Influence of the lateral pterygoid muscle on traumatic temporomandibular joint bony ankylosis.

BACKGROUND: The pathogenesis of traumatic TMJ ankylosis remains unclear. This study aimed to verify the role of the lateral pterygoid muscle in the pathogenesis of traumatic temporomandibular joint (TMJ) bony ankylosis. METHODS: Eight 6-month-old male sheep were used in this study. Bilateral TMJ osteotomies were performed to induce sagittal fractures of the mandibular condyle. The lateral one-fourth segment of the disc was removed to establish a model of TMJ bony ankylosis. Subsequently, the function of the left and right lateral pterygoid muscles was blocked (experimental group) or maintaine

Non-infectious myositis of the lateral pterygoid muscle: a report of four cases.

Non-infectious myositis is a condition characterized by chronic localized myalgia originating from central nervous system effects. It is also known as centrally mediated myalgia associated with neurogenic inflammation. When this condition occurs in the lateral pterygoid muscle, clinical evaluation is difficult due to its inaccessible anatomic location. In order to diagnose this rare condition, careful clinical examination and advanced imaging are necessary. The authors report herein four cases of non-infectious myositis of the lateral pterygoid muscle diagnosed by magnetic resonance or enhance

Positional changes of the masseter and medial pterygoid muscles after surgical mandibular advancement procedures: an MRI study.

This study evaluated whether surgical mandibular advancement procedures induced a change in the direction and the moment arms of the masseter (MAS) and medial pterygoid (MPM) muscles. Sixteen adults participated in this study. The sample was divided in two groups: Group I (n=8) with a mandibular plane angle (mpa) <39° and Group II (n=8) with an mpa >39°. Group I patients were treated with a bilateral sagittal split osteotomy (BSSO). Those in Group II were treated with a BSSO combined with a Le Fort I osteotomy. Pre- and postoperative direction and moment arms of MAS and MPM were compared in th

Effect of suprahyoid muscles on mouth opening with and without lateral pterygoid muscle: 3D inverse dynamic model analysis.

Objective: To evaluate the contribution of suprahyoid muscles to mouth opening in different hyoid bone positions. Methods: The jaw-opening and closing movements were imposed on the 3D inverse dynamic jaw model with and without the lateral pterygoid muscle (LPM). The activation of the muscles was evaluated for eight different positions of the hyoid bone. Results: The muscles with 100% activation provided maximum mouth opening (MMO). When the hyoid bone was replaced, the muscles could achieve MMO at the 135º, 180º, and 225º points with the LPM. Mouth opening was estimated to be 36.8 mm in the abse

Evaluation of lateral pterygoid muscles in painful temporomandibular joints by signal intensity on fluid-attenuated inversion recovery images.

OBJECTIVES: Pathological changes of the lateral pterygoid muscle (LPM) have been investigated using various modalities, including magnetic resonance (MR) imaging and electromyography. Fluid-attenuated inversion recovery (FLAIR) is an MR sequence that we hypothesized can be used to evaluate abnormalities of the LPM. The purpose of this study was to analyze the FLAIR signal intensity of the LPM in painful temporomandibular joints (TMJs) and investigate the pathological changes of the muscle. METHODS: The study was based on 149 TMJs of 77 patients who were referred for MR imaging of the TMJ. Pat

Presentation of cysticercosis of the lateral pterygoid muscle as temporomandibular disorder: A diagnostic and therapeutic challenge.

Orofacial pain can often be the chief complaint of many systemic disorders. Cysticercosis involving the lateral pterygoids may cause limitation of mouth opening and may mimic clinical symptoms of a temporomandibular disorder. A 37-year-old female presented with 1-month-old complaint of limited mandibular range of motion. She reported a similar episode a year earlier and was diagnosed with a temporomandibular joint disorder by her primary dentist. Comprehensive intra- and extra-oral examinations were performed, which revealed a limitation of mouth opening accompanied by mild limitation of contr

A novel method for intraoral access to the superior head of the human lateral pterygoid muscle.

BACKGROUND: The uncoordinated activity of the superior and inferior parts of the lateral pterygoid muscle (LPM) has been suggested to be one of the causes of temporomandibular joint (TMJ) disc displacement. A therapy for this muscle disorder is the injection of botulinum toxin (BTX), of the LPM. However, there is a potential risk of side effects with the injection guide methods currently available. In addition, they do not permit appropriate differentiation between the two bellies of the muscle. Herein, a novel method is presented to provide intraoral access to the superior head of the human L

Myositis ossificans traumatica of the medial pterygoid muscle following a third molar extraction.

Myositis ossificans (MO) is a rare disease involving heterotopic ossification in the muscle or soft tissue. Myositis ossificans traumatica (MOT) disease presents as a calcification within the injured muscle, resulting from a single or repetitive injury. There are few reports of MOT in the masticatory muscles. The case of a patient with MOT in the medial pterygoid muscle caused by a complication related to the extraction of an erupted upper third molar is reported. The major symptom was severe trismus. Despite surgical treatment, the disease relapsed. MOT can lead to serious consequences for th

Intraoral myotomy of the lateral pterygoid muscle for treatment of recurrent dislocation of the mandibular condyle.

This article describes an intraoral procedure for myotomy of the lateral pterygoid muscle as treatment of recurrent dislocation of the mandibular condyle and five treated cases are presented. In none of the cases were anatomic structures damaged, and none of the patients has developed recurrence. The main effect of this treatment is ascribed to scarring anterior to the joint capsule, limiting excursion of the condyle. Although the follow-up is limited, the results seem promising.

Relationship between dimensions of muscles of mastication (masseter and lateral pterygoid) and skeletal dimensions: study of 40 cases.

INTRODUCTION: The masticatory muscles play an important part in determining the morphology of the facial skeleton. Skeletal typology and the characteristics of the masticatory muscles are closely linked. Several authors have studied muscle characteristics as related to facial typology. The aim of this work is to study the relationship between vertical and transverse skeletal dimensions and the dimensions (length, width and thickness) of two muscles of mastication, the masseter and the lateral pterygoid. MATERIALS AND METHOD: Our study was based on CT-scan examinations of a sample composed of

Management and Prognostic Implications of Posteriorly Advanced OSCC in Contact With the Medial Pterygoid Muscle.

BACKGROUND: T4b was previously defined as unresectable; however, it has been demonstrated that compartmental surgery can achieve treatment outcomes comparable to those of T4a cases. We frequently encounter posteriorly advanced oral squamous cell carcinoma (OSCC) in contact with the medial pterygoid muscle, and determining the appropriate extent of resection and managing subsequent recurrence in the masticator space remain challenging. Therefore, this study aimed to investigate the association between the tumor's spatial relationship with the medial pterygoid muscle and patient outcomes, provid

A Novel Injection Technique to the Lateral Pterygoid Muscle for Temporomandibular Disorders: A Cadaveric Study.

BACKGROUND: Lateral pterygoid muscle activity is associated with the pathological mechanisms of some temporomandibular disorders. The authors aimed to define and demonstrate a novel, practical, and safe technique for botulinum toxin type A injection to the lateral pterygoid muscle based on their findings. Their secondary aims were to standardize the injection pattern according to the variations of the lateral pterygoid muscle and its surrounding anatomical structures, and to establish its advantages over intraoral injection. METHODS: Twenty cadaver heads were dissected. The lateral pterygoid

Chronic stress effects in contralateral medial pterygoid muscle of rats with occlusion alteration.

Temporomandibular disorder (TMD) has a high prevalence in our society, characterized by a severe pain condition of the masticatory muscles and temporomandibular joint. Despite the indication of multiple factor initiators of TMD, there is still controversy about its etiology and its pathophysiology is poorly understood. Using rats as experimental animals we investigated the effect of unpredictable chronic stress with or without unilateral molar extraction on the contralateral medial pterygoid muscle. Our hypothesis is that these two factors induce changes in morphology, oxidative metabolism and

MRI findings concerning the lateral pterygoid muscle in patients with symptomatic TMJ hypermobility.

Clinical studies have shown a close association between temporomandibular joint hypermobility (TMJH) and temporomandibular disorders (TMD). While pathological change of the lateral pterygoid muscle (LPM) is one of the most emphasized in studies of TMD, there have been no detailed clinical reports of the LPM studies using magnetic resonance imaging (MRI) in TMJH. This study investigates structural and pathological alterations involving the LPM in patients with TMJH using MRI. A retrospective analysis was made of high-field MRI images from 98 patients with TMJH. LPMs of 143 joints were analyzed.

Myositis ossificans of the left medial pterygoid muscle: case report and review of the literature of myositis ossificans of masticatory muscles.

Myositis ossificans is a disease that is characterized by nonneoplastic, heterotopic bone formation within a muscle. Myositis ossificans traumatica, also called myositis ossificans circumscripta, is a disease in which muscles are ossified presumably following acute trauma, burns, surgical manipulation, or repeated injury. It is often remitted after surgical excision though some patients have repeated recurrences. Myositis ossificans traumatica of masticatory muscles is not frequently reported in the literature, with the most common clinical finding being a progressive limitation of motion in t
