[Prevention and treatment of procedural pain in pediatric wards: collected guidelines].

Present knowledge and available pharmacological agents allow for adequate prevention and treatment of pain in children. We present guidelines we prepared for the prevention and treatment of procedural pain in children in our general pediatric ward. This followed extensive review of the literature, participation in scientific meetings, discussions with experts and consultation with interested clinicians. Successful implementation of the guidelines requires increased appreciation of the importance of pain prevention, participation of the nursing, as well as medical staff, and ability to evaluate

Vocal responses of piglets to castration: identifying procedural sources of pain.

The aims of this study were to identify which aspects of castration are painful to piglets, and to determine if less painful methods are available. Previous work has shown that piglets produce more high frequency calls (>1kHz) when castrated than when handled identically but not castrated, or when castrated using a local anesthetic. In Experiment 1, we used 90 piglets to assess the vocal responses to different components of the castration procedure: restraint, washing the ano-genital region, scrotal incision, and pulling/severing of the spermatic cords. Compared to sham-operated animals (which

Procedural Pain Scale Evaluation (PROPoSE) study: protocol for an evaluation of the psychometric properties of behavioural pain scales for the assessment of procedural pain in infants and children aged 6-42 months.

INTRODUCTION: Infants and children are frequently exposed to painful medical procedures such as immunisation, blood sampling and intravenous access. Over 40 scales for pain assessment are available, many designed for neonatal or postoperative pain. What is not well understood is how well these scales perform when used to assess procedural pain in infants and children. AIM: The aim of this study was to test the psychometric and practical properties of the Face, Legs, Activity, Cry and Consolability (FLACC) scale, the Modified Behavioural Pain Scale (MBPS) and the Visual Analogue Scale (VAS) ob

[Neuroablative procedures in pain therapy].

Although surgical ablative procedures can be effective in the management of chronic pain of malignant and non-malignant origin, they are often disregarded as treatment options due to the fact that in the past these procedures were associated with high complication rates. The complications include the development of new neurological deficits and in cases of long-term follow-up, the occurrence of the old or new pain syndromes by deafferentation. On the other hand there exist many less invasive, e.g. neuromodulatory procedures or non-invasive measures (systemic oral or transdermal opioids) which

Alternatives to sedation for painful procedures.

Conscious sedation is used frequently to perform procedures that may be undertaken with or without minimal sedation. Fracture manipulation can be performed with minimal discomfort in the awake patient using various techniques-for example, intravenous regional anesthesia, nerve blocks, and hematoma blocks. These have been used for many years and are very safe. They may require some specific equipment (eg, automatic tourniquet) to perform the anesthesia but patients have the same results and are able to go home much quicker because sedation is not used.Use of topical anesthesia for both intact s

Radiofrequency procedures in chronic pain.

Radiofrequency current is simply a tool used for creating discrete thermal lesions in neural pathways in order to interrupt transmission. In pain medicine, radiofrequency lesions have been used to interrupt nociceptive pathways at various sites. This is a palliative treatment not without complications, so its use should be limited to those patients with cancer pain or chronic non-cancer pain for whom conservative non-surgical therapies have been ineffective or intolerable. With the development of alternatives such as intrathecal opioid infusion and neuromodulation technologies, the number of p

[Psychotherapy procedures in chronic pain].

Psychotherapeutic possibilities of treating chronic pain are employed in two different ways: Firstly in the form of psychotherapy in patients whose strategies for coping with pain and/or pain genesis can be described in terms of psychopathological categories; secondly in the form of pain-coping training, which is generally indicated in the presence of chronic pain, in particular that which is traditionally termed organic. With respect to their effectiveness in achieving, in the first place, mood change, and in the assessment of symptoms, the individual procedures have been well tested. The que

Interventional procedures for facial pain.

Interventional pain procedures are critical in the diagnosis and management of a variety of facial pain conditions. Trigeminal neuralgia (TN) is the most frequent diagnosis for facial pain, with a reported prevalence 10 times greater than persistent idiopathic facial pain (PIFP). Although pharmacological treatments and psychological interventions benefit many patients with these diagnoses, the pain remains disabling for a significant portion of others. Percutaneous interventions targeting the gasserian ganglion and its branches have proven effective in the management of TN, while there is also

[Diagnostic procedures for painful foot].

The diagnostic process in case of painful foot must always begin with a first clinical phase that is the most important. The history, conditions of happening and the clinical examination are essentials to orient appropriately the secondary examinations, if necessary, and establish a proper diagnosis. After the taking of the history, the clinical examination of the foot, bilateral and comparative, should begin with an evaluation of the entire musculoskeletal system for systemic and general conditions which can affect the foot and ankle. The function and structure of the foot may be evaluated us
