Interactions of lung stretch, hyperoxia, and MIP-2 production in ventilator-induced lung injury.

The use of positive pressure mechanical ventilation can cause ventilator-induced lung injury (VILI). We hypothesized that hyperoxia in combination with large tidal volumes (VT) would accentuate noncardiogenic edema and neutrophil infiltration in VILI and be dependent on stretch-induced macrophage inflammatory protein-2 (MIP-2) production. In rats ventilated with VT 20 ml/kg, there was pulmonary edema formation that was significantly increased by hyperoxia. Total lung neutrophil infiltration and MIP-2 in bronchoalveolar lavage (BAL) fluid were significantly elevated, in animals exposed to high

Bucillamine induced pulmonary injury occurs with immunoglobulin decrease.

We describe pulmonary involvement in 3 patients with rheumatoid arthritis treated with bucillamine. In all 3, synovitis was in remission. The common radiological characteristic was mottled dense infiltrates, in which were air bronchograms, in the bilateral central lung, sparing the periphery. Lung biopsies in 2 cases revealed organizing exudates in the alveolar ducts and alveoli, and cellular infiltration into alveoli and septa. Serum immunoglobulin (Ig) levels decreased markedly at the time of involvement. After discontinuation of bucillamine, lung injury resolved without exacerbation of syno

SOX2 and SOX21 in Lung Epithelial Differentiation and Repair.

The lung originates from the ventral foregut and develops into an intricate branched structure of airways, alveoli, vessels and support tissue. As the lung develops, cells become specified and differentiate into the various cell lineages. This process is controlled by specific transcription factors, such as the SRY-related HMG-box genes SOX2 and SOX21, that are activated or repressed through intrinsic and extrinsic signals. Disturbances in any of these processes during the development of the lung may lead to various pediatric lung disorders, such as Congenital Diaphragmatic Hernia (CDH), Conge

Four-dimensional visualization of subpleural alveolar dynamics in vivo during uninterrupted mechanical ventilation of living swine.

Pulmonary alveoli have been studied for many years, yet no unifying hypothesis exists for their dynamic mechanics during respiration due to their miniature size (100-300 μm dimater in humans) and constant motion, which prevent standard imaging techniques from visualizing four-dimensional dynamics of individual alveoli in vivo. Here we report a new platform to image the first layer of air-filled subpleural alveoli through the use of a lightweight optical frequency domain imaging (OFDI) probe that can be placed upon the pleura to move with the lung over the complete range of respiratory motion.

111In-chloride thorax scintigraphic study for demonstration of pulmonary edema: canine model.

Pulmonary edema induced by injections of oleic acid was demonstrated using 111In-chloride lung/heart image in five anesthetized dogs. The anesthetized dogs were positioned under a gamma camera interfaced to a computer. After 111In-chloride IV injections, dynamic data were recorded at 1 frame/min before and after induced pulmonary edema. The computer generated the curve of the lung-to-heart activity ratio; the ratio rose from 0.5-0.6 at baseline to 0.85-1.5 at the end of the study in the dogs. At the end of each study, the removed lungs demonstrated edematous fluid, and a few red blood cells we

A study of spectrum of pulmonary pathology and expression of thyroid transcription factor-1 during neonatal period.

CONTEXT: Neonatal period is the single most hazardous period of life. The major causes of neonatal death are prematurity and respiratory distress syndrome. We report a series of neonatal autopsies in our Neonatal Intensive Care Unit with special emphasis on pulmonary pathology. The spectrum of pathological changes in the lungs and thyroid transcription factor-1 (TTF-1) expression was studied in detail with reference to its spatial distribution. AIMS: This study aims to analyze the causes of neonatal death with special attention to pulmonary pathology along with associated histopathological ch

A computational physiological model of acute respiratory distress syndrome patients for positive end-expiratory pressure titration.

BACKGROUND AND OBJECTIVE: Patients with acute respiratory distress syndrome (ARDS) experience alveolar collapse, leading to reduced ventilation and blood oxygenation. Clinical treatment with mechanical ventilation involves the use of positive end-expiratory pressure (PEEP) to recruit collapsed alveoli. The identification of an optimal PEEP level has remained a subject of debate in critical care for over 50 years. This work proposes a 0D cardiopulmonary computational physiological model (CPM) to investigate the issue of PEEP titration by studying the impact of PEEP on the cardiopulmonary system

Pulmonary alveolar proteinosis in a cat.

BACKGROUND: Pulmonary alveolar proteinosis is an extremely rare lung disease in animals and humans. It is characterized by the deposition of a large amount of phospholipoproteinaceous material in the alveoli. There are several possible etiologies, both congenital and acquired. Alveolar macrophages play an important role in the clearance of surfactant. This is the first report of pulmonary alveolar proteinosis in the feline species. CASE PRESENTATION: Pulmonary alveolar proteinosis was diagnosed in an 8-month-old cat with chronic tachypnea, failure to thrive and finally respiratory distress. T

Misalignment of lung vessels: a syndrome causing persistent neonatal pulmonary hypertension.

The cases of two female infants with persistent neonatal pulmonary hypertension are described. Combinations of pulmonary parenchymal lesions were present, including underdevelopment of alveoli and interstitial fibrosis as well as misalignment of lung vessels. As a result of the misalignment, the pulmonary veins joined the pulmonary arteries, rather than following a course away from them. One of the infants had additional congenital malformations. This syndrome should be considered in the evaluation of infants with persistent fetal circulation.

[Histologic changes in lungs of mice experimentally infected with amoebae of the "limax" group].

The studies revealed that the infection with the "limax" group amoebae resulted in the severe changes in the respiratory system: in interstitial pneumonia, the injuries of the bronchi, the bronchioles and the blood vessels. The histopathological examinations showed the focal destruction of the alveoli in case of the mild amoeba invasion while the massive invasion caused the complete destruction of the respiratory epithelium, the blood vessels, the bronchi and the bronchioles. Apart from all the changes in the alveoli, in most cases the significant pulmonary hyperaemia and the numerous blood fo

Pulmonary alveolar microlithiasis: A case report and brief review of literature.

BACKGROUND: Pulmonary alveolar microlithiasis (PAM), a rare autosomal recessive pulmonary disease, is mainly characterized by extensive calcium phosphate microliths deposition in the alveoli. The major mutation, causing the characteristic of this disease, occurs in solute carrier family 34 members 2 (SLC34A2), which is placed on chromosome 4p15.2. SLC34A2 encodes sodium-phosphate cotransporter type IIb, NPT2b, which plays a critical role in the transportation of phosphate ions from pulmonary alveoli into type II pneumocytes. CASE PRESENTATION: Herein, we have reported a 50-year-old male prese

Pulmonary vasculitis (Wegener's granulomatosis). Immunohistochemical study of T and B cell markers.

A case of pulmonary vasculitis (Wegener's granulomatosis) is reported in which immunoperoxidase studies demonstrated that the vascular lymphoid infiltrates were composed predominantly of T cells and monocytes. Occasional T cells, B cells and monocytes were identified in the alveolar septa. IgG, IgA, IgM, and C3 were not identified in pulmonary vessels, septa, or alveoli. These findings suggest that cellular rather than humoral-immune mechanisms predominate in pulmonary vasculitis.

Effects of inhaled chromium dioxide dust on rats exposed for two years.

Rats were exposed by inhalation to chromium dioxide (CrO2) dust at design concentrations of 0, 0.5 mg/m3 (stabilized and unstabilized, respectively), or 25 mg/m3 (stabilized) for 6 hr/day, 5 days/week for 2 years. No dust-exposure-related pathological changes other than lung lesions were observed in all exposed rats. There were no significant differences in pulmonary responses between unstabilized and stabilized CrO2 at the 0.5 mg/m3 exposure level. The lungs showed minute dust deposition in the alveoli adjacent to the alveolar ducts, but maintained an intact general architecture. The pulmonar

Clinicopathological studies on pulmonary hypoplasia in very low birth weight infants.

Seven VLBW infants with pulmonary hypoplasia (P.H.) were studied retrospectively and their necropsied lungs were analyzed morphologically and histometrically in an attempt to establish the clinical and pathological criteria of P.H. Clinically they had various features as follows; 1) causes with oligohydramnios due to amniotic fluid leakage, 2) bell-shaped chest and/or small lung radiographically, 3) severe respiratory distress immediately after birth, 4) frequent complications with air leaks, lethal within several hours after birth. Pathologically they showed a series of changes; 1) remarkably

Pathophysiology of light phenotype SARS-CoV-2 interstitial pneumonia: from histopathological features to clinical presentations.

Little is known about the light phenotype of SARS-CoV-2 pneumonia, which behaves in an unusual way, unlike other known respiratory diseases. We believe that the histopathological features of early COVID-19 could be considered the pathophysiological hallmark of this disease. Lung cryobiopsies show almost pristine alveoli, enlarged/hyperplasic alveolar capillaries along with dilatation of the post capillary pulmonary venules. Hypoxemia could therefore be explained by a reduction of the normal V/Q ratio, due to blood overflow around well ventilated alveoli. This could clarify typical manifestatio

The efficacy and safety of KL4-surfactant in preterm infants with respiratory distress syndrome.

The present study was undertaken to determine if a synthetic peptide, KLLLLKLLLLKLLLLKLLLLK (KL4), in which K = lysine and L = leucine, in an aqueous dispersion of phospholipids (DPPC and POPG), would expand pulmonary alveoli and improve gas exchange in premature human infants with respiratory distress syndrome (RDS). The KL4 peptide was synthesized to resemble the amino acid pattern of surfactant protein B (SP-B). Forty-seven infants with RDS were treated within 4 h of birth with the KL4-peptide/phospholipid mixture, called KL4-Surfactant. The average arterial-to-alveolar oxygen tension ratio

Pulmonary mucosa-associated lymphoid tissue lymphoma: CT and pathologic findings.

OBJECTIVE: The objective of this study was to determine the CT findings in pulmonary mucosa-associated lymphoid tissue lymphoma and to correlate these CT findings with histologic specimens. CONCLUSION: In the 11 patients reviewed, the most common CT appearance of pulmonary mucosa-associated lymphoid tissue lymphoma was consolidation with air bronchograms, correlating histologically with a cellular lymphocytic infiltrate expanding the interstitium and compressing adjacent alveoli, producing air bronchograms.

Pulmonary alveolar microlithiasis with pectus excavatum. Case report.

Pulmonary alveolar microlithiasis is a rare disease of unknown etiology. There is a formation of calcific bodies within the alveoli. The X-Ray shows extensive infiltration of both lungs. So far less than 100 cases were reported. However, we are unware of a case with pectus excavatum. In this report we discuss a case of pulmonary alveolar microlithiasis with pectus excavatum, and its treatment and medical intervention.

Diffuse smooth muscle proliferation of the lungs with severe pulmonary hypertension.

We describe the second example of a distinctive clinicopathologic syndrome of severe pulmonary hypertension and interstitial lung disease caused by diffuse smooth muscle proliferation of the lungs (DSMPL) occurring in a 44-year-old man of normal intelligence with no cutaneous or neurological lesions. The smooth muscle proliferation involved the walls of bronchioles, alveoli, small pulmonary arteries, and pulmonary veins. DSMPL is distinct from pulmonary lymphangioleiomyomatosis and tuberous sclerosis, and does not appear to be neoplastic or reactive. The lesions are probably best regarded as m
