A pathway to personalization of integrated treatment: informatics and decision science in psychiatric rehabilitation.

Personalization of treatment is a current strategic goal for improving health care. Integrated treatment approaches such as psychiatric rehabilitation benefit from personalization because they involve matching diverse arrays of treatment options to individually unique profiles of need. The need for personalization is evident in the heterogeneity of people with severe mental illness and in the findings of experimental psychopathology. One pathway to personalization lies in analysis of the judgments and decision making of human experts and other participants as they respond to complex circumstan

[Rehabilitation of psychiatric patients in the halfway house--a 10-year retrospective study].

In a period of ten years 159 residents were admitted to a transitional home. 90% of them were diagnosed as chronic schizophrenics with a marked psychic and social handicap. With an average age of 28.7 years and a mean duration of illness of 6.8 years most of the patients had been hospitalized several times; 95% had no job, lived on social assistance or were invalidity pensioners. There was poor success in vocational rehabilitation (22%). Better results were seen in solving the housing problems, showing a clear-cut tendency to move from the family of origin into protected group homes or indepen

[Rehabilitation of psychiatrically ill and handicapped patients--historical, conceptual and scientific aspects].

Since the early days in the 19th century, psychiatry has been dealing with chronic courses of illness and mental disabilities. The 1975 German "Psychiatry Enquete Report" has played a major role in achieving progress in the care system, allowing mentally ill patients with impending or established handicaps to be rehabilitated or to have long-term care of varying intensity and even at home. In particular, differentiated and graduated systems of sheltered accommodation, a sheltered labour market and leisure-time support have been created. Rehabilitation may induce a response in the disabled pati

Digital psychoeducational rehabilitation supported by serious games for psychiatric inpatients: a protocol study.

INTRODUCTION: Schizophrenia spectrum disorders (SSD) are characterized by impairments in thought, perception, emotion, and behavior, leading to reduced personal and social functioning. While pharmacological treatments have limited efficacy on negative and cognitive symptoms, psychosocial interventions play a crucial role but remain difficult for many patients to access, particularly among youths. Digital interventions, particularly serious games, offer a promising approach to enhance recovery by improving accessibility, engagement, and motivation. The present protocol study aims to develop and

[Comorbid psychiatric disorders in cancer patients in acute inpatient treatment and medical rehabilitation].

An association between mental disorders, especially affective and anxiety disorders, and cancer has been reported in many clinical studies with inpatients. However, no data exist about the prevalence of mental disorders in patients undergoing acute care or rehabilitation treatment for cancer in Germany. The present study investigated 4-week, 6-month and lifetime prevalence rates of comorbid mental disorders in cancer. 256 patients from 2 rehabilitation clinics and 2 acute care hospitals were examined with standardized screening scales for psychological burden (GHQ-12, HADS) and quality of life

Social cognition moderates the influence of child physical abuse on inpatient psychiatric rehabilitation.

Moderating effects of social cognition in the relationship between the severity of history of child physical abuse (CPA) and social functioning were examined using 12-month longitudinal data among 143 participants with severe mental illness (SMI) in an inpatient psychiatric rehabilitation program. The adverse effects of the severity of history of CPA on social functioning were compensated for by greater social inference and lower external locus of control. Specifically, despite the severity of history of CPA, individuals with intact or greater social inference showed greater social functioning

Psychodynamic accessibility: a testable framework for supported agency in social psychiatry and psychiatric rehabilitation.

Recovery-oriented social psychiatry and psychiatric rehabilitation increasingly emphasize rights, participation, community-based care, supported decision-making, supported employment, and prevention-oriented early intervention. Yet formal access to services, information, rehabilitation, legal-administrative procedures, or participation formats does not by itself ensure that people can use these resources when shame, mistrust, affective overload, fatigue, cognitive narrowing, or structural vulnerability are already present. This Hypothesis and Theory article introduces psychodynamic accessibili

The feasibility of implementing cognitive remediation for work in community based psychiatric rehabilitation programs.

OBJECTIVE: Adding cognitive remediation to vocational rehabilitation services improves cognitive and work functioning in people with serious mental illness, but despite interest, the uptake of cognitive programs into community services has been slow. This study evaluated the feasibility of implementing an empirically supported cognitive remediation program in routine rehabilitation services at 2 sites. METHOD: The Thinking Skills for Work (TSW) program was adapted for implementation at 2 sites of a large psychiatric rehabilitation agency providing prevocational services, but not community-bas

A study on the socio-psychiatric aspects of short-term ARP (Alcohol Rehabilitation Program).

The duration of the majority of Alcohol Rehabilitation Programs (ARP) at major hospitals specializing in this treatment is three months. Recently, however, because of diverse backgrounds and patients needs, long periods of hospitalization have become difficult. In response to this, Okamotodai Hospital initiated a one-month ARP program three years ago. One feature of this program is that it can be freely structured to meet the requirements of individual patients. This study compares the prognosis of patients treated under this one-month program with that of those who have undergone the three-mo

[Evaluating work-personality insufficiency of schizophrenic patients; an assessment of employability in psychiatric rehabilitation].

Work-Personality means a person's motivation, values and attitude toward labor, general abilities which make his/her work performance efficient, which can be grasped through observation of behavior patterns in working situations. Problems connected with Work-Personality may cause schizophrenics some vocational difficulties. The author made out a "Work-Personality Insufficiency (WPI) Rating Scale" with 15 items, in order to estimate the employability of schizophrenics, and examined its inter-rater reliability by means of ANOVA-ICC and the results were sufficient enough. Then, WPI was applied to

An integrated system of community services for the rehabilitation of chronic psychiatric patients in Shenyang, China.

The evolution of the Zhengyang Community Mental Health Rehabilitation Centre described in this paper proves that community-based mental health services initiated and developed by enthusiastic and committed community members can be successful in the Chinese setting. The most important step is to utilise community resources to secure a stable source of income, preferably by establishing a profitable welfare enterprise that can both finance the provision of other services and provide work for mentally ill clients who cannot obtain regular employment. The ultimate goal is to reintroduce as many cl

Associations between methamphetamine use, psychiatric comorbidities and treatment outcome in two inpatient rehabilitation centers.

The use of methamphetamine is spreading globally and provokes the need for effective treatment options. Previous research showed increased psychiatric comorbidities in methamphetamine users, but its impact on treatment success is still unclear. This study investigates data from two German addiction rehabilitation centers including 108 methamphetamine using individuals. The participants were tested and interviewed at the beginning of the addiction treatment program and at the end of treatment after about six months. In total, 95% of the participants had at least another psychiatric diagnosis. A

[Occupational rehabilitation of chronic psychiatric patients. Results of a prospective study over 3 years].

Vocational rehabilitation of mentally ill patients is now focused outside the psychiatric hospital. Three types of programmes play a major role: outpatient work therapy programmes at psychiatric hospitals, jobs in (partly) sheltered employment facilities as in companies specially adapted for employing mentally ill persons, and workshops for the mentally disabled. In the north-western German region of Westphalia-Lippe (population: 9 million) we carried out a three-year prospective study on a regionally representative sample from such rehabilitation institutions to assess the course and outcome

[Clerical work therapy at a psychiatric hospital - a hitherto neglected rehabilitation aid (author's transl)].

As a rule, rehabilitation programmes provided at state psychiatric hospitals and sheltered workshops in the Federal Republic of Germany suffer from a lack of training possibilities for patients from commercial, administrative and clerical occupations. The situation is the same for school pupils and students. With the introduction of a work therapy concentrating on clerical activities into the rehabilitation programme of a psychiatric hospital, patients from jobs requiring particular intellectual skills can benefit from an appropriate and graded programme designed to re-develop previously train
