Comparison of Optic Nerve Sheath Diameter Measurements in Coronary Artery Bypass Grafting Surgery with Pulsatile and Non-Pulsatile Flow.

Background and Objectives: In coronary artery bypass grafting (CABG) surgeries, monitoring intracranial pressure (ICP) is crucial due to neurological risks. Although pulsatile flow (PF) during cardiopulmonary bypass (CPB) is considered more physiological than non-pulsatile flow (NPF), its impact on ICP remains unclear. This study aimed to compare preoperative and postoperative optic nerve sheath diameter (ONSD) measurements between PF and NPF techniques to evaluate their effect on ICP changes. Materials and Methods: Sixty patients undergoing elective CABG (aged 45-75 years, ASA II-III-IV) were

Analysis of pressure head-flow loops of pulsatile rotodynamic blood pumps.

The clinical importance of pulsatility is a recurring topic of debate in mechanical circulatory support. Lack of pulsatility has been identified as a possible factor responsible for adverse events and has also demonstrated a role in myocardial perfusion and cardiac recovery. A commonly used method for restoring pulsatility with rotodynamic blood pumps (RBPs) is to modulate the speed profile, synchronized to the cardiac cycle. This introduces additional parameters that influence the (un)loading of the heart, including the timing (phase shift) between the native cardiac cycle and the pump pulses

Bi-directional cavopulmonary shunt: is accessory pulsatile flow, good or bad?

OBJECTIVE: Evaluation of the effect and long-term outcome of accessory pulsatile blood flow versus classical bi-directional cavopulmonary connection (BCPC). METHODS: Retrospective review of the medical and surgical records. RESULTS: Two-hundred and five patients (119 boys, 86 girls) underwent BCPC from 1990 to 1996. Accessory pulsatile flow was present in 68%, flow being maintained through the pulmonary trunc in 46%, systemic-to-pulmonary artery shunt in 13% and mixed in 7%, or patent ductus arteriosus in 2%. Patients with accessory pulsatile flow had lower hospital mortality (3% versus 5%),

Conversion technique of pulsatile-to-continuous flow single ventricular assist device.

Patients with a congenital heart disease and unfavourable anatomy that are not amenable to the primary correction of the defect might need a ventricular assist device as a bridge to a transplant. Pulsatile paracorporeal ventricular assist devices represent excellent options for supporting these patients with complex conditions. The Berlin Heart EXCOR is a such a paracorporeal pulsatile ventricular device. Pulsatile flow is more physiologic because it resembles the natural circulation pumped by the heart. The Berlin Heart EXCOR is a displacement pump; therefore, the risk for increased haemolysi

Modelling the period-average transport of species within pulsatile blood flow.

Time-averaging is a technique that is often applied in models of the cardiovascular system to filter out temporal variations. In the context of modelling pulsatile blood flow, it is used to derive the time-invariant state, about which the flow periodically oscillates. It is known from past studies, that blood flow oscillations can interact to produce disturbances that directly influence the period-average of their local flow field. In this study, it is shown that the transport of blood-borne species also inherits disturbances that can influence its period-average. Over the course of a period o

Two-equation turbulence modeling of pulsatile flow in a stenosed tube.

The study of pulsatile flow in stenosed vessels is of particular importance because of its significance in relation to blood flow in human pathophysiology. To date, however, there have been few comprehensive publications detailing systematic numerical simulations of turbulent pulsatile flow through stenotic tubes evaluated against comparable experiments. In this paper, two-equation turbulence modeling has been explored for sinusoidally pulsatile flow in 75% and 90% area reduction stenosed vessels, which undergoes a transition from laminar to turbulent flow as well as relaminarization. Open-heart surgery in infants using pulsatile high-flow cardiopulmonary bypass.

Pulsatile high-flow cardiopulmonary bypass (2.5 l/m2/min with a rectal temperature of 28 degrees C) combined with the Pulsatile Bypass Pump (Kontron Instrument) has been used at Fukuoka Children's Hospital in 259 cases of open-heart surgery in patients less than 1 year of age for a period of 5 years beginning July 1982. The overall results were satisfactory with a surgical mortality of 6.2% (VSD: 96 cases/2 deaths, TGA: 48/1, TAPVD: 34/3, Complete ECD: 15/1, IAA: 10/1, DORV: 10/1, HLHS: 7/4, TOF: 6/0, Truncus Art: 5/0, Others 28/3). [Observation of CSF pulsatile flow in MRI--the signal void phenomenon].

In a comparative study of MR images of 289 neurosurgical patients, loss of the signal intensity (signal void phenomenon) of CSF in the aqueduct was observed in 77 patients. This signal void phenomenon was seen most frequently in infants with chronic subdural hematoma (12 of 18) and patients of all age groups suffering from communicating hydrocephalus (10 of 14). It is known that CSF in the cranial cavity flows toward the spinal CSF space in to and fro manner responding to brain parenchyma pulsations. The velocity of this flow is to be faster in the narrower parts through the ventricular system

Does pulsatility matter in the era of continuous-flow blood pumps?

Despite significant improved survival with continuous flow left ventricular assist devices (LVADs), complications related to aortic valve insufficiency, gastrointestinal bleeding, stroke, pump thrombosis, and hemolysis have dampened the long term success of these pumps. Evolution has favored a pulsatile heart pump to be able to deliver the maximum flow at different levels of systemic vascular resistance, confer kinetic energy to the flow of blood past areas of stenosis and generate low shear stress on blood elements. In this perspective, we suggest that lack of pulsatility may be one factor th

Neocortical capillary flow pulsatility is not elevated in experimental communicating hydrocephalus.

While communicating hydrocephalus (CH) is often characterized by increased pulsatile flow of cerebrospinal fluid (CSF) in the cerebral aqueduct, a clear-cut explanation for this phenomenon is lacking. Increased pulsatility of the entire cerebral vasculature including the cortical capillaries has been suggested as a causative mechanism. To test this theory, we used two-photon microscopy to measure flow pulsatility in neocortical capillaries 40 to 500 μm below the pial surface in adult rats with CH at 5 to 7 days (acute, n=8) and 3 to 5 weeks (chronic, n=5) after induction compared with intact c

Comparison of physiological and simple pulsatile flows through stenosed arteries.

Most experimental and numerical studies of pulsatile flow through stenosed arteries have been performed for a first harmonic oscillatory flow. In this paper, numerical solutions are presented for a physiological pulsatile flow as well as for an equivalent simple pulsatile flow, having the same stroke volume as the physiological flow, and the differences in their flow behavior are discussed. The analysis is restricted to laminar flow, Newtonian fluid and axisymmetric rigid stenosis. Comparison of results shows that the behaviors of the two flows are similar at some instances of time, however, i

Assessing cerebral microvascular pulsatility using flow pulsatile-resolved pseudo-continuous arterial spin labeling MRI.

PURPOSE: To develop a flow pulsatile-resolved pseudo-continuous arterial spin labeling (FPR-pCASL) technique for noninvasively assessing cerebral microvascular pulsatility. METHODS: In FPR-pCASL, a pCASL sequence with single-shot three-dimensional gradient and spin echo acquisition was performed while simultaneously recording cardiac pulse signals using a pulse oximeter, and the pCASL images were retrospectively binned into multiple cardiac phases to resolve pulsatile microvascular blood flow. Both numerical simulations and in vivo experiments were performed to investigate the confounding fac

A numerical and experimental investigation of transitional pulsatile flow in a stenosed channel.

In the present paper, a closely coupled numerical and experimental investigation of pulsatile flow in a prototypical stenotic site is presented. Detailed laser Doppler velocimetry measurements upstream of the stenosis are used to guide the specification of velocity boundary conditions at the inflow plane in a series of direct numerical simulations (DNSs). Comparisons of the velocity statistics between the experiments and DNS in the post-stenotic area demonstrate the great importance of accurate inflow conditions, and the sensitivity of the post-stenotic flow to the disturbance environment upst

Physiological pulsatile flow experiments in a model of the human aortic arch.

An experimental investigation of physiologically relevant pulsatile flow in a model of the human aortic arch has been conducted. The model aortic arch flow chamber was fabricated in clear acrylic from an in situ casting of the human aorta and was incorporated in a mock-circulatory system. The model excluded the coronary sinuses and the three major branching arteries of the mid-arch region in order to concentrate only upon the effects of the multi-dimensional curvatures and tapering in the aorta. Furthermore, a flow straightening section was placed upstream to the flow chamber to eliminate any

Changes in intracranial venous blood flow and pulsatility in Alzheimer's disease: A 4D flow MRI study.

Cerebral blood flow, arterial pulsation, and vasomotion may be important indicators of cerebrovascular health in aging and diseases of aging such as Alzheimer's disease. Noninvasive markers that assess these characteristics may be helpful in the study of co-occurrence of these diseases and potential additive and interacting effects. In this study, 4D flow MRI was used to measure intra-cranial flow features with cardiac-gated phase contrast MRI in cranial arteries and veins. Mean blood flow and pulsatility index as well as the transit time of the peak flow from the middle cerebral artery to the

[Mechanical model of ocular pulsatile flow for evaluating the ocular blood flow device with known pressure pulsations].

BACKGROUND: Ocular perfusion consists of steady-state and pulsatile components of flow. The latter can be measured clinically by means of the 'Ocular Blood Flow' (OBF)-device (O.B.F. Ltd, Crowshearst, GB). METHODS: 1) Mechanical 'eye': To mechanically simulate the effect of pulsatile flow in the eye, a mechanical 'eye' model was built: A brass chamber (9 cm3) was machined and fitted with in- and outflow connections. The front opening was covered with a taughtly fixed rubber membrane (COSANO, no. 5203.106, Migros AG, Zurich) which, as mechanical 'cornea', pulsated with changes in pressure with

Computational fluid dynamics-based study of possibility of generating pulsatile blood flow via a continuous-flow VAD.

Until recent years, it was almost beyond remedy to save the life of end-stage heart failure patients without considering a heart transplant. This is while the need for healthy organs has always far exceeded donations. However, the evolution of VAD technology has certainly changed the management of these patients. Today, blood pumps are designed either pulsatile flow or continuous flow, each of which has its own concerns and limitations. For instance, pulsatile pumps are mostly voluminous and hardly can be used for children. On the other hand, the flow generated by continuous-flow pumps is in c
