[The different types of dyspnoea].

Dyspnoea can be classified into four types with each arising by means of a different mechanism. Air hunger originates from chemoreflex activity. It is inhibited by pulmonary inflation, probably through a reflex induced by stretch receptors in the large airways. The feeling of increased work of breathing particularly occurs during volitional breathing and is augmented by every cause of increased work of breathing or muscle weakness. This feeling is probably induced by activity of the cerebral motor cortex. Tightness of the chest can be due to bronchospasm. This feeling possibly arises from stim

Pharmacological and electrophysiological analysis of nicotine induced apnoea in the cat.

Intravenous nicotine (20-60 micrograms/kg) produced an initial brief apnoea followed by hyperventilation in anaesthetized cats. The apneic response to nicotine remained uneffected by atropine, by phentolamine or propranolol. Hexamethonium and guanethedine sulphate antagonized the apneic response. In bilateral vagotomized cats, nicotine failed to produce respiratory apnoea. Veratridine and phenyldiguanide produced apnoea similar to that produced by nicotine within 2-3 sec. administered intraartrially. Nicotine failed to stimulate pulmonary stretch receptors as did veratridine in artificially ve

Velopharyngeal closure pattern exerted by contraction of the pharyngeal constrictor muscle in dogs.

OBJECTIVE: This study was performed to investigate aspects of velopharyngeal closure demonstrated by movements of the parapharyngeal wall under various physiological conditions and especially the stimulation of several intrapneumonal receptors. MATERIALS AND METHODS: Fifteen mongrel dogs were anesthetized and a nasopharyngeal fiberscope was inserted from the oronasal fistula at the center of the hard palate, with the tip fixed around the choana. Mechanical and chemical stimulation was applied, and ongoing velopharyngeal movements were observed. In each case examined, the muscle nerve located

Bronchopulmonary afferent nerves.

Vagal afferent nerves are the primary communication pathways between the bronchopulmonary system and the central nervous system. Input from airway afferent nerves to the CNS is integrated in the brainstem and ultimately leads to sensations and various reflex outputs. Afferent nerves innervating the airways can be classified into various distinct phenotypes. However, there is no single classification scheme that takes all features, including conduction velocity, cell body diameter, ganglionic origin, and stimuli to which they respond (modality) into account. At present, bronchopulmonary afferen

A comparative study of bronchopulmonary slowly adapting receptors between rabbits and rats.

Pulmonary mechanosensory receptors provide important inputs to the respiratory center for control of breathing. However, what is known about their structure-function relationship is still limited. In these studies, we explored this relationship comparing bronchopulmonary slowly adapting receptor (SAR) units in rabbits and rats. In morphological studies, sensory units in tracheobronchial smooth muscle labeled with anti-Na+ /K+ -ATPase (α3 subunit) were found to be larger in the rabbit. Since larger structures may result from increased receptor size or more numerous receptors, further examinatio

Reflexes from airway rapidly adapting receptors.

Rapidly adapting receptors (RARs) occur throughout the respiratory tract from the nose to the bronchi. They have thin myelinated nerve fibres, an irregular discharge and adapt rapidly to a maintained volume stimulus, but often slowly to a chemical stimulus. They are polymodal, responding to mechanical and chemical irritant stimuli, and to many inflammatory and immunological mediators. RARs show very varied sensitivities to different stimuli, and diverse reflex responses. Those in the larynx are usually called 'irritant' receptors. They probably cause cough, the expiration reflex and other lary

Contribution of pulmonary receptors to the heart rate response to acute hypoxemia in rabbits.

We studied the effect of pulmonary afferent activity on the heart rate response to a progressive, isocapnic decrease in oxygen saturation (SaO2) in anesthetized rabbits. To abolish the effect of rapidly adapting receptors, we used inhaled bupivacaine aerosol, and to abolish the effect of slowly adapting stretch receptor activity, we used sulfur dioxide insufflation. The heart rate (HR) response (delta HR/delta SaO2) under control conditions was 0.39 +/- 1.29 beats/min/% (mean +/- SD, n = 11; values greater than 0 indicate a tachycardiac response to hypoxemia). After sulfur dioxide insufflation

The effect of sulfur dioxide on the response of rabbits to expiratory loads.

The role of slowly adapting pulmonary stretch receptors (PSR) in the reflex control of expiratory duration was investigated in anesthetized, spontaneously breathing rabbits. Graded resistive and elastic loads were applied for single expirations to produce expiratory volume-time curves. The PSR were selectively blocked by administration of 200-400 ppm sulfur dioxide (SO2) to the inspired air of the rabbits. Volume-time curves were generated for pre-treatment, SO2 exposure, recovery and post-bilateral cervical vagotomy. SO2 exposure significantly reduced the volume-dependent modulation of expira

Control of breathing in asthma.

The hyperventilation seen in asthma is due to an increase in respiratory drive. This may be reflected by increased neural output to the respiratory muscles and probably by alterations in the respiratory pattern. This increased respiratory drive is most likely in response to irritant receptor stimulation in the lung. Our understanding of the genesis of hyperventilation and an abnormal breathing pattern in asthma has paralleled in growth in our understanding of the mechanism of respiratory control. New methods for studying the control of respiration have been applied to the clinical problem of a

Deflation-activated receptors, not classical inflation-activated receptors, mediate the Hering-Breuer deflation reflex.

Many airway sensory units respond to both lung inflation and deflation. Whether those responses to opposite stimuli come from one sensor (one-sensor theory) or more than one sensor (multiple-sensor theory) is debatable. One-sensor theory is commonly presumed in the literature. This article proposes a multiple-sensor theory in which a sensory unit contains different sensors for sensing different forces. Two major types of mechanical sensors operate in the lung: inflation- and deflation-activated receptors (DARs). Inflation-activated sensors can be further divided into slowly adapting receptors

Analysis of the cardiovascular changes induced in the rat by graded levels of systemic hypoxia.

1. In rats anaesthetized with Saffan, we have further analysed the respiratory, cardiac and regional vascular responses induced by 3 min periods of graded hypoxia (breathing 15, 12, 8 or 6% O2 in N2). 2. Frequently, hypoxia evoked an episode, lasting 1-1.5 min, of tachycardia, renal and mesenteric vasoconstriction and skeletal muscle vasodilatation. The tachycardia and muscle vasodilatation persisted after vagotomy indicating they were not initiated by pulmonary stretch receptors secondary to hyperventilation. We propose that such episodes represented the cardiovascular components of the alert

Cardiorespiratory interactions in neural circulatory control in humans.

The reflex mechanisms and interactions described in this overview provide some explanation for the range of neural circulatory responses evident during changes in breathing. The effects described represent the integrated responses to activation of several reflex mechanisms, including peripheral and central chemoreflexes, arterial baroreflexes, pulmonary stretch receptors, and ventricular mechanoreceptors. These interactions occur on a dynamic basis and the transfer characteristics of any single interaction are, in all likelihood, also highly dynamic. Nevertheless, it is only by attempting to u

Hypertonic saline stimulates vagal afferents that respond to lung deflation.

In our present studies, we seek to determine whether increased osmolarity stimulates deflation-activated receptors (DARs). In anesthetized, open-chest, and mechanically ventilated rabbits, we recorded single-unit activities from typical slowly adapting receptors (SARs; responding only to lung inflation) and DAR-containing SARs (DAR-SARs; responding to both lung inflation and deflation) and identified their receptive fields in the lung. We examined responses of these two groups of pulmonary sensory units to direct injection of hypertonic saline (8.1% sodium chloride; 9-fold in tonicity) into th

Cardiovascular and respiratory effects of adenosine in humans after pulmonary denervation.

Adenosine infusion causes tachycardia in normal subjects. A proposed mechanism is secondary pulmonary stretch receptor activation due to hyperventilation, induced by direct stimulation of carotid body chemoreceptors by adenosine. We examined responses to adenosine (incremental doses given by intravenous infusion, 5 min each rate) in six normal volunteers and six lung-transplant recipients with pulmonary denervation. Adenosine caused chemoreceptor stimulation in both groups with minute ventilation increasing from 5.8 +/- 0.4 to 10.7 +/- 1.3 l/min (P < 0.05) in the volunteers and from 4.8 +/- 0.

Effect of chronic pulmonary denervation on ventilatory responses to exercise.

To assess the role of intrapulmonary receptors on the ventilatory responses to exercise we studied six beagle dogs before and after chronic pulmonary denervation and five dogs before and after sham thoracotomies. Each exercise challenge consisted of 6 min of treadmill exercise with measurements taken during the third minute at 3.2 km/h, 0% grade, and during the third minute at 5.0 km/h, 0% grade. Inspiratory and expiratory airflows were monitored with a low-dead-space latex mask and pneumotachographs coupled to differential pressure transducers. Both pre- and postsurgery, all dogs exhibited a

Differential modulation by pulmonary stretch afferents of some reflex cardioinhibitory responses in the cat.

1. Cats were anaesthetized with a mixture of chloralose and urethane, and were artificially ventilated, the thorax being opened via a medial sternotomy. 2. Various cardiovascular receptors were stimulated during reflex inhibition of the central inspiratory neurones produced by electrical stimulation of both superior laryngeal nerves simultaneously, while the afferent input from the lungs was held constant by temporarily interrupting artificial respiration, the lungs being held static in their expiratory position. 3. Reflex cardioinhibitory responses were elicited by stimulation of (a) the caro

Afferent pathways for the cough reflex.

Cough is most often a reflex act elicited by the stimulation of nervous receptors in the respiratory tract. The most important tussigenic areas are at the level of the larynx and the more distal portion of the trachea with its bifurcation. Laryngeal rapidly adapting receptors with myelinated fibres are promptly activated by tussigenic stimuli, but it is unlikely that they are the only ones responsible for laryngeal coughing. In fact, some evidence supports the involvement of laryngeal C-fibre receptors. Cough in response to inhalation of aqueous aerosols containing low chloride concentrations
