Group based prenatal care in a low-and high risk population in the Netherlands: a study protocol for a stepped wedge cluster randomized controlled trial.

BACKGROUND: CenteringPregnancy (CP) is a multifaceted group based care-model integrated in routine prenatal care, combining health assessment, education, and support. CP has shown some positive results on perinatal outcomes. However, the effects are less obvious when limited to the results of randomized controlled trials: as there are few trials and there is a variation in reported outcomes. Furthermore, former research was mostly conducted in the United States of America and in specific (often high risk) populations. Our study aims to evaluate the effects of CP in the Netherlands in a general

Review of methods for measuring and comparing center performance after organ transplantation.

The assessment of outcomes after transplantation is important for several reasons: it provides patients with data so that they can make informed decisions about the benefits of transplantation and the success of the transplant unit; it informs commissioners that resources are allocated properly; and it provides clinicians reassurance that results are acceptable or, if they are not, provides early warning so that problems can be identified, corrections can be instituted early, and all interested parties can be reassured that scarce resources are used fairly. The need for greater transparency in

A hybrid process fidelity assessment in a home-based randomized clinical trial.

A process fidelity assessment was conducted as a nested study within a home-based randomized clinical trial teaching self-management to 101 long-term indwelling urinary catheter users in the treatment group. Our hybrid model combined external assessments (outside observations and tape recordings) with internal evaluation methods (through study nurse forms and notes) for a comprehensive process fidelity assessment. Barriers, patient-related issues, and nurse perspectives were identified demonstrating the complexity in home care intervention research. The complementary and synergistic approaches

Developing an integrated evaluation indicator system for non-communicable disease management in China's primary care: a modified Delphi-analytic hierarchy process study based on the structure-process-outcome framework.

BACKGROUND: The aging population and increasing prevalence of chronic conditions have led to higher healthcare costs and greater resource utilization globally, exerting considerable pressure on healthcare system sustainability. Integrating services in primary healthcare can improve access and effectiveness for chronic disease management. However, China’s primary healthcare institutions have not achieved full integration of essential clinical services and public health services, hindering the continuity of care and chronic disease management. Guided by the Structure- Process- Outcome framework,

Quality measures for children's health care.

BACKGROUND: The ability to measure and improve the quality of children's health care is of national importance. Despite the existence of numerous health care quality measures, the collective ability of measures to assess children's health care quality is unclear. A review of existing health care quality measures for children is timely for both assessing the current state of quality measures for children and identifying areas requiring additional research and development. OBJECTIVES: To identify and collect current health care quality measures for child health and then to systematically catego

[The effect of a health promotion program at the worksite--an approach to hyperlipoproteinemia in workers before middle age].

A one-year educational program at the worksite was designed to reassess individual physical activities and acquire knowledge of hyperlipoproteinemia in workers aged 30, 35 and 40 who had increased serum cholesterol (CHO) or triglyceride (TG). A total of 420 subjects were selected in 8 yr. The mean percentage of attendance for the program consisting of measuring maximum oxygen uptake, a lecture on a healthy life, an eating-experience class and measuring serum lipid was 86, 59, 35 and 61%, respectively. The male subjects (214 subjects for 1 yr and 125 subjects for 5 yr) who had measured maximum

Development of an evaluation indicator system for wound care quality based on the structure-process-outcome model: a mixed-methods study.

BACKGROUND: As population aging accelerates and the burden of chronic diseases increases, the incidence of chronic wounds such as diabetic foot ulcers and pressure injuries continues to rise. This trend not only worsens patient outcomes and quality of life but also strains the healthcare system. Traditional wound care assessments rely on isolated indicators, lack systematic monitoring, and fail to provide comprehensive evaluation throughout the care process, thereby limiting improvements in nursing quality. Based on Donabedian's structure-process-outcome framework, a thorough evaluation system

A systematic review and meta-analysis comparing open versus endoscopic in situ decompression for the treatment of cubital tunnel syndrome.

To examine whether endoscopic in situ decompression (EISD) or open in situ decompression (OISD) would have superior outcomes with lower morbidity in patients with idiopathic cubital tunnel syndrome, we reviewed all studies compared both surgical techniques with regard to postoperative outcomes and complication profile in a systematic review design with meta-analysis. Two independent reviewers conducted a PRISMA-compliant search of PubMed, EMBASE, and the Cochrane Library databases for relevant studies about clinical comparisons of OISD and EISD in cubital tunnel syndrome. We performed all meta

OBJECTIVE: To compare the outcome of per oral endoscopic myotomy (POEM) and laparoscopic Heller myotomy (LHM) for the treatment of esophageal achalasia. BACKGROUND: Over the last 2 decades, LHM has become the primary form of treatment in many centers. However, since the first description of POEM in 2010, this technique has widely disseminated, despite the absence of long-term results and randomized trials. METHODS: A systematic Medline literature search of articles on LHM and POEM for the treatment of achalasia was performed. The main outcomes measured were improvement of dysphagia and postt

A critique of project evaluations.

In recent years an increased stress has been placed on the evaluation of mental health, education, and welfare service programs. The majority of studies readily available to most evaluators represent local project evaluations which usually contain diverse references to different aspects of the evaluative process. For evaluative results to be even minimally useful to other projects, however, certain requirements must be met. These are: (1) internal validity, (2) external validity, (3) specification of the population and treatment being implemented, and (4) standardization of indicators of treat

Dismantling cognitive-behaviour therapy for panic disorder: a systematic review and component network meta-analysis.

Cognitive-behaviour therapy (CBT) for panic disorder may consist of different combinations of several therapeutic components such as relaxation, breathing retraining, cognitive restructuring, interoceptive exposure and/or in vivo exposure. It is therefore important both theoretically and clinically to examine whether specific components of CBT or their combinations are superior to others in the treatment of panic disorder. Component network meta-analysis (NMA) is an extension of standard NMA that can be used to disentangle the treatment effects of different components included in composite int

Family therapy for eating disorders in youth: current confusions, advances, and new directions.

PURPOSE OF REVIEW: Family interventions for eating disorders are often recommended for the treatment of children and adolescents. Treatment studies and a range of treatment guidelines now recommend family-based treatment (FBT) for adolescents with anorexia nervosa (AN) and bulimia nervosa. The current report focused on studies that have been conducted over the past 2 years, most of them aimed at augmenting or improving outcomes using a range of new family approaches or adding other forms of therapy to standard FBT. RECENT FINDINGS: There is increasing confusion of what type of family therapy

Minimally invasive approaches to craniosynostosis.

Craniosynostosis (CS) is defined as the premature fusion of one or more calvarial sutures. This carries several consequences, including abnormal/asymmetric cranial vault development, increased intracranial pressure, compromised neurocognitive development, and craniofacial deformity. Definitive management is surgical with the goal of protecting cerebral development by re-establishing normal cranial vault expansion and correcting cosmetic deformity. In today's practice, CS surgery has advanced radically from simple craniectomies to major cranial vault reconstructive (CVR) procedures. More recent
