Proteus mirabilis bloodstream infections: risk factors and treatment outcome related to the expression of extended-spectrum beta-lactamases.

Bloodstream infection (BSI) due to Proteus mirabilis strains is a relatively uncommon clinical entity, and its significance has received little attention. This study was initiated to evaluate risk factors and treatment outcome of BSI episodes due to P. mirabilis producing extended-spectrum beta-lactamases (ESBLs). Twenty-five BSI episodes caused by P. mirabilis occurred at our hospital (Ospedale di Circolo e Fondazione Macchi, Varese, Italy) over a 7.5-year period. Phenotypic and molecular methods were used to assess ESBL production. Clinical records of BSI patients were examined retrospective

Tuberculous Otitis with Proteus mirabilis Co-Infection: An Unsuspected Presentation Encountered in Clinical Practice.

Tuberculosis, a contagious bacterial disease which is caused by Mycobacterium tuberculosis, primarily involves the lungs.Though Pulmonary tuberculosis (PTB) is the commonest clinical presentation, there is a need for alertness towards uncommon presentations which involve other organs. Tuberculous otitis media (TOM) is one such rare presentation seen in paediatric practice. It is characterized by painless otorrhoea which fails to respond to the routine antibacterial treatment. TOM usually occurs secondary to PTB. Here is a case of tuberculous otitis media with Proteus mirabilis co-infection, wi

HIV And HCV infection among opiate-dependent patients and methadone doses: the PROTEUS study.

Human immunodeficiency virus (HIV) and hepatitis C virus (HCV) are prevalent infections in opiate-dependent patients. Opiate replacement treatment (ORT) with methadone or buprenorphine is associated with several important outcomes among patients with opiate dependence. However, little is known about outcomes in patients with HIV and/or HCV infections that are in ORT. Also, it is not well established whether the presence of HCV or HIV infection could be associated with higher methadone doses. This paper reanalyzes the database of PROTEUS study, using two principal variables: methadone dose and

Necrotizing suppurative nephritis in a Japanese black feedlot steer due to Proteus mirabilis infection.

A Japanese black feedlot steer suddenly died after exhibiting astasia and cramping of the extremities. Necropsy of the animal revealed that the right kidney was enlarged and pale with severe nephrolithiasis. The urinary bladder displayed mucosal hemorrhage. Upon bacteriological investigation, Proteus mirabilis was isolated from the liver, spleen, right kidney, lungs and urine. Histopathological examination revealed necrotizing suppurative nephritis with the presence of numerous gram-negative bacilli and fibrinous suppurative cystitis with no bacilli. Immunohistochemical analysis revealed that

Assessment of genetic determinants of virulence and fluoroquinolone resistance in Proteus mirabilis isolates from urinary tract infections in Egypt.

BACKGROUND: Proteus mirabilis (P. mirabilis) is a leading cause of urinary tract infections (UTIs) in both community and healthcare settings, especially in catheterized patients. Proteus species express various types of fimbriae that can serve as colonization-related virulence factors. The severity of infection escalates when these virulent strains acquire antibiotic resistance. The aim of this study was to investigate the prevalence, antimicrobial susceptibility, and genetic determinants of fluoroquinolone resistance in P. mirabilis isolates. Further, to screen for the presence of virulence g

Use of Vancomycin Powder in Spinal Deformity Surgery in Cerebral Palsy Patients is Associated With Proteus Surgical Site Infections.

PURPOSE: Surgical site infection (SSI) rates in pediatric spinal deformity surgery for cerebral palsy (CP) patients are higher than that in idiopathic scoliosis. The use of vancomycin powder is associated with decreased risk of SSI in neuromuscular patients. Prior studies in adult and pediatric early-onset scoliosis patients have shown that vancomycin powder alters microbacterial profile in patients that develop SSI. However, the effects of topical vancomycin powder on microbiology in spinal deformity surgery for CP patients has not been studied. METHODS: An international multicenter database

Dissemination of integrons and carbapenemase-encoding genes among multidrug resistant Proteus mirabilis isolated from urinary tract infections in Egypt.

BACKGROUND: Proteus mirabilis (P. mirabilis) is an opportunistic pathogen responsible for various community-acquired and nosocomial infections, particularly urinary tract infections (UTIs). Rising resistance to broad-spectrum antibiotics, including carbapenems, is narrowing treatment options and poses a major public health concern. This study aimed to investigate the antimicrobial susceptibility patterns of P. mirabilis isolates obtained from UTI patients. Then, to assess the molecular determinants of carbapenem-resistant isolates with a focus on the role of integrons in resistance gene dissem

Vaccination to Protect Against Proteus mirabilis Challenge Utilizing the Ascending Model of Urinary Tract Infection.

Proteus mirabilis is a major cause of complicated urinary tract infections (UTIs). P. mirabilis' urease activity hydrolyzes urea and raises urine pH levels, which can catalyze bladder and kidney stone formation. This urolithiasis leads to harder-to-treat infections, possible urinary blockage, and subsequent septicemia. Development of a mucosal vaccine against P. mirabilis urinary tract infections is critical to protect against this potentially deadly infection process. Here, we describe the methodology necessary to produce a vaccine candidate conjugated to cholera toxin, administer the vaccine

Genomic analyses of a novel bacteriophage (VB_PmiS-Isfahan) within Siphoviridae family infecting Proteus mirabilis.

Proteus mirabilis is one of the most common causes of complicated urinary tract infections (UTI), especially in catheter-associated UTIs. The increased resistance to antibiotics, among P. mirabilis isolates has led us to search for alternative antibacterial agents. In this study, genome of a lytic Proteus phage VB_PmiS-Isfahan, isolated from wastewater, and active against planktonic and biofilms of P. mirabilis, isolated from UTI, was analyzed. Accordingly, the genome was sequenced and its similarity to other phages was assessed by the Mauve and EasyFig softwares. "One Click" was used for phyl

The Pathogenic Potential of Proteus mirabilis Is Enhanced by Other Uropathogens during Polymicrobial Urinary Tract Infection.

Urinary catheter use is prevalent in health care settings, and polymicrobial colonization by urease-positive organisms, such as Proteus mirabilis and Providencia stuartii, commonly occurs with long-term catheterization. We previously demonstrated that coinfection with P. mirabilis and P. stuartii increased overall urease activity in vitro and disease severity in a model of urinary tract infection (UTI). In this study, we expanded these findings to a murine model of catheter-associated UTI (CAUTI), delineated the contribution of enhanced urease activity to coinfection pathogenesis, and screened

Contribution of three tRNA modification enzymes to Proteus mirabilis fitness and catheter-associated urinary tract infection.

RNA modifications play essential roles in cellular physiology by modulating RNA structure, stability, and translation efficiency. Among these, dihydrouridine is a conserved tRNA modification synthesized by the flavin-dependent enzymes DusA, DusB, and DusC, which introduce flexibility at defined uridine positions in the D-loop. While structural and biochemical studies in Escherichia coli have clarified dihydrouridine synthase (Dus) enzyme substrate specificities, their functional relevance in other bacteria remains uncharacterized. Here, we present the first functional characterization of the c

Expanding the diagnostic spectrum of malignant otitis externa: a case report of proteus infection in a non-immunosuppressed patient.

INTRODUCTION: Malignant otitis externa (MOE) is a severe and potentially life-threatening infection of the external auditory canal, most frequently caused by Pseudomonas aeruginosa. Although typically seen in immunocompromised individuals-especially those with diabetes mellitus-up to 45% of cases have been reported in non-diabetic patients, highlighting the importance of recognizing atypical presentations. Rare pathogens, such as Proteus mirabilis, have been identified in only a few documented cases. Early diagnosis and prompt intervention are essential to prevent serious complications, includ

Nails and skin co-infection by Fusarium verticillioides and Proteus vulgaris secondary to arterial occlusion of lower extremity.

BACKGROUND: Post-traumatic Fusarium infection is rare. Arterial occlusive disease, a common vascular disorder in the elderly, often leads to ischemic necrosis of the lower extremities, which in turn increases the likelihood of secondary infections. Those secondary infections can be caused by bacteria, virus, or fungi. CASE REPORT: We present the case of a 64-year-old male patient with a co-infection by Fusarium verticillioides and Proteus vulgaris on nails and foot skin, secondary to senile arterial occlusion on the lower extremities. The skin and nails recovered well after following stent im

Ultrastructure of Proteus mirabilis swarmer cell rafts and role of swarming in catheter-associated urinary tract infection.

Proteus mirabilis is a common cause of catheter-associated urinary tract infection (C-UTI). It blocks indwelling urethral catheters through the formation of extensive crystalline biofilms. The obstruction of urine flow can induce episodes of pyelonephritis, septicemia, and shock. P. mirabilis exhibits a type of motility referred to as swarming, in which multicellular rafts of elongated, hyperflagellated swarmer cells form and move rapidly in concert over solid surfaces. It has been suggested that swarming is important in the pathogenesis of C-UTI. In this study we generated a set of stable tra
