BACKGROUND: Proximal femoral fractures (PFFs) are a public health issue due to their high frequency. The frequency of a second PFF on the other side is estimated at 10%. This estimation is controversial, however, and the risk factors have not been evaluated in a large population of French patients. The objective of this retrospective case-control study was to determine: (1) the incidence of second PFFs and (2) their risk factors.
HYPOTHESIS: The incidence of second PFFs is >2% after 1 year and >5% after 3 years.
MATERIAL AND METHODS: We conducted a case-control study in a population of conse

Two factors are primarily responsible for complications after treatment of proximal femoral fractures. First, the strong deforming forces across the hip joint and proximal femur can make fracture reduction difficult. Second, the placement of the implant affects fracture healing and outcome more dramatically than in other areas of the body. In subtrochanteric fractures, the use of appropriate reduction and stabilization techniques can prevent varus malreduction and subsequent failure of the fixation device. In intertrochanteric fractures, lag screw cutout can be prevented by correct implant pos

The epidemiology of proximal femur fractures (FXf) was studied in the city of La Plata, Argentina. At the time of the study the total population was 288,000 inhabitants and 73,900 (30,700 men and 43,200 women) were 50 years of age and over (Table 1). From May 1, 1989 to April 30, 1990 all the patients admitted in local hospitals with FXf were personally interviewed by one of the authors. Patients living outside the city of La Plata or fractures due to metastatic or Paget disease were excluded. During the period of the study 195 patients with FXf (164 women and 31 men) were registered. The medi

Due to demographic changes and increased survival rates of total hip arthroplasties, the incidence of periprosthetic proximal femoral fractures is increasing. The current treatment concept requires accurate preoperative planning. Besides patient-related risk factors, fracture type, prosthesis stability, and bone quality influence whether osteosynthesis or a revision arthroplasty is required.

BACKGROUND: Proximal humeral and trochanteric femoral fractures in older patients are typically caused by low-energy trauma and are therefore often associated with osteoporosis. The treatment of such fragility fractures can be difficult as implant purchase is reduced in osteoporotic bone. By augmenting the fixation with cement the contact surface between implant and bone can be increased, which improves the stability of the osteosynthesis.
OBJECTIVE: This article describes the operative technique for the augmentation of trochanteric femoral fractures treated with the Trochanteric Fixation Nai

Hip fracture occurrence was examined cross-sectionally in Japanese patients with rheumatoid arthritis (RA). Between January 2005 and June 2006 we studied RA outpatients with a past history of hip fractures. Patients included 1 man and 25 women. As 3 women had bilateral hip fractures, the total number was 29. Age at the time of fracture was 72.1 +/- 4.5 years. Of the 29 fractures, 22 were cervical and 7 were trochanteric. Four fractures were spontaneous while the others occurred in falls. 24 fractures were associated with oral steroid administration. All 5 fractures unassociated with prednisolo

The circumstances of subtrochanteric fractures following the use of multiple pins for fixation of femoral neck fractures were reviewed in four women (age range, 67-94 years). Internal fixation by use of multiple pins in the proximal femur is a common orthopedic procedure. The pins, however, create a significant stress riser in the lateral subtrochanteric cortex. Subtrochanteric fracture has been reported after internal fixation of slipped capital femoral epiphysis with multiple conventional pins and, recently, after internal fixation of femoral neck fractures with large Garden screws.

BACKGROUND: Ipsilateral fractures of the proximal femur and femoral shaft are extremely uncommon injuries which occur in young adults who sustain a high energy trauma. A variety of management modalities have been tried to treat this complex fracture pattern ranging from conservative approach to recently introduced reconstruction nails. All these approaches have their own difficulties. We studied the outcome of long proximal femoral nail (LPFN) in the management of concomitant ipsilateral fracture of the proximal femur and femoral shaft.
MATERIALS AND METHODS: We analysed the prospective data

INTRODUCTION: The gamma-proximal femoral nail (GPFN) and the expandable proximal femoral nail (EPFN) are two commonly used intramedullary devices for the treatment of AO 31A1-3 proximal femur fractures. The aim of this study was to compare outcomes and complication rates in patients treated by both devices.
PATIENTS AND METHODS: A total of 299 patients (149 in the GPFN group and 150 in the EPFN group, average age 83.6 years) were treated for AO 31A1-3 proximal femur fractures in our institution between July 2008 and February 2013. Time from presentation to surgery, level of experience of the

Subtrochanteric femoral fractures are one of the common fractures encountered in today's Orthopaedic practice. This area consists of mostly cortical bone with high stress generation thus heal slowly which leads implant failure. The inherent instability of this fracture and forces of the muscles with comminuted medial calcar is giving the fracture a tendency to varus collapse. Intramedullary nails are not suitable for short proximal segment and wide medullary canal while extramedullary implants are associated with higher rate of implant failure. Recently proximal femoral locking compression pla

PURPOSE OF THE STUDY: The management of intracapsular femoral neck fractures remains an unsolved issue of the present-day trauma surgery of the musculoskeletal system. These fractures are conventionally treated with the use of spongious screws or a dynamic hip screw combined with an antirotational spongious screw. The aim of this study was to evaluate the outcomes of surgical therapy using a different type of implant (proximal femoral nail) for intracapsular femoral neck fractures.
MATERIAL: From January 2000 to March 2004, intracapsular fractures were treated with the use of a proximal femor

OBJECTIVE: Subtrochanteric femoral fractures are severe injuries. Although many treatment methods have been developed, controversy exists regarding the optimal management of these fractures. This study evaluated the clinical outcome of subtrochanteric femoral fractures fixed with long proximal femoral nail antirotation (PFNA-long).
METHODS: Between October 2006 and February 2008, 25 patients with traumatic subtrochanteric fractures of the femur were treated with PFNA-long. Closed reduction and fixation were performed in 20 cases. In the remaining 5 cases, closed reduction was difficult, so li

OBJECTIVE: Femoral reconstruction nails (RN) and three cannulated screws combined with a plate (TCP) are commonly employed for the treatment of ipsilateral femoral neck and shaft fractures (IFNSF). However, these fixation strategies are associated with a considerable incidence of postoperative complications, reflecting the biomechanical challenges of this fracture pattern. Therefore, this study aims to compare the biomechanical performance of a novel long proximal femoral bionic nail (PFBN) with these conventional fixation methods for the treatment of this complex fracture pattern.

UNLABELLED: Intertrochanteric fractures, common in the elderly, are often treated with intramedullary (IM) nails. The comparison of outcomes for different IM nails has not been thoroughly evaluated. This meta-analysis compares postoperative outcomes of PFNA, InterTan, and the newer PFBN. PubMed, Embase, and Google Scholar were searched until April 2025. Comparative studies evaluating ≥2 IM nails reporting key outcomes were included. Outcomes included union/weight-bearing/hospital stay times, operative time, blood loss, complications (delayed/non-union, hardware failure, infections, deep vein t

INTRODUCTION: PFNA design compacts the cancellous bone to provide increased stability and has been bio-mechanically proven to retard rotation and varus collapse.
METHODS: Between 2006 and 2007, 62 consecutive patients with unstable proximal femoral fractures were treated with the PFNA.
RESULTS: Twenty males and 42 females with a mean age of 78 years (44-94) were reviewed. In 48 patients, the fracture resulted from a low energy injury. The majority of the fractures belonged to AO/ASIF types 31A2.3 (22) and 31A3.2 (29). Twelve patients required open reduction. The PFNA blade position was centr
