Facial fractures in a level I trauma centre: the importance of protective devices and alcohol abuse.

Urban trauma centres have recently noted a shift in the causative mechanism of facial fractures away from motor vehicle crashes (MVC) to blunt assaults (BA). This study was conducted to examine the incidence and aetiology of facial fractures at our institution as well as the relationship with alcohol and protective device use. Trauma registry records of all patients admitted to a level I trauma centre from 1 January 1988 to 1 January 1999 were reviewed. There were 13594 trauma admissions during the 11-year period. Facial fractures were sustained by 1429 patients (10.5%) and this group forms th

Embolic protection devices for carotid artery stenting: better results than stenting without protection?

AIMS: Carotid artery stenting (CAS) for carotid artery stenoses has become an alternative to carotid endarterectomy. However, CAS itself can cause cerebral ischaemic events. Embolic protection devices (PD) promise to reduce the incidence of these events. METHODS AND RESULTS: From July 1996 to March 2003, 1483 patients from 26 hospitals were included in the prospective CAS Registry of the ALKK study group. A PD was used in 668 of 1483 patients (45%). The use of a PD has grown rapidly over the years and reached 100% in 2003. Patients treated with a PD had prior carotid artery dilatation more of

[Design and application of a ventilator circuit interface protective device for weaning].

With the continuous advancement and innovation in medical equipment technology, the transition between high-flow oxygen therapy, non-invasive ventilation, and invasive ventilation can be easily achieved by adjusting the ventilation mode of ventilators. During the weaning phase for tracheotomized patients, it is necessary to disconnect the ventilator circuit, change the ventilator mode, and gradually extend the weaning time to achieve complete ventilator liberation. During the weaning process, due to patients' excessive dependence on the ventilator, there may be situations where respiratory end

Proximal Embolic Protection Devices in Complicated Carotid Artery Stenting.

The most frequent complication during carotid artery stenting (CAS) is intraoperative distal embolization. Three categories of embolic protection devices (EPDs) are routinely used through a transfemoral or transcervical approach: distal occlusion devices, distal EPDs with flow preservation using filters, and the proximal occlusive protective systems. We report the case of the internal carotid artery (ICA) plaque rupture during CAS using a proximal EPD (the Mo.Ma system; INVATEC), treated with immediate surgery. The proximal occlusive protective system permitted the common carotid artery and IC

Embolic Protection Devices During TAVI: Current Evidence and Uncertainties.

Transcatheter aortic valve implantation (TAVI) is now the principal therapeutic option in patients with severe aortic stenosis deemed inoperable or at high surgical risk. Implementing TAVI in a lower risk profile population could be limited by relatively high cerebrovascular event rates related to the procedure. Diffusion-weighted magnetic resonance imaging studies have demonstrated the ubiquitous presence of silent embolic cerebral infarcts after TAVI, with some data relating these lesions to subsequent cognitive decline. Embolic protection devices provide a mechanical barrier against debris

Hearing protection devices: Use at work predicts use at play.

Use of hearing protection devices (HPDs) at work is widespread and well researched, but less is known about HPD use in high-noise leisure activities. We investigated HPD use of 8,144 Australians in leisure settings. An online survey asked questions about HPD use at work and leisure and examined whether age, gender, HPD use at work, and tinnitus predicted HPD use in leisure activities. Leisure-based HPD use was most common during high-risk work-related activities. Use of HPDs at work was the most significant predictor of leisure-based use, with workplace users up to 5 times more likely to use H

FiberNet--a new embolic protection device for carotid artery stenting.

BACKGROUND: Though distal filter protection during carotid stenting reduces the risk of cerebrovascular events, periprocedural stroke remains a risk despite their broad usage. This observation may be related to the pore size of common filters. The FiberNet distal filter system is unique by its very small pore size (40 µm) as well as its low profile and flexibility. Little data is available regarding the clinical performance and safety of this device. PURPOSE: The aim was the evaluation of the safety of the FiberNet embolic protection system during carotid artery stenting. METHODS: All consec

Feasibility trial of carotid stenting with and without an embolus protection device.

PURPOSE: To evaluate the feasibility of stenting and cerebral protection in patients with carotid bifurcation disease. METHODS: Carotid stenting with self-expanding nitinol stents was evaluated in 261 patients (157 men; mean age 70.9+/-9.7 years, range 41-90) treated at 33 clinical sites. Among these patients, 141 (54.3%) were symptomatic, and 71 (27.2%) had postendarterectomy restenotic lesions. An embolus protection device became available in the latter third of the study. The primary endpoint of the study was the 30-day composite occurrence of major adverse clinical events (all deaths, str

Choice of protection devices in carotid angioplasty. Two case reports.

Patient selection is of the utmost importance with all endovascular procedures. It is equally important to select the appropriate protection device during carotid angioplasty/stenting (CAS). CAS in two patients was unsuccessful due to the chosen protection device. Occlusion of the external carotid artery while deploying a Parodi Anti-Embolic-System Device (PAEC - ArteriA) resulted in cerebral ischaemia. Conversion to the Angioguard System (Cordis) made carotid angioplasty possible, but was complicated by thrombosis of the filter. In the second case it was impossible to negotiate tortuous vesse

Evaluation of the SafeHeal Colovac+ Anastomosis Protection Device: A Preclinical Study.

BackgroundThe protective ostomy is the current standard of care to protect a low colorectal anastomosis from leakage, but exposes patients to complications requiring an alternative strategy. The Colovac+ is a vacuum-based intraluminal bypass device designed to shield the anastomosis from fecal content, preventing the clinical outcomes of anastomotic leakage. The objective of this study was to evaluate the preliminary efficacy, safety, and technical feasibility of the Colovac+ in a porcine model. Methods: Twelve pigs received a colorectal anastomosis with Colovac+ implantation. The device was l

The hazards of using floor mats as a fall protection device at the bedside.

OBJECTIVES: In this study, we evaluated the clinical safety of the floor mat, a device intended to reduce severity of injury in falls from the bed or at the bedside. METHODS: Fifteen participants with a range of gaits and fall risk were video-recorded as they approached and entered a hospital bed and then as they exited and walked away. For 10 participants, a 1-inch-thick floor mat with a beveled edge was used as a fall protection device at the bedside. The videos were coded and analyzed for the purpose of determining the mechanisms by which the floor mat affected balance and gait while parti

The impact of embolic protection device and stent design on the outcome of CAS.

The importance of angioplasty and stenting in the treatment of carotid artery disease cannot be underestimated. Successful carotid stenting does not only depend on the operator's skills and experience but also on an adequate selection of cerebral protection devices, and carotid stents can help avoid neurological complications. A broad spectrum of carotid devices is currently on the market, and because each of them has its own advantages and disadvantages, it is virtually impossible to claim that one specific device is the best. The individual characteristics of each specific protection system
