[The prone position in ARDS. A successful therapeutic strategy].

As early as 1974, Brian advocated the prone position for ventilated patients. He suggested that this position might enhance ventilation of the dorsal parts of the lungs, thereby improving oxygenation. These considerations have been confirmed by several experimental and clinical studies. Better secretion removal, decreased intrapulmonary shunting, and an increased FRC are thought to be responsible for the observed improvement of oxygenation. However, the prone position never became very popular in the clinical treatment of the adult respiratory distress syndrome (ARDS). Routine performance of t

Spinal Anesthesia in the Prone Position: A Case Series.

Background Spinal anesthesia (SA) is a safe and effective strategy for many procedures, providing several advantages over general anesthesia. SA is usually performed in a sitting or lateral position, followed by careful repositioning to achieve the final surgical position. This mobilization process may cause technical difficulties related to the patient, consume valuable time, and lead to an unpredictable spread of the local anesthetic agent. To overcome these challenges, some authors have successfully performed SA with the patient already in the prone position. Methods In this article, we pre

Prone position in COVID 19-associated acute respiratory failure.

PURPOSE OF REVIEW: Prone position has been widely used in the COVID-19 pandemic, with an extension of its use in patients with spontaneous breathing ('awake prone'). We herein propose a review of the current literature on prone position in mechanical ventilation and while spontaneous breathing in patients with COVID-19 pneumonia or COVID-19 ARDS. RECENT FINDINGS: A literature search retrieved 70 studies separating whether patient was intubated (24 studies) or nonintubated (46 studies). The outcomes analyzed were intubation rate, mortality and respiratory response to prone. In nonintubated pat

[ECG pearl: prone position ECG for COVID-19 patients].

Prolonged prone position ventilation reduces the 30-day mortality in acute respiratory distress syndrome (ARDS) and probably in COVID-19 infection, too. Although the respiratory illness is the most important clinical manifestation of COVID-19, a large number of patients with COVID-19 suffer from new onset cardiac dysfunction where ECG and ECG monitoring play a critical role. However, the effects of prone position on the ECG is unknown. A healthy, 30-year-old man is presented with the change of ECG mimicking old myocardial infarction in the V1-3 leads after prone position. This could help us to

Prone Versus Supine Percutaneous Nephrolithotomy: What Is Your Position?

PURPOSE OF REVIEW: Percutaneous nephrolithotomy (PCNL) is the gold standard surgical procedure for treating large, complex renal stones. Due to its challenging nature, PCNL has undergone many modifications in surgical technique, instruments, and also in patient positioning. Since the first inception of PCNL, prone position has been traditionally used. However, alternative positions have been proposed and assessed over the years. This is a comprehensive review on the latest developments related to positioning in the practice of PCNL. RECENT FINDINGS: The prone position and its modifications ar

[Compression of facial skin by headrest in prone position].

The higher body pressures of patients compared to their capillary blood pressures in the limited areas of their skin induced peripheral blood flow disturbances, thus causing skin injuries. Use of horseshoe-shaped headrest for prone-positioning during surgeries can induce blood flow occlusion, skin ischemia, and skin injury due to high pressure and shear stress on the forehead and both cheeks, which are protrusions on facial bone. The use of pressure dispersion headrests is preferred, redistributing the body pressure equally and absorbing pressure from the entire head in the prone position for

Prone position and reduced thoracoabdominal asynchrony in preterm newborns.

OBJECTIVE: To assess the effect of prone and supine positions on breathing pattern variables, thoracoabdominal motion and peripheral oxygen saturation of hemoglobin of premature newborn infants recovering from respiratory distress syndrome, while breathing spontaneously and in rapid eye movement sleep. METHODS: This was a quasi-experimental study. Twelve preterms weighing > 1,000 g at enrollment were studied in both positions, in random order. Respiratory inductive plethysmography was used to analyze breathing pattern (tidal volume, respiratory rate, minute ventilation, mean inspiratory flow)

Lateral medullary syndrome after prone position for general surgery.

We describe postoperative lateral medullary syndrome with myoclonic spasm. Improper head rotation during positioning in the anesthetized patient might obstruct the flow of the vertebral artery at the neck and result in diminished perfusion of the associated regions in patients with a history of cervical spinal trauma.

[Design of a head auxiliary support device in prone position].

Prone position ventilation (PPV) is an important protective strategy for lung ventilation, widely used in clinical practice, especially since the novel coronavirus infection pandemic. Since PPV is a non-physiological position, improper implementation and management can lead to serious adverse events such as pressure injury, facial edema, unplanned extubation and (or) reintubation, and even asphyxia. At present, preventive and protective strategies are mainly used to manage PPV-related complications in clinical practice. These strategies not only increase the workload of medical staff and the u

Respiratory motion tracking of spine stereotactic radiotherapy in prone position.

PURPOSE: The CyberKnife system is a specialized device for non-coplanar irradiation; however, it possesses the geometric restriction that the beam cannot be irradiated from under the treatment couch. Prone positioning is expected to reduce the dose to normal lung tissue in spinal stereotactic body radiotherapy (SBRT) owing to the efficiency of beam arrangement; however, respiratory motion occurs. Therefore, the Xsight spine prone tracking (XSPT) system is used to reduce the effects of respiratory motion. The purpose of this study was to evaluate the motion-tracking error of the spine in the pr

Prolonged prone position in pregnant woman with COVID-19 pneumonia.

The manuscript describes a case report of 2 prolonged prone position cycles (72 h each) of a coronavirus disease 2019 pneumonia in an intubated pregnant woman (at 22 weeks of gestational age), being successfully discharged from intensive care unit after 20 days. There were no signs of fetal sufferance at daily obstetric monitoring during prone position, and the fetus was born fully vital and without consequences. At our knowledge, this is the first case of prolonged prone position in a pregnant woman, and we feel that our manuscript could be a valuable contribution to the literature and help in

[Post operative visual loss after cervical laminectomy in prone position].

Postoperative visual loss is a rare and devastating complication. The estimated incidence is 0.01-1% after non ocular surgery. It has been reported after spine, cardiac and head and neck surgeries. We report a patient who was operated for cervical laminectomy in prone position and complained of loss of vision in one eye postoperatively. He was thoroughly investigated after visual loss. The case was diagnosed as central retinal artery occlusion (CRAO) of the left eye. Here we consider the potential etiological factors causing this unilateral loss of vision and try to suggest strategies to reduc
