Proton radiation therapy for the treatment of retinoblastoma.

PURPOSE: To investigate long-term disease and toxicity outcomes for pediatric retinoblastoma patients treated with proton radiation therapy (PRT). METHODS AND MATERIALS: This is a retrospective analysis of 49 retinoblastoma patients (60 eyes) treated with PRT between 1986 and 2012. RESULTS: The majority (84%) of patients had bilateral disease, and nearly half (45%) had received prior chemotherapy. At a median follow-up of 8 years (range, 1-24 years), no patients died of retinoblastoma or developed metastatic disease. The post-PRT enucleation rate was low (18%), especially in patients with ea

Particle Therapy: Protons and Heavy Ions.

Particle therapy is a comparatively new radiotherapy treatment form allowing the application of extremely conformal treatment plans. In head and neck malignancies, especially in the paranasal sinus and anterior skull base, this technology is very valuable in order to maintain normal tissue tolerance of critical structures while still applying high doses to the tumour. The following paper will further explore the rationale of particle therapy and available clinical experience of the most commonly treated malignancies arising in these anatomical sites. The potential of particle therapy for re-ir

Consensus Statement on Proton Therapy in Mesothelioma.

PURPOSE: Radiation therapy for mesothelioma remains challenging, as normal tissue toxicity limits the amount of radiation that can be safely delivered to the pleural surfaces, especially radiation dose to the contralateral lung. The physical properties of proton therapy result in better sparing of normal tissues when treating the pleura, both in the postpneumonectomy setting and the lung-intact setting. Compared with photon radiation, there are dramatic reductions in dose to the contralateral lung, heart, liver, kidneys, and stomach. However, the tissue heterogeneity in the thorax, organ motio

Proton Therapy for Head and Neck Cancers.

Because of its sharp lateral penumbra and steep distal fall-off, proton therapy offers dosimetric advantages over photon therapy. In head and neck cancer, proton therapy has been used for decades in the treatment of skull-base tumors. In recent years the use of proton therapy has been extended to numerous other disease sites, including nasopharynx, oropharynx, nasal cavity and paranasal sinuses, periorbital tumors, skin, and salivary gland, or to reirradiation. The aim of this review is to present the physical properties and dosimetric benefit of proton therapy over advanced photon therapy; to

Proton therapy for tumors of the base of the skull.

Relative to conventional photon irradiation, proton therapy has distinct advantages in its ability to more precisely target tumor while shielding adjacent normal tissues. In the setting of skull base tumors, proton therapy plays a critical role in the dose-escalation required for optimal tumor control of chordomas, chondrosarcomas, and malignancies of the paranasal sinuses and nasal cavity. For benign tumors such as craniopharyngiomas, pituitary adenomas and meningiomas, proton therapy can limit long-term adverse effects, such as secondary malignancies. This review summarizes published literat

[Current situation and perspectives of proton therapy].

Proton beam therapy is indicated as a treatment for some rare tumours and paediatric tumours because the technique allows a good local control with minimal toxicity; the growing number of centres that use proton beam therapy is associated with an increase of dosimetric and clinical data for other malignant tumours as well. This paper reviews potential indications of proton beam therapy. A systematic review on Medline was performed with the following keywords proton beam therapy, cancer, heavy particle, charged particle. No phase III trial has been published using proton beam therapy in compari

Proton therapy for pediatric and adolescent esthesioneuroblastoma.

BACKGROUND: Esthesioneuroblastoma (EN) of the paranasal sinus comprises less than 3% of tumors of in pediatric and adolescent patients [1]. The collective adult literature indicates a critical role for radiotherapy in attaining cure [2], yet pediatric outcome data is limited. Radiation in pediatric patients with EN can cause significant morbidity due to the proximity of critical structures. Proton radiotherapy offers a potential dosimetric benefit that may improve long-term survival and toxicity outcomes in the pediatric population [3]. METHODS: We retrospectively identified eight patients tr

Proton pump inhibitory therapy: then and now.

Proton pump inhibitors (PPIs) have been established as the new "gold standard" for traditional acid-inhibitory treatment of the so called "peptic" diseases. Due to the high antisecretory and ulcer-healing potency of omeprazole, no major improvements of the efficacy in ulcer healing and pain relief can be expected. Pantoprazole, as a further development in PPIs, is characterized by improved pharmacokinetic behavior as well as by higher tissue selectivity and binding specificity and by a very low potential to interact with the cytochrome P450 enzyme system. These characteristics may provide the

Proton beam therapy: neuroradiology considerations and challenges.

Proton beam therapy (PBT) is increasingly used in paediatric neuro-oncology due to its favourable dose distribution and potential to reduce treatment toxicity. As its clinical use expands, neuroradiologists play a central role in recognising therapy-related changes and supporting long-term surveillance. This review outlines key neuroradiological considerations across the PBT pathway. MRI is fundamental to planning, but its integration requires modified workflows, specialised immobilisation equipment, and tailored sequence design to balance geometric accuracy, signal-to-noise constraints, and p

Proton beam therapy for malignant pleural mesothelioma.

Malignant pleural mesothelioma (MPM) is a rare disease with a poor prognosis. Surgical techniques have made incremental improvements over the last few decades while new systemic therapies, including immunotherapies, show promise as potentially effective novel therapies. Radiation therapy has historically been used only in the palliative setting or as adjuvant therapy after extrapleural pneumonectomy, but recent advances in treatment planning and delivery techniques utilizing intensity-modulated radiation therapy and more recently pencil-beam scanning (PBS) proton therapy, have enabled the deli

Prioritized optimization in intensity modulated proton therapy.

In this work a prioritized optimization algorithm is adapted and applied to treatment planning for intensity modulated proton therapy (IMPT). Originally, this algorithm was developed for intensity modulated radiation therapy (IMRT) with photons. Prioritized optimization converts the clinical hierarchy of treatment goals into an effective optimization scheme for treatment planning. It presents an alternative to conventional methods that combine all optimization goals into a single optimization run with a weighted sum of all planning aims in the objective function. The highest order goal in the

Advances in Pediatric Radiation Oncology: Proton Therapy.

Radiation therapy is an integral part of treatment of many pediatric cancers, but is associated with long-term side effects. Recent advances in radiation therapy, and in particular in the use of particle therapy with protons, have allowed the delivery of more effective doses of radiation with fewer late effects. The physical properties of protons allow for significantly lower doses to be given to normal tissues outside of the intended treatment volume, which provides meaningful long-term benefits across multiple disease sites, including better neurocognitive outcomes, less ototoxicity, fewer e

Tubarial salivary gland sparing with proton therapy.

The recently identified bilateral macroscopic tubarial salivary glands present a potential opportunity for further toxicity mitigation for patients receiving head and neck radiotherapy. Here, we show superior dosimetric sparing of the tubarial salivary glands with proton radiation therapy (PRT) compared to intensity-modulated radiotherapy (IMRT) for patients treated postoperatively for human papillomavirus (HPV)-associated oropharyngeal squamous cell carcinoma (OPSCC). This was a retrospective, single institutional study of all patients treated with adjuvant PRT for HPV-associated OPSCC from 2

A simulation study of in-beam visualization system for proton therapy by monitoring scattered protons.

Recently, in-beam positron emission tomography (PET) has been actively researched for reducing biological washout effects and dose monitoring during irradiation. However, the positron distribution does not precisely reflect the dose distribution since positron production and ionization are completely different physical processes. Thus, a novel in-beam system was proposed to determine proton dose range by measuring scattered protons with dozens of scintillation detectors surrounding the body surface. While previous studies conducted a preliminary experiment with a simple phantom, we simulated m

Evolution of Proton Radiation Therapy Brainstem Constraints on the Pediatric Proton/Photon Consortium Registry.

PURPOSE: Increasing concern that brainstem toxicity incidence after proton radiation therapy might be higher than with photons led to a 2014 University of Florida (UF) landmark paper identifying its risk factors and proposing more conservative dose constraints. We evaluated how practice patterns changed among the Pediatric Proton/Photon Consortium Registry (PPCR). MATERIAL AND METHODS: This prospective multicenter cohort study gathered data from patients under the age of 22 years enrolled on the PPCR, treated between 2002 and 2019 for primary posterior fossa brain tumors. 3Din vivodose verification in prostate proton therapy with deep learning-based proton-acoustic imaging.

Dose delivery uncertainty is a major concern in proton therapy, adversely affecting the treatment precision and outcome. Recently, a promising technique, proton-acoustic (PA) imaging, has been developed to provide real-timein vivo3D dose verification. However, its dosimetry accuracy is limited due to the limited-angle view of the ultrasound transducer. In this study, we developed a deep learning-based method to address the limited-view issue in the PA reconstruction. A deep cascaded convolutional neural network (DC-CNN) was proposed to reconstruct 3D high-quality radiation-induced pressures us
