Views on consent approaches used in emergency and critical care research: a rapid, systematic review.

BACKGROUND: Obtaining informed consent can be challenging in emergency and critical care research due to the acute and severe nature of the patient's condition. However, such research is urgently needed to inform practice and optimise patient outcomes. While alternative consent approaches have been commonly used, opinions may vary, particularly among diverse and underserved patient groups and in the context of the recent COVID-19 pandemic. The objective of this review was to assess views of alternative consent methods in emergency and critical care research. METHODS: We conducted a rapid syst

The Anaesthetic Mortality Assessment Committee 1979-1984.

A review of 324 cases reported to the Anaesthetic Mortality Assessment Committee over a five year period confirms a low incidence of cases associated primarily with anaesthesia. The committee has identified potential problem areas affecting anaesthesia which require continued vigilance on the part of the anaesthetist. These include the fluid balance of patients, the potential for hypothermia, altered patterns of drug action in the elderly and critically ill and the influence of other disease processes-notably those affecting the heart. As well there are the rare problems more directly associat

Improving the Detection, Assessment, Management and Prevention of Delirium in Hospices (the DAMPen-D study): protocol for a co-design and feasibility study of a flexible and scalable implementation strategy to deliver guideline-adherent delirium care.

INTRODUCTION: Delirium is a complex condition in which altered mental state and cognition causes severe distress and poor clinical outcomes for patients and families, anxiety and stress for the health professionals and support staff providing care, and higher care costs. Hospice patients are at high risk of developing delirium, but there is significant variation in care delivery. The primary objective of this study is to demonstrate the feasibility of an implementation strategy (designed to help deliver good practice delirium guidelines), participant recruitment and data collection. METHODS A

Barriers to and Facilitators of Implementing Overnight Nursing Teleconsultation in Small, Rural Long-Term Care Facilities: Qualitative Interview Study.

BACKGROUND: Teleconsultation has expanded rapidly in recent years, especially during the COVID-19 pandemic, and has become standard practice among physicians. The benefits of teleconsultation, namely, improving access to care, ensuring continuity and quality of care, increasing patient satisfaction, and reducing costs and wait times, are well documented. However, its use in nursing practice, especially in long-term care settings, remains underresearched despite its significant transformative potential, particularly in resource-limited and rural settings, where it could address major challenges

[Abortion after the 12th completed pregnancy week. Report of experience of a specialist commission].

In the GDR abruptio after 12th completed week of gestation is only allowed in cases of severe threat of health or danger for life. The decision is made by a group of specialists. Analysing the years 1972-1986 all the 135 cases worked up by the commission of the district hospital of Gera were evaluated. Reasons for delayed diagnosis of pregnancy and arguments for wanted abruptio were most interesting. To determine possible trends, the results were subdivided in two periods, 1972-1979 and 1980-1986. In the second period an unfavourable development was detected regarding number of cases, distribu

The data and safety monitoring board and acquired immune deficiency syndrome (AIDS) clinical trials.

The urgency of the Acquired immune deficiency syndrome (AIDS) epidemic has mandated that multiple therapeutic approaches be developed and that these approaches be evaluated through clinical trials. To oversee these trials, the National Institute of Allergy and Infectious Diseases (NIAID) has created three large clinical trial programs monitored by a Data and Safety Monitoring Board (DSMB). For each clinical trial, this Board uses a standardized approach employing contemporary biostatistical, medical, and ethical principles. The DSMB is responsible for reviewing interim data on clinical trial p

Developing a hospital emergency preparedness program.

The emergency preparedness program is not a static entity that only needs to be taken off the shelf and looked at once or twice a year. The various components of the program require that it be a daily part of the hospital's routine. Only by becoming a part of the everyday operation of the hospital can the full benefits of an emergency preparedness program be realized in an actual disaster. A part of the recovery activities after a hospital experiences an actual disaster should be to examine all phases of the emergency preparedness program. The program should be viewed as if it were being desig

Who's the boss? Ethical conflicts in home care.

In 1987 a committee of the Home Health Assembly of New Jersey began a study of ethical issues in home care situations where the autonomy of the elderly patient is at risk. The study was based on four case histories describing actual situations of elderly patients, their families, and the professional practice of nursing and social work in the home. To determine concerns of home health agency administrators and their staffs, the committee did surveys, held round-table discussions, and conducted telephone interviews. As a result, it identified four key ethical concerns: 1. Maintaining agency sol

Assessment of oral mucositis in adult and pediatric oncology patients: an evidence-based approach.

Oral mucositis is a frequent side effect of cancer treatment and can lead to delayed treatment, reduced treatment dosage, altered nutrition, dehydration, infections, xerostomia, pain, and higher healthcare costs. Mucositis is defined as "inflammatory lesions of the oral and/or gastrointestinal tract caused by high-dose cancer therapies. Alimentary tract mucositis refers to the expression of mucosal injury across the continuum of oral and gastrointestinal mucosa, from the mouth to the anus" (Peterson, Bensadoun, & Roila, 2008, p. ii122). Evidence demonstrates that oral mucositis is quite distre

[Coordination and Role-Sharing between Health Care Professionals].

In recent years, people's demands for medical services are increasing and, simultaneously, the overwork of health professionals has been pointed out. It is important to establish a system with which each medical professional can show higher levels of expertise and work as a team member to provide efficient and safe medical care. For this reason, a list of medical activities which can be performed by not only doctors but also other health professionals was devised by the Director General of the Health Policy Bureau on December 28th, 2007, as the "Promotion of role-sharing between doctors and ot
