaxis in the United States.

IMPORTANCE: With the goal of ending the HIV epidemic in the United States, access to HIV pre-exposure prophylaxis (PrEP) is essential to help curb new HIV infections. There has been differential uptake of PrEP by region, with the South lagging behind other regions. Discriminatory benefit design (benefit design that prevents or delays people with complex or expensive conditions from obtaining appropriate treatment) through prior authorization requirements could be a systemic barrier that contributes to the decreased PrEP uptake in the South. OBJECTIVES: To investigate whether there are regiona

Use of a Commercially Available Clinical Decision Support Tool to Expedite Prior Authorization in Partnership With a Private Payer.

PURPOSE: The aim of this study was to determine if a clinical decision support (CDS) tool could be used in partnership with a private payer to successfully expedite the prior authorization process for advanced (ie, MRI, CT, PET, nuclear medicine) imaging requests. METHODS: A single academic institution integrated a commercially available CDS tool utilizing the ACR Appropriateness Criteria into the ordering process for outpatient advanced imaging tests within the electronic health record. Ordering providers could elect to use the CDS tool or ignore the available technology. In partnership with

Buprenorphine treatment episode duration, dosage, and concurrent prescribing of benzodiazepines and opioid analgesics: The effects of Medicaid prior authorization policies.

BACKGROUND: Buprenorphine is an effective medication for the treatment of opioid use disorder (OUD), but the association between prior authorization policies and quality of care for individuals receiving buprenorphine treatment is not well-understood. METHODS: Using 2006-2013 Medicaid Analytic eXtract (MAX) data from 34 states and the District of Columbia, we identified 294,031 episodes of buprenorphine treatment for OUD among individuals aged 14-64 years. We estimated generalized difference-in-differences models to examine the association between buprenorphine prior authorization policies an

Do prior authorization policies discourage first-line antipsychotic use in patients newly discharged from a hospitalization for schizophrenia in Saskatchewan?

BACKGROUND: Drug benefit providers can decrease prescribing of specific medications through prior authorization policies. In Saskatchewan, certain second generation antipsychotics (SGAs) are recognized as first-line agents to manage schizophrenia; but, require prior authorization because their coverage is restricted in other conditions. We aimed to determine if the need for prior-authorization substantially diminishes prescribing of first-line SGAs in comparison to unrestricted agents. OBJECTIVES: To conduct an ecological comparison of SGA prescribing with changes in prior- authorization poli

The effects of prior authorization policies on medicaid-enrolled children's use of antipsychotic medications: evidence from two mid-Atlantic states.

OBJECTIVE: The purpose of this study was to examine the impact of prior authorization policies on the receipt of antipsychotic medication for Medicaid-enrolled children. METHODS: Using de-identified administrative Medicaid data from two large, neighboring, mid-Atlantic states from November 2007 through June 2011, we identified subjects <18 years of age using antipsychotics, from the broader group of children and adolescents receiving behavioral health services or any psychotropic medication. Prior authorization for antipsychotics was required for children in State A <6 years of age from Septe

Assessment of changes in utilization of health-care services after implementation of a prior authorization policy for atypical antipsychotic agents.

BACKGROUND: In 2004, the Georgia Medicaid program implemented a prior authorization (PA) policy for certain atypical antipsychotic agents, resulting in an average savings of $2.7 million per year. OBJECTIVE: To determine whether implementation of a PA policy for atypical antipsychotic drugs increased health-care utilization in the Georgia Medicaid program from July 2003 to April 2006. METHODS: A single cohort observational study employing segmented regression and time series analysis was conducted to determine health-care services utilization, including emergency department (ED) visits, outp

Artificial intelligence in prior authorization and coverage decisions: A systematic review of methods, evidence gaps, and future implications for patient access.

BACKGROUND: Prior authorization (PA) is intended to support appropriate use and spending of services and medications, yet 1 in 6 insured adults report PA-related problems linked to delayed care, worse access, and higher financial burden. As artificial intelligence (AI) expands in utilization management and other pharmacy benefit operations, evidence guiding responsible AI use in PA remains fragmented. OBJECTIVE: To systematically map AI applications across PA workflow and evaluate model development, validation, equity, and implementation outcomes relevant to managed care pharmacy and patient

Predictors of insurance denial with and without prior authorization in patients undergoing spine surgery: a year-long, single-center cohort analysis.

BACKGROUND CONTEXT: Insurance prior authorization (PA), initially intended to curb healthcare costs and wasteful spending, has become a growing regulatory barrier to timely spine surgery. Yet, the process of PA and drivers of coverage denials remain elusive. PURPOSE: We present a one-year review of insurance referrals for spine surgery at a single academic, tertiary care center to identify predictors of PA and ultimately coverage approval and denial. STUDY DESIGN: We retrospectively reviewed 740 adult patients referred for spine surgery between June 2023 and March 2025 at our single academic

A Description of Outcomes Experienced by Patients Whose Orders for Peripheral Artery Revascularization Were Reviewed by a Nondenial Prior Authorization Program.

OBJECTIVE: After a nondenial prior authorization program evaluates orders for peripheral artery revascularization (PAR), ordering physicians sometimes withdraw their orders based upon program recommendations. Some patients with withdrawn orders receive PAR if claudication does not resolve. To characterize patient outcomes under this program, we evaluated whether associations existed between the withdrawal of patients' initial PAR orders and the presence of claims for PAR and claims mentioning intermittent claudication (IC) in the following 16 weeks. METHODS: Orders for PAR placed from 1/1/19

Buprenorphine Use Trends Following Removal of Prior Authorization Policies for the Treatment of Opioid Use Disorder in 2 State Medicaid Programs.

IMPORTANCE: State Medicaid programs have implemented initiatives to expand treatment coverage for opioid use disorder (OUD); however, some Medicaid programs still require prior authorizations (PAs) for filling buprenorphine prescriptions. OBJECTIVE: To evaluate the changes in buprenorphine use for OUD among Medicaid enrollees in states that completely removed buprenorphine PA requirements. DESIGN SETTING AND PARTICIPANTS: This retrospective cross-sectional study analyzed the immediate and trend changes on buprenorphine use during 2013 to 2020 associated with removal of PA requirements using

Description of the outcomes of prior authorization of palivizumab for prevention of respiratory syncytial virus infection in a managed care organization.

BACKGROUND: Respiratory syncytial virus (RSV) is the leading cause of upper and lower respiratory tract infections in infants and young children. Most children are exposed to the virus before they are 2 years old and experience such symptoms as cough, fever, and irritability. In a select population of infants, the virus can cause hypoxemia and hospitalization. To avoid hospitalization, good infection control practices should be employed, and for those infants at high risk, prophylaxis with palivizumab is indicated. Palivizumab has been shown to reduce hospitalization rates in high-risk infants

The Burden of Prior Authorizations in Bleeding Disorder Specialty Drugs.

INTRODUCTION: Prior authorizations (PAs) are widely used to control healthcare costs and ensure appropriate use of high-cost therapies, but they often create undue administrative burdens. In haemophilia care, timely access to specialty medications is critical, yet Haemophilia Treatment Centres (HTCs) must frequently navigate payer-driven PA requirements that can delay treatment and disrupt care delivery. To our knowledge, this study is the first of its kind to examine how the PA process affects patient care and provider workloads in HTCs METHODS: Twenty staff from federally designated HTCs acr

The rates and implications of prior authorizations for advanced shoulder imaging.

BACKGROUND: Shoulder pain is common across all ages and socioeconomic groups. Magnetic resonance imaging (MRI) is vital for diagnosing shoulder pathology, but prior authorizations (PAs) create significant barriers to access. Initially implemented by insurers to control costs, PAs increase administrative burdens on providers, leading to higher costs and delays. While the negative effects of PAs on patient care are documented, their specific impact on shoulder MRI access remains unclear. This study examines trends in PAs for shoulder MRIs and explores associations between PA denials and insuranc

Evaluating time to access opioid treatment for patients with insurance that requires the Safe & Effective Management of Pain prior authorization form vs others.

BACKGROUND: In 2016, the Safe & Effective Management of Pain (SEMP) organization created guidelines regarding how to safely and effectively ensure patients were being treated with opioids. Two insurance companies in West Virginia used the SEMP guidelines to create a prior authorization (PA) form. This form requires extensive information about the patient to be submitted and then reviewed, including those with cancer or being treated with palliative intent. OBJECTIVE: To evaluate the potential delay in accessing opioid prescriptions for patients with cancer or in palliative care who are requir
