Neurological Complications Following Arthroscopic and Related Sports Surgery: Prevention, Work-up, and Treatment.

Arthroscopy of the shoulder, elbow, hip, and knee has become increasingly utilized due to continued advancements in technique, training, and instrumentation. In addition, arthroscopy is generally safe and effective in the utilization of joint preservation surgical techniques. The arthroscopist must utilize a thorough understanding of the surgical anatomy, detailed care with patient positioning, and safe instrumentation portals to prevent associated neurological injury. In the event of postoperative neurological complications, the physician must carefully document the patient history and physic

An Empirical Study on the Operative Treatment of Symptomatic Urolithiasis in Germany.

PURPOSE: The guidelines of the German, European, and American Urological Associations on urolithiasis advise against general ureteral stenting before and after an uncomplicated ureterorenoscopy (URS). However, German and European guidelines state that stenting prior to URS facilitates stone extraction and reduces intraoperative complications. According to the published literature, German practice seems to deviate from recommendations. This nationwide survey aimed to evaluate the treatment modalities of urolithiasis. METHODS: In November 2018 and March 2019, a total of 199 urological hospital

A roadmap for evaluating the use and value of durable ventricular assist device therapy.

Heart failure is a disease characterized by profound human suffering with limitations in survival despite treatment with guideline-directed medical therapies. Patients with heart failure frequently progress to advanced stages and often require cardiac transplantation or implantation of left ventricular assist devices (LVADs) to extend survival and improve quality of life. As the number of suitable heart donors, number of experienced medical centers and patient comorbidities place restrictions on the feasibility of cardiac transplantation, implantation of LVADs has emerged as a more frequently

Review of craniofacial pain syndromes involving the greater occipital nerve: relevant anatomy, clinical findings, and interventional management.

Craniofacial pain syndromes exhibit a high prevalence in the general population, with a subset of patients developing chronic pain that significantly impacts their quality of life and results in substantial disabilities. Anatomical and functional assessments of the greater occipital nerve (GON) have unveiled its implication in numerous craniofacial pain syndromes, notably through the trigeminal-cervical convergence complex. The pathophysiological involvement of the greater occipital nerve in craniofacial pain syndromes, coupled with its accessibility, designates it as the primary target for va

Endohepatology: Current perspectives and future directions.

Endoscopic procedures have been integral to the management of patients with liver disease, primarily focusing on luminal interventions such as variceal band ligation. Since the introduction of endoscopic ultrasound, the breadth of procedures has rapidly expanded, leading to the emergence of a new field known as endoscopic hepatology or "endohepatology." This domain integrates endoscopic diagnostic and therapeutic techniques, particularly those which are endoscopic ultrasound-based, into hepatology practice. The comprehensive nature of endohepatology has revolutionized patient care by facilitat

The Difference in Surgical Site Infection Rates Between Open and Minimally Invasive Spine Surgery for Degenerative Lumbar Pathology: A Retrospective Single Center Experience of 1442 Cases.

BACKGROUND: Surgical site infection (SSI) in spinal surgery contributes to significant morbidity and healthcare resource utilization. Few studies have directly compared the rate of minimally invasive surgery (MIS) SSI with open surgery. OBJECTIVE: To investigate whether MIS techniques had a lower SSI rate in degenerative lumbar procedures as compared with traditional open techniques. METHODS: A single-center, retrospective review of a prospectively collected database was queried from January 2013 to 2016 for adult patients who underwent lumbar decompression and/or instrumented fusion for whi

Interval gangrene complicating superficial femoral artery stent placement.

Interval gangrene-necrosis of tissue proximal to a successful distal revascularization procedure-is an exceeding rare complication. To date, only nine cases have been reported in the literature, and all were secondary to traditional open bypass procedures. We report the first case, to our knowledge, of interval gangrene after endovascular stent placement in the superficial femoral artery. We believe that with the increasing utilization of endovascular techniques to treat limb ischemia, the serious complication of interval gangrene must be revisited. Assessment of collateral circulation, precis

Trends in Medicare procedural and reimbursement rates for spinal CSF leak repair (2000-2021).

OBJECTIVE: CSF leaks are a common complication of spinal surgery, occurring in 3%-16% of elective cases and up to 12% of operatively managed traumatic spinal injuries. They can also occur following lumbar puncture. However, there are limited data on utilization and reimbursement trends within Medicare for CSF leak repairs. Characterizing the economic burden and market of spinal CSF leak repair has implications for optimizing effective management strategies. This study evaluates reimbursement and utilization trends for both surgical repair and epidural blood patch (EBP) management of spinal CSF

Unique Clinical Aspects of Nasal Scarring.

Various methods are available for refining scars of the external nose and optimal scar revision frequently requires the utilization of multiple techniques. Differing anatomy of nasal subunits and their underlying structural framework limit surgical options in nasal scar revision compared with other areas of the face. An understanding of a variety of laser technologies and their specific applications can vastly aid in fine, controlled scar revision. Achieving optimal scar reduction regularly requires multiple stages of intervention, close follow-up, and repeat procedures.

The associations between serum biomarkers and stenosis of the coronary arteries.

BACKGROUND: In young and active patients, long head of biceps (LHB) tenodesis has become a common procedure for managing LHB pathology, but it remains unclear whether it is performed in isolation or along with other shoulder procedures and whether open and arthroscopic techniques produce different complications. QUESTIONS/PURPOSES: We sought to determine and compare open and arthroscopic LHB tenodesis in terms of (a) trends in overall use, (b) trends in use in isolation and in association with rotator cuff repair (RCR) and superior labral tear from anterior-to-posterior (SLAP) debridement/rep

Transseptal left heart catheterization and the incidence of persistent iatrogenic atrial septal defects.

INTRODUCTION: Access to the left atrium for invasive and interventional cardiac procedures requires a transseptal (TS) puncture that creates an iatrogenic atrial septal defect (iASD). The utilization of TS access is increasing in cardiology, and the frequency of iASD is, therefore, likely to increase as well. Here, we discuss the TS technique, review existing series of iASD with regard to incidence and clinical significance of residual iASD, and present emerging techniques utilizing TS access where iASD may result. METHODS AND RESULTS: A PubMed search for "iatrogenic atrial septal defects" wa
