[The study on the microsurgical anatomy of maxillary sinus approach pterygopalatine fossa].

OBJECTIVE: To provide morphologic data for the operation of pterygopalatine fossa (PPF) region by measuring the bony landmarks and dissecting vessel and nerve. METHOD: The shape, size and adjacent important structures of PPF were observed in 20 dry skulls (forty sides). Ten (twenty sides) skulls were fixed in 4% paraformaldehyde and their arteries were perfused with red lacteprene. The pterygopalatine fossa was exposed from the middle sagittal section dissected to inner wall of maxillary sinus and PPF and observed the course of trigeminal nerve and arteria maxillaris under the operating micro

A unique case of an epidermoid cyst of the pterygopalatine fossa and its management.

Epidermoid cysts are rare benign tumors that are derived from the development of abnormally situated ectodermal tissue. Although they may originate from any part of the body head and neck region is occasionally affected. Epidermoid cyst originating from pterygopalatine fossa has not been reported previously in the literature. We report the first case of epidermoid cyst of pterygomaxillary fossa in this study.

Primary nasal-type natural killer/T-cell lymphoma of pterygopalatine fossa.

Primary tumors that originated from the pterygopalatine fossa are seen rarely. In this paper, the case of a 69-year-old woman who complained of a 2-month history of headache, extending to the left part of the face, teeth, and shoulder, is reported. She had undergone right total mastectomy and axillar dissection of a spindle cell-type metaplastic carcinoma of the breast 1 year ago. Computed tomographic and magnetic resonance images showed a mass originating from the pterygopalatine fossa. Pathologic examination of the biopsy specimen of the mass revealed a nasal-type extranodal natural killer/T

Transnasal endoscopic management of angiofibroma extending to pterygopalatine and infratemporal fossae.

INTRODUCTION: Surgical approaches to the pterygopalatine and infratemporal fossae are complex and cause significant morbidity. The commonest benign tumour to extend to the pterygopalatine and infratemporal fossae is angiofibroma. PATIENTS AND METHODS: This prospective study included 15 male patients aged 12-27 years with recurrent, severe epistaxis. After computed tomography and magnetic resonance imaging, a modified Wormald and Robinson's two-surgeon approach was used. Follow up, with endoscopy and magnetic resonance imaging, ranged from two to five years. RESULTS: Twelve patients were cure

[A comparative study of the pterygopalatine fossa by computed tomography and magnetic resonance].

The pterygopalatine fossa (PPF) is a narrow, bone lined space containing many major cranial nerves and vessels connecting it to the middle and infratemporal cranial fossa, the oral, nasal and orbital cavities and the rhinopharynx. The internal maxillary artery, the second branch of the V cranial nerve and the sphenopalatine ganglion are the main structures which are found in the PPF. Because of its strategic site, the PPF can be involved in many pathologic conditions originating from the nasal fossa, the orbit, the paranasal sinuses or the rhinopharynx roof. Severe craniofacial trauma can invo

Trigeminal neuralgia as a consequence of circulatory disorders in the infratemporal and pterygopalatine fossae.

An extensive review of the well documented experimental and clinical findings related to trigeminal neuralgia shows, that it is possible to set up an unifying theory for trigeminal neuralgia, which would explain most clinical features of the disease and also would be congruent with several other theories that have previously been presented. Thus, trigeminal neuralgia is proposed to be the result of a combination of predisposing anatomical variations, impaired arterial and venous circulation, and morphological changes in the infratemporal fossa and cavum Meckeli. The process is initiated by dis

Endoscopic transorbital transconjunctival approach to the pterygopalatine fossa: anatomical cadaver study.

OBJECTIVE: This study aimed to provide an anatomical description of the endoscopic transorbital (ETO) transconjunctival approach to the pterygopalatine fossa (PPF). METHODS: Five formalin-fixed, silicone-injected human cadaver heads were studied at the Rhoton Anatomy Laboratory, Bahçeşehir University Faculty of Medicine, Istanbul, Turkey. By using the ETO transconjunctival route, the authors performed dissections on 10 PPFs (both orbits of each specimen). A 0° Olympus endoscope, along with a fiberoptic system, light source, camera, and digital video recording, was used for documentation. RES

Non-invasive treatment of pterygopalatine fossa schwannoma with gamma knife radiosurgery.

BACKGROUND: Surgical resection for pterygopalatine fossa schwannomas can be challenging due to the complex anatomy and potential morbidity. Gamma knife radiosurgery (GKRS) offers a minimally invasive alternative with precise targeting and minimal damage to surrounding structures. CASE REPORT: A 21-year-old female patient who had a history of progressive left-sided facial numbness over the past year and was diagnosed with progressive pterygopalatine fossa schwannoma underwent gamma knife radiosurgery. The radiological and clinical outcomes of the patient were evaluated over a period of 15 year

Modified Maxillary-Swing Approach for Resection of Primary Malignancies in the Pterygopalatine Fossa.

BACKGROUND: En bloc resection of malignancies in the pterygopalatine fossa (PPF) poses critical challenges. Using the modified maxillary-swing (MMS) approach, we achieved monobloc removal of primary malignancies in this region. This study provides a detailed account of the surgical techniques and indications used. METHODS: We enrolled seven patients with primary malignancies in the PPF during a period from January 2012 to January 2019 in this retrospective study. After malignancies were confirmed by biopsy as well as evaluation with computed tomography (CT) and magnetic resonance imaging (MRI

Preliminary results on a study to locate the pterygopalatine fossa using mathematical formulae.

Blocking the contents of the pterygopalatine fossa (PPF) has been shown to be effective in treating most orofacial pain including that associated with trigeminal neuralgia. However, the technique is not widely used, and we propose it to be due to the vague descriptions of the techniques in the literature. The aim of this study was therefore to achieve an alternative method of locating the PPF. One hundred and sixty skulls from the department of Anatomy, University of Pretoria, were used. Distinct landmarks (both anthropometric and clinical) accompanied by existing and new anthropometric measur

Wegener granulomatosis involving the pterygopalatine fossa: an unusual case of trigeminal neuropathy.

We report on a case of Wegener granulomatosis in the pterygopalatine fossa that was associated with trigeminal neuropathy. MR and CT examinations were useful in depicting the extent of the lesion and suggesting a perineural spread. Diagnosis was confirmed with positive serum assay findings for the presence of cytoplasmic antineutrophil cytoplasm antibody.

Fluoroscopy-guided suprazygomatic approach to pterygopalatine fossa block: a brief technical report.

BACKGROUND: The pterygopalatine fossa (PPF) is a compartment with rich autonomic and somatic innervation of the head, making it a crucial target for pain management interventions. Among the various techniques described, the suprazygomatic approach to the PPF may be one of the safest. However, the application of fluoroscopy to guide this approach has not been previously reported. METHODS: To the best of our knowledge, this is the first report in the literature describing a fluoroscopy-guided suprazygomatic approach to the PPF block. This brief technical report provides a step-by-step descripti

Bidirectional trigeminal perineural spread of adenoid cystic carcinoma via the pterygopalatine fossa.

Adenoid cystic carcinoma (ACC) is characterized by a strong propensity for perineural invasion (PNI), which complicates skull base evaluation and treatment planning. We report a case of buccal mucosal ACC demonstrating simultaneous proximal extension to the cavernous sinus and distal orbital involvement following infiltration of the pterygopalatine fossa (PPF). Contrast-enhanced magnetic resonance imaging (MRI) demonstrated continuous tumor propagation from the posterior superior alveolar nerve to the maxillary nerve (V2), with bidirectional spread originating from the PPF. Endoscopic surgical strategy for highly vascular pediatric sinonasal and pterygopalatine fossa tumors.

Pediatric sinonasal tumors are rare, and minimally invasive approaches are preferred because of unknown long-term effects on development. We report three cases of highly vascular pediatric sinonasal and pterygopalatine fossa tumors that were successfully removed using an endonasal endoscopic approach with transcatheter arterial embolization (TAE). The patients (9-11 years old) had contrast-enhanced tumors in the left ethmoid sinus near the sphenopalatine foramen in case 1, in the right nasal cavity near the sphenopalatine foramen in case 2, and in the right pterygopalatine fossa in case 3, wit

Injectate distribution patterns in posterior infrazygomatic and transoral approaches to the pterygopalatine fossa.

BACKGROUND: Injectate distribution patterns in the pterygopalatine fossa may differ based on the drug administration approach used. This study primarily aimed to assess and compare injectate distribution following the posterior infrazygomatic and transoral approaches. The secondary aim was to evaluate the safety of both approaches. METHODS: Injectate distribution patterns were evaluated in 13 cadaveric head specimens. The vessels were perfused with a gelatin-based solution containing an iodinated contrast agent. The ultrasound-guided posterior infrazygomatic approach and transoral approach we
