EDTA-dependent pseudothrombocytopenia: further insights and recommendations for prevention of a clinically threatening artifact.

Ethylenediaminetetra-acetic acid (EDTA) is widely used as anticoagulant in laboratory medicine. EDTA-dependent pseudothrombocytopenia is a rare phenomenon (i.e., around 0.1% in the general population), which is mostly due to the presence of EDTA-dependent antiplatelet antibodies that react optimally between 0°C and 4°C, recognize the cytoadhesive receptors gpIIb-IIIa, stimulate the expression of activation antigens, trigger activation of tyrosine kinase, platelet agglutination and clumping in vitro, which finally lead to a spuriously decreased platelet count. The reliable and timely identifica

Decision-making logic for samples of clinically significant EDTA-dependent pseudothrombocytopenia with machine learning.

OBJECTIVES: To establish practical, clinically meaningful criteria for identifying significant platelet aggregation in suspected samples, defined by a platelet-count deviation exceeding the reference change value (RCV). METHODS: Suspected aggregated samples were collected. Venous blood anticoagulated with citrate/EDTA underwent platelet counting via impedance method (PLT-I) and blood smear analysis at 5-, 30-, 60-, and 120-min intervals. Manual microscopy (30-min smears) assessed aggregation cluster size/number, while digital cytomorphology (PLT-pro) quantified clusters at all time points. Us

Anti-ethylene Diamine Tetra-Acetate (EDTA)-Induced Pseudothrombocytopenia in a Nigerian Middle-Aged Man.

Ethylene diamine tetra-acetate-induced pseudothrombocytopenia (EDTA-PTCP) is a rare laboratory phenomenon where antibodies target platelet glycoproteins (GP) IIb/IIIa, causing platelet clumping and falsely low platelet counts. This can lead to misdiagnosis, unnecessary treatments, and an artificially elevated white blood cell count. We report a 51-year-old man referred for worsening thrombocytopenia without signs of active bleeding. He had persistent low-grade fever and generalized weakness while receiving treatment for malaria. At our facility, his platelet count dropped from 25,000/mm³ to 9,

More on false thrombocytopenias: EDTA-dependent pseudothrombocytopenia associated with a congenital platelet release defect.

We report herein the case of a 54-year-old woman with a moderate bleeding tendency, diagnosed as an EDTA-dependent pseudothrombocytopenia associated with a congenital platelet release defect. The patient, at the age of 12, had a misleading diagnosis of idiopathic thrombocytopenic purpura and all the recurrent bleeding problems she had during her life were referred to that disease. The recent correct diagnosis of a false thrombocytopenia stimulated further laboratory investigation on the cause of the patient's bleeding tendency with the consequent identification of a congenital platelet deficie

A Case of Pseudothrombocytopenia due to Pre-Analytical Issues Revealed by the Presence of Fibrin in a Blood Film.

BACKGROUND: Pseudothrombocytopenia (PTCP) can be caused by anticoagulants or pre-analytical issues. The authors present a case of PTCP attributed to pre-analytical issues in a 68-year-old male patient. METHODS: The platelet count results were obtained using both the impedance and fluorescence channels of Sysmex XN-10. The blood film was scanned using both Cellavision DM96 and a microscope. RESULTS: The flag for PLT-Clumps and the scattergram from the PLT-F channel indicated the presence of platelet aggregation. Fibrin could be observed at the feathered end of the blood film. A diagnosis of P

Detecting Pseudothrombocytopenia in the Era of Artificial Intelligence: Integration of Automated Hematology, Digital Morphology, and Expert Review.

Pseudothrombocytopenia is an artefactual reduction in platelet count that occurs during laboratory testing, most commonly because ethylenediaminetetraacetic acid (EDTA) induces in vitro platelet aggregation. Although clinically benign, it may lead to unnecessary investigations, treatment, transfusion, referral, procedural delay, and patient anxiety when not recognised. Advances in hematology analyzers, including impedance, optical, fluorescence, digital morphology, and artificial intelligence (AI)-assisted technologies, offer opportunities to improve recognition of platelet aggregation and red

Effects of time, anticoagulant and detection channel on platelet count in ethylenediaminetetraacetic acid (EDTA)-dependent pseudothrombocytopenia.

BACKGROUND: EDTA-dependent pseudothrombocytopenia (EDTA-PTCP) is an in vitro phenomenon that may lead to expensive, time-consuming, and invasive diagnostic procedures as well as unnecessary patient treatment. The purpose of this study was to explore the effects of time, anticoagulant and detection channel on the platelet (PLT) count of EDTA-PTCP samples, and to suggest a better method for correcting spurious low PLT counts. METHODS: In this study, 43 identified EDTA-PTCP samples were collected. The Sysmex XN-9100, Mindray BC-6900 and Mindray BC-5390 haematology analysers were used to test the

Correction of Platelet Counts in EDTA-Induced Pseudothrombocytopenia via the Sysmex XN-20 PLT-F Scattergram.

BACKGROUND: Ethylenediaminetetraacetic acid-induced pseudothrombocytopenia (EDTA-PTCP) is a well-documented in vitro artifact characterized by platelet clumping, leading to spuriously low platelet counts despite normal platelet quantity and function. This phenomenon can result in inappropriate clinical management, including unnecessary platelet transfusions. To address this issue, we propose a post-analytical correction strategy that integrates kanamycin treatment with event-based analysis in the PLT-F channel of the Sysmex XN-20 automated hematology analyzer to improve platelet count accuracy

Decision-Making Error in Platelet Transfusion Caused by EDTA-Dependent Pseudothrombocytopenia: a Case Report and Literature Review.

BACKGROUND: Ethylenediaminetetraacetic acid-dependent pseudothrombocytopenia (EDTA-PTCP) is a rare phenomenon characterized by pseudo low platelet counts when using EDTA as anticoagulant and can result in false decision making of platelet transfusion. METHODS: An application for platelet transfusion from a patient who planned to undergo spinal surgery was received by the Department of Transfusion service. The preoperative laboratory test results showed thrombocytopenia (platelet counts: 27 x 109/L). The surgeon planned to transfuse platelets before the operation to avoid bleeding in operation

The Potential Effects of Centipede Venom and Ethylenediaminetetraacetic Acid (EDTA) Leading to Pseudothrombocytopenia in an 11-Year-Old Girl.

Centipede bites are uncommon and mostly occur in subtropical regions. While most cases present with local swelling and tenderness around the bite site, with few systemic manifestations, the potential complications can be deadly. This case describes an unusual presentation of a centipede bite, in which the patient experienced an unexpected fluctuation in platelet counts, which occurred up to weeks after the bite. While the cause remains unclear, we question whether this occurred solely due to the direct effects of the centipede venom or if there were other factors at play. The aim of this repor

Efficacy of point-of-care thromboelastography 6s to evaluate platelet function in a patient with pseudothrombocytopenia undergoing cardiopulmonary bypass: a case report.

BACKGROUND: Pseudothrombocytopenia is a phenomenon caused by in vitro platelet aggregation induced by anticoagulants contained in blood sampling tubes. Thromboelastography (TEG) 6s is a common point-of-care viscoelastic test to assess intraoperative coagulation status, which may reduce the need for blood transfusions. The reliability and usefulness of TEG6s for patients with pseudothrombocytopenia has not been established. CASE PRESENTATION: We present a patient with pseudothrombocytopenia, who underwent suprarenal abdominal aortic aneurysm repair under cardiopulmonary bypass. At the beginnin

Platelet aggregation and pseudothrombocytopenia induced by 1-desamino-8-D-arginine vasopressin (DDAVP) in type IIB von Willebrand's disease patient.

Our study shows that the thrombocytopenia described in type IIB von Willebrand's disease (vWd) after 1-desamino-8-D-arginine vasopressin (DDAVP) infusion is, at least partially, a pseudothrombocytopenia. There was a discrepancy in platelet counts in blood anticoagulated with EDTA (less than 10 x 10(3)/microliters) or citrate (55 x 10(3)/microliters) in one patient with type IIB vWd and chronic thrombocytopenia (80 x 10(3)/microliters) after DDAVP infusion. Furthermore, DDAVP induced a normalization of patient's prolonged bleeding time. Spontaneous platelet aggregation (SPA) observed in platele

Marked discrepancy of platelet counts between in-hospital and external laboratory testing due to pre-analytical factors: A case highlighting pseudothrombocytopenia.

Discrepancies in platelet counts between laboratories may lead to diagnostic confusion and unnecessary interventions. We report a case in which platelet counts were consistently normal in an in-house laboratory but repeatedly decreased in external laboratory testing. The patient had no bleeding symptoms, and coagulation findings were normal. Peripheral blood smear examination revealed platelet aggregation in externally processed samples, whereas no aggregation was observed in samples processed immediately in-house. These findings strongly support ethylenediaminetetraacetic acid (EDTA)-dependen

1-Desamino-8-D-arginine vasopressin (DDAVP) infusion in type IIB von Willebrand's disease: shortening of bleeding time and induction of a variable pseudothrombocytopenia.

We have investigated the effects of 1-desamino-8-D-arginine vasopressin (DDAVP) infusion on platelet count and bleeding time in 4 patients with type IIB von Willebrand's disease (vWd). Three of four patients showed a normalization of the bleeding time within 1 h after the infusion, while bleeding time was not modified in the fourth. In accordance with the literature, thrombocytopenia was observed after DDAVP infusion, but this thrombocytopenia was due to the anticoagulants used for blood collection. In two patients (F.I., G.F.) no thrombocytopenia was observed when platelets were counted by fi

Integration of an MC-80 Digital Image Analyzer With an Automated BC-6800Plus Hematology Analyzer Enables Accurate Platelet Counting in Samples With EDTA-Induced Pseudothrombocytopenia.

BACKGROUND: EDTA-induced pseudothrombocytopenia (PTCP) during whole blood collection requires significant laboratory resources to obtain accurate results. We evaluated platelet-deaggregation function in EDTA-induced PTCP and platelet-clump flagging by the BC-6800Plus hematology analyzer using integrated digital image analysis. METHODS: We prospectively collected 132 whole blood samples suspected of platelet clumping (102 in EDTA and 30 in sodium citrate) from 88 individuals. We compared platelet counts determined using the platelet count by impedance (PLT-I) function of the DxH 900 hematology

Pseudothrombocytopenia or platelet clumping as a possible cause of low platelet count in patients with viral infection: a case series from single institution focusing on hepatitis A virus infection.

INTRODUCTION: Pseudothrombocytopenia (PTCP) is the phenomenon of ethylenediaminetetraacetic acid anticoagulant-activated platelet clumping, which results in artificially low platelet counts. Other investigators have reported a few cases of PTCP associated with viral infections. The objective of this study was to demonstrate the association of viral infection with PTCP. METHODS: Medical records of patients with thrombocytopenia who were tested for peripheral blood smear examination between March 2009 and February 2011 were reviewed for platelet clumping and viral infection. RESULTS: Thrombocy

Amikacin can be added to blood to reduce the fall in platelet count.

Our objective was to develop an effective method to prevent the fall in platelet count for patients with anticoagulant-dependent (AD) pseudothrombocytopenia, a spurious phenomenon due to anticoagulant-induced aggregation of platelets. We report a case of insidious multianticoagulant-dependent pseudothrombocytopenia in which AD pseudothrombocytopenia may be caused by 4 anticoagulants, eg, EDTA, sodium citrate, heparin, and sodium fluoride (NaF). Multianticoagulant-dependent pseudothrombocytopenia was confirmed by finding clumped platelets on microscopic evaluation in 4 anticoagulated blood samp

National recommendations of the Croatian Chamber of Medical Biochemists and Working group for Laboratory hematology of the Croatian Society of Medical Biochemistry and Laboratory Medicine: Management of samples with suspected EDTA-induced pseudothrombocytopenia.

Pseudothrombocytopenia (PTCP) is defined by the occurence of spouriously low platelet count as a consequence of in vitro platelet aggregation. It is a rare and benign artifact, not associated with any specific disorder or therapy, that becomes clinically relevant when it is not timely and reliably recognized. Thus, it may result in inappropriate clinical decisions (i.e. unnecessary further testing, misdiagnoses and potential patients' mismanagement) unavoidably compromising patient safety. The most common form of PTCP is caused by ethylenediaminetetraacetic acid (EDTA). Several approaches for
