OBJECTIVES: To present a case of biopsy proven acanthamoeba keratitis requiring penetrating keratoplasty in a patient with keratoconus whose clinical course was remarkable for its lack of ocular injection and pain. The absence of these key findings may have contributed to a delay in diagnosis and a delay in instituting antiamoebic therapy. METHODS: Case report. RESULTS: A 21-year-old woman who wore soft contact lenses for management of keratoconus presented with a painful suppurative corneal infiltrate and epithelial defect. The patient was initially seen in an emergency department where she

The effect of corneal hypesthesia on the duration of proparacaine anesthetic eyedrops.

The duration of action of proparacaine is known in the normal cornea but not in the hypesthetic cornea. To determine this, we examined both eyes in seven patients with documented unilateral corneal hypesthesia associated with inactive herpetic disease. Cochet-Bonnet measurements were made in both eyes before and at two- to five-minute intervals after the instillation of one drop of 0.5% proparacaine until baseline corneal sensitivity levels were again achieved. Mean recovery time was 34.86 minutes in eyes with normal corneal sensitivity, compared to 45.43 minutes in hypesthetic corneas. In all

Topical anesthesia of the eye as a diagnostic test.

We investigated whether ocular response to topical anesthetic could assist the physician in distinguishing between patients with simple corneal injury and those with iritis, conjunctivitis, or other causes of acute eye pain. Seventy-one patients who presented to the emergency department filled out visual analog pain scores (range, 0 to 10) before and after use of topical anesthetic (proparacaine 0.5%). Mean initial pain scores were higher (6.3 +/- 3.0 vs 4.8 +/- 2.8, P less than .05), final pain scores were lower (1.1 +/- 1.7 vs 3.5 +/- 2.5, P less than .001), and changes in scores were greate

Effect of selective topical anaesthesia on the initial comfort of rigid contact lenses.

PURPOSE: The primary sensory location of contact lens discomfort remains unclear. This study utilised selective topical anaesthesia of the ocular surface to investigate if the relative contribution of different tissues to lens-related discomfort could be determined. METHODS: This was a randomised, subject-masked, contralateral eye pilot study involving 10 participants. Mechanical sensitivity was measured at three locations on the ocular surface (cornea, upper and lower eyelid marginal conjunctiva) using a Cochet-Bonnet aesthesiometer. Following sensitivity measurements, proxymetacaine hydroch
