BACKGROUND: Inferior scapular notching is a common radiographic complication of reverse shoulder arthroplasty. The purpose of this study is to determine the impact of prosthesis design on the incidence and severity of notching.
MATERIALS AND METHODS: Eighty-eight patients (mean age, 72 years) who underwent reverse shoulder arthroplasty with a minimum of 12 months' follow-up (mean, 31 months) were retrospectively reviewed. Patients were grouped based on prosthesis design: Tornier (Stafford, TX, USA) (45%), Zimmer (Warsaw, IN, USA) with a nonretentive liner (35%), and Zimmer with a retentive li

The palatal lift prosthesis is used to alleviate rhinopharyngeal closure dysfunction. It is generally problematical to use in edentulous patients, because the palatal lift prosthesis requires secure retention of the denture base and is difficult for the patient to retain while eating. This article describes the fabrication of a complete denture with a removable palatal lift prosthesis and a clinical evaluation of the denture's stability. A sprue pin and tube were used as a connecting attachment between the removable palatal lift prosthesis and the denture base. The force required for lifting t

The prosthetic treatment of patients with total maxillectomies is an enormous reconstruction challenge because of insufficient support and retention from the residual tissues. It is not possible to place the bulb in the presence of unfavorable undercuts throughout the nasal cavity floor and borders. The purpose of this article was to describe a prosthetic reconstruction technique which accomplishes optimum sealing, retention, stabilization, and easy placement of an open-hollow acrylic resin obturator using a spring with a resilient liner for a patient who underwent a total maxillectomy. It was a great challenge for the prosthodontist to rehabilitate and sustain the prosthesis in patients with bilateral maxillectomy defects due to mucormycosis seen with Covid-19 during second wave in India. In such extensive defects there was difficulty in retaining the obturator due to the absence of soft tissue or anatomical undercuts, condition of existing dentition, retained inferior turbinates and limitation in taking retention from defect side during healing phase. In such extensive maxillectomy defects, retention, stability and support can be enhanced by maximum preservation of hard and

BACKGROUND: Periprosthetic joint infection (PJI) is a challenging complication of arthroplasty. When curative treatment is not feasible, palliative alternatives may be considered. The aim of this study was to evaluate the outcomes of guided persistent fistula (GPF) as a salvage procedure for chronic PJI, focusing on treatment failure, prosthesis retention, and functional preservation.
METHODS: A retrospective observational study was conducted across five French regional referral centres. Adult patients with chronic PJI treated with GPF as a palliative measure were included. The primary outcom

Maxillectomy performed in acquired lesions is often very extensive or bilateral, sparing very less hard and soft tissue in the oral cavity. These defects need both surgical and prosthetic rehabilitation. A definitive prosthesis is inserted after complete healing, when the patient is accustomed to the interim prosthesis. Definitive obturator warrants better retention and stability and needs a meticulous planning in design for long-term use. Conventional design has a few disadvantages; it needs modification many times to improve retention and psychological comfort of the patient. The article exp

This report presents the use of a dental implant with an anti-rotational attachment for the retention of a thumb prosthesis. A retention system was manufactured with an attachment (UCLA) screwed into the implant with a two-bar system that was cast in metallic silver palladium. A substructure made from heat-cured acrylic resin was joined to the retention system by clips to join the thumb to the finger (bar clip) in the cast with implant rejoinder. The silicone material, Silastic-MDX 44210, was used to achieve function and aesthetics. Following osseointegration, no skin problems were observed. W

PATIENTS: This report describes the digital workflow for the design and fabrication of a telescopic prosthesis that incorporates a polyetherketoneketone (PEKK) primary crown and PEKK framework to restore multiple teeth in the mandible of a 56-year-old female patient. The process includes intraoral scanning, computer-aided design, digital milling of the telescopic crown and PEKK framework, and 3D printing of the casts. Good retention, mastication, aesthetics, and biological outcomes are observed in the four-year follow-up.
DISCUSSION: Two clinical outcomes are observed. (1) A PEKK telescopic p

By preserving the anatomic integrity of the stomatognathic system, a prosthesis allows its physiologic activities to proceed normally. A function imprint provides a picture of the prosthesis support and relationship between mobile tissues and prosthesis rims. A high-quality function imprint required, but current literature offers different opinions and views concerning the choice and method of taking a function imprint. Several procedures covering all methodologic differences can still be singled out: taking a function imprint without pressure, taking a function imprint with pressure, taking a

Rehabilitation of maxillofacial defects following oncological surgery remains a clinical challenge. Patients undergoing extensive ablative procedures often present with complex intraoral and extraoral defects that severely compromise function, esthetics, and quality of life. This case report describes the prosthetic rehabilitation of a 61-year-old male patient with a combined intraoral and extraoral defect secondary to surgical resection and radiotherapy for carcinoma of the right buccal mucosa. The patient was rehabilitated with a maxillary obturator prosthesis and a silicone extraoral facial

The use of speech prosthesis after total laryngectomy has become an international standard for voice restoration today. Provox II voice prosthesis is not permanently inserted, and as such, it must meet the criterion of achieving prolonged retention time within the walls of tracheo-esophageal fistula (TEF). Complications after the insertion of speech prosthesis are familiar and anticipated but efforts are being made in order to reduce them. Part of the complications is caused by inadequate choice of the length of the prosthesis. The Department of Otorhinolaryngology and Head and Neck Surgery in

The rehabilitation of patients with acquired defects of the maxilla is a challenge in terms of reestablishing oronasal separation. In most patients these goals are met by means of prosthetic rehabilitation with an obturator prosthesis. If the remaining dentition does not offer sufficient retention and support, the placement of zygoma implants can enhance the stability of the prosthesis. Due to the anatomic intricacies of the zygomatic bone and the implant length, computer-supported navigated implant placement can be advantageous. In the following clinical report, a diabetic patient with a stat

Two-stage temporomandibular joint (TMJ) arthroplasty may involve the placement of a spacer until a final prosthesis is placed. In cases where a patient is not immobilized via intermaxillary fixation (IMF), the spacer is considered to be articulating, and the patient can open their mouth and function more normally during this interval. This study was designed to retrospectively assess the volumetric changes of the spacer within the temporomandibular joint following the use of an articulating bone cement spacer in a two-stage reconstruction. Fourteen patients (thirteen females, one male with mea

Rehabilitation of hemimaxillectomy patients can be challenging. The most common problem with prosthetic treatment in such patients is in getting adequate retention, stability, and support. The size and location of the defect usually influences the amount of impairment and difficulty in prosthetic rehabilitation. The obturator prosthesis is commonly used as an effective means for rehabilitating hemimaxillectomy cases. In cases of large maxillary defects, movement of the obturator prosthesis is inevitable and requires a form of indirect retention to limit the rotation of the prosthesis. The goal

Prosthetic rehabilitation of an edentulous cleft lip and palate patient with a combined hard and soft palate defect is a great challenge, due to the lack of retention of the obturator prosthesis as a result of its weight and the inability to obtain a border seal. Dental implants improve the retention, stability, and occlusal function of prostheses when used in carefully selected cleft lip and palate cases. This clinical report presents an edentulous unilateral cleft lip and palate patient who has hard and soft palate defects and an atrophied maxilla, treated with an implant-supported speech-ai

Management of malignancies and abnormal growths in maxillary sinus often include ablative surgeries. The closure of the resulting anatomical defect can be achieved with an obturator prosthesis. Reduction in weight of the obturator is an important consideration in improving the retention and stability of the prosthesis. Hollowing the bulb of the obturator (extension of the prosthesis into the defect) is an effective method of reducing the weight of the prosthesis. Such obturators can be one piece or two pieces. Of the many methods to make an obturator hollow, four techniques are described here.

The unfortunate loss of all or part of a finger caused by traumatic amputation has a negative impact on physical and psychological well-being of a person. Thus an aesthetic prosthesis can offer psychological, functional and rehabilitative advantages. One of the major problems with the prosthetic replacement of a partially amputated finger is inadequate retention of the prosthesis and there are numerous means to enhance the retention. The most routinely used are adhesives, adhesive tape, magnets and implants. The present article describes a technique which utilizes room temperature vulcanizing

The authors report their experience with 34 patients who had large full thickness nasal defects reconstructed with an implant-retained prosthesis. Their technique of modifying post-rhinectomy defects is described and factors influencing implant success are evaluated. 111 implants were placed to retain a nasal prosthesis. Age, sex and tumour histology did not affect the outcome. Smoking, extent of rhinectomy, use of radiotherapy (pre- and post-implant), hyperbaric oxygen, length and location of the implant and type of retention (bar/magnets) influenced implant success. The overall success rate

Maxillary defects occur either as a result of surgical resection of malignant tumors of the nasal cavity and paranasal sinuses or of the congenital causes. Rehabilitation of the patients with maxillectomy defects presents a challenge in restoring the lost form, function and speech. Maxillary interim obturators in prosthetic reconstruction of the defects are often complicated with lack of adequate retention, stability, and support. This case report presents the simplified approach, to rehabilitate a case of sub-total maxillectomy due to squamous cell carcinoma of maxillary sinus, using a closed
