Pupil diameter in darkness differentiates progressive supranuclear palsy (PSP) from other extrapyramidal syndromes.

The most important features that characterize and differentiate progressive supranuclear palsy from other Parkinsonian syndromes are postural instability, supranuclear gaze palsy, pseudobulbar palsy, parkinsonism, and cognitive disturbances. In this article, we demonstrate that progressive supranuclear palsy patients exhibit pathologically decreased pupil diameters after dark adaptation recorded by TV pupillography. A cut off value of 3.99 mm was defined to differentiate progressive supranuclear palsy patients from patients with other extrapyramidal disorders like Parkinson's disease and multi

Bilateral anterior choroidal artery infarction presenting with progressive somnolence.

A 55-year-old woman was admitted with a 3 days history of increasing lethargy with bradyphrenia and apathy. She progressively developed severe somnolence with marked abulia, right hemiparesis, right hemianopsia, and pseudobulbar palsy. Brain magnetic resonance imaging showed the rare image of bilateral acute anterior choroidal artery infarction. Pseudobulbar mutism and in rare cases abulia have been described in acute anterior choroidal artery infarction contralateral to an older lesion in mirror position. Although neurologic deterioration is not infrequent in anterior choroidal artery territo

[Progressive supranuclear paralysis with levodopa-induced dyskinesia].

Progressive supranuclear palsy (PNP) is a neurodegenerative disease characterized by a combination of progressive akinetic-rigid syndrome, postural instability with frequent falls, supranuclear ophthalmoplegia, pseudobulbar syndrome and frontal dementia. The disease usually develops after the sixth decade of life, and has a progressive course. An own description of the clinical case of progressive supranuclear palsy in a 79-year-old patient with oromandibular hyperkinesis while taking levodopa is presented.

Bilateral perisylvian ulegyria: an under-recognized, surgically remediable epileptic syndrome.

PURPOSE: Interest in the association of epilepsy and pseudobulbar palsy was rekindled since the identification through magnetic resonance imaging (MRI) of bilateral perisylvian polymicrogyria (PMG). Seizures are often intractable, but resective epilepsy surgery has not been recommended. However, a similar clinical picture can be encountered in patients with bilateral perisylvian destructive lesions, which fit the description of ulegyria (ULG). We report a series of patients with epilepsy and pseudobulbar palsy due to bilateral perisylvian ULG (BP-ULG), show that hippocampal sclerosis (HS) is o

The operculum syndrome: an unusual complication of tuberculous meningitis.

The operculum syndrome, not previously documented as a complication of tuberculous meningitis, is described in a three-year-old African boy. The appearance on CT scan correlates radiographically with anatomical descriptions of the operculum syndrome, and is useful in distinguishing the condition from pseudobulbar palsy.

[Late onset of pseudobulbar paralysis and dystonia in a case of hemispheric cortical dysplasia].

The atypical clinical course of a young male with encephalopathy due to right hemispheric cortical dysplasia (pachygiria) is described. From the first months of life the course of the disease was a static encephalopathy with left hemiparesis, epilepsy and mild mental retardation. When he was 14 years old a subacute pseudobulbar palsy, dystonia and spread of the paresis to the right side occurred. Epileptic seizures, paroxysmal EEG abnormalities and drug ingestion were excluded. Neuropsychological studies showed a low level of cognitive functions, probably related to the malformative encephalop

Differential diagnosis in progressive infantile spastic tetraparesis.

Progressive infantile spastic tetraparesis spans a wide spectrum of partially rare differential diagnoses. Based on a clinical example the differential diagnostic thoughts are discussed in detail. Though juvenile motor neuron disease is a rare entity, it has to be kept in mind for differential diagnostics in cases of slowly progressive spastic tetraparesis, especially when a pseudobulbar palsy or distal amyotrophies add to the clinical picture. Electromyography can be helpful for early detection of lower motor neuron involvement. The glutamate antagonist riluzole slows the disease progression,

Extrapontine myelinolysis caused by electrolyte imbalance during the management of suprasellar germ cell tumors. Report of two cases.

Extrapontine myelinolysis (EPM) is caused by marked fluctuation of the serum electrolyte level. Patients with suprasellar germ cell tumors frequently present with diabetes insipidus, which is often aggravated by administration of steroid hormone. In addition, cisplatin-based chemotherapy is sometimes accompanied by marked serum electrolyte fluctuation because it needs massive hydration to prevent renal damage. Two children with suprasellar germ cell tumors in whom EPM developed secondary to profound hyponatremia and was rapidly corrected are described. The central pons was spared in both cases

Progressive supranuclear palsy. Clinical and pharmacologic update.

A distinct clinicopathologic entity, PSP is differentiated from Parkinson's disease by the presence of supranuclear ophthalmoparesis. Downward gaze palsy that can be overcome by oculocephalic maneuver is the most characteristic clinical finding. The other distinguishing clinical features of PSP include axial distribution of rigidity and broad-based gait with early postural instability. Furthermore, pseudobulbar palsy is an early feature, while profound dementia usually occurs late in the course of the disease. Because of the variable clinical presentation and occasional lag in the onset of oph

Congenital perisylvian dysfunction - is it a spectrum?

AIM: This study examines the overlap between children with bulbar cerebral palsy (Worster-Drought syndrome [WDS]) and perisylvian polymicrogyria. METHOD: A total of 121 children (81 males, 40 females; mean age 5y 5mo, SD 3y 6mo; age range 1mo-15y 4mo) were studied using retrospective clinical data and magnetic resonance imaging. In all, 70 children had WDS with normal perisylvian imaging, 31 had congenital bilateral perisylvian polymicrogyria (CBPP), and 20 had congenital unilateral perisylvian polymicrogyria (CUPP). RESULTS: All groups shared aetiological markers (male sex, congenital contr

Clinical Features of CADASIL.

Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is a rare hereditary disease characterized by recurrent transient ischemic attacks, strokes, and vascular dementia. Various mutations in the Notch3 gene cause the disease, whereas the mechanism of how they cause the disorder remains unknown. We recently identified two Japanese CADASIL families with an R141C mutation. The mean age of onset was 44.6, and the main symptoms were recurrent strokes and progressive motor disturbances in extremities, as well as pseudobulbar palsy. Besides those in whit

A clinically recognizable neuronal migration disorder: congenital bilateral perisylvian syndrome. Case report with long-term clinical and EEG follow-up.

Congenital bilateral perisylvian syndrome (CBPS) is a recently described, neuronal migration disorder, characterized by pseudobulbar palsy, epilepsy and mental retardation and bilateral perisylvian dysplasia. A 15-year-old boy was diagnosed with CBPS according to the typical clinical, and magnetic resonance imaging (MRI) features. The patient was suffering from atypical absence seizures, repeating daily in spite of antiepileptic drug therapy, since age 7 years. He had also experienced rare generalized tonic-clonic seizures and complex partial seizures. Neurological examination showed severe re

[Progressive supranuclear palsy].

Progressive supranuclear palsy, first described as clinical entity by Steele, Richardson and Olszewski, is a degenerative disorder of the central nervous system. Besides progressive supranuclear oculomotor disturbances, other characteristic signs are pseudobulbar paresis, axial rigidity, gait disturbances and subcortical dementia. Misinterpretation in the early stage as Parkinson's disease is frequently seen. A causal therapy is still missing.

[Progressive supranuclear palsy: neurological, neuropathological and neuropsychological aspects].

INTRODUCTION: Progressive supranuclear palsy is a syndrome characterized by supranuclear ophthalmoplegia, pseudobulbar paralysis, dysarthria, dystonic rigidity of neck and trunk and other cerebellar and pyramidal symptoms. In this article we review the principal neurological, neuropathological, neurochemical, clinical, therapeutic and neuropsychological aspects of progressive supranuclear palsy. DEVELOPMENT: Although progressive supranuclear palsy is usually included with the subcortical dementias, some neuropathological and neuroimaging findings show cortical involvement, which casts doubt o

Cerebellar ataxia with recovery related to central pontine myelinolysis.

Development of severe ataxia and mild pyramidal signs without mental deterioration, tetraparesis or pseudobulbar palsy during recovery from withdrawal delirium and initial hyponatraemia are unusual clinical features consistent with central pontine myelinolysis. This diagnosis was confirmed by magnetic resonance imaging (MRI) in an alcoholic man. Clinical and electrodiagnostic improvement occurred, whereas the MRI findings remained unchanged in a follow-up study.

Osmotic Demyelination Syndrome in a Patient With Schizophrenia: A Case Report.

BACKGROUND: Primary polydipsia affects approximately 11%-20% of patients with chronic schizophrenia, and up to one-third develop symptomatic hyponatremia. Rapid correction of hyponatremia can precipitate osmotic demyelination syndrome (ODS), involving demyelination in the central pons and extrapontine regions. ODS presenting with severe extrapyramidal symptoms (EPS) in schizophrenia has not been previously documented. CASE PRESENTATION: A 34-year-old man with chronic schizophrenia, maintained on oral olanzapine and monthly flupentixol decanoate, developed polydipsia-induced hyponatremia. His

[A case report of bilateral cerebral peduncular infarction manifesting tetraparesis and pseudobalbur palsy].

A 63-year-old female had presented with right hemiparesis and slight dysarthria. MRI had showed the infarction of left pons and left peduncle. Three months later she further presented with left hemiparesis, severe dysarthria and swallowing disturbance. MRI showed bilateral cerebral peduncular infarction. And the angiogram showed the occlusion of basilar artery at the just distal portion of the superior cerebellar artery. We recognized our case as the infarction due to the basilar artery occlusion. The 16 cases of bilateral cerebral peduncular infarction were reported. In these reports, the sym

[Dopaminergic drug-induced jaw-opening dystonia in a patient with progressive supranuclear palsy].

A 74 year-old man with progressive supranuclear palsy (PSP) was adimitted to our hospital. He developed bradykinesia 13 years previously. Neurological examination showed cognitive dysfunction, supranuclear vertical gaze palsy, pseudobulbar palsy, and parkinsonism such as akinesia, rigidity, and resting tremor. His chief complaint was glossoptosis with jaw-opening dystonia associated with rapid dose-elevation and/or overdose of dopaminergic drugs. After gradual tapering of dopaminergic drugs, he could keep his mouth closed all day. Drug-induced dystonia is a frequently encountered but often ove
