Persistently elevated intracranial pressure in cryptococcal meningitis- 76 therapeutic lumbar punctures.

Cryptococcal meningitis still remains the most common form of adult meningitis in sub-Saharan Africa, due to the burden of HIV/AIDS. Increased intracranial pressure (ICP) is a major complication of cryptococcosis and requires aggressive management with therapeutic lumbar punctures (LPs). In this report, we describe a patient with persistently elevated ICP who underwent 76 LPs over 46 days with good outcome. While unusual, this highlights the importance of serial therapeutic LPs. 2012 Elsevier Ltd. All rights reserved.

Computed tomography head scans prior to lumbar punctures in suspected meningitis.

BACKGROUND: The literature suggests that certain clinical criteria to determine raised intracranial pressure (ICP) should be met if a patient suspected of meningitis is to undergo a computed tomography (CT) head scan prior to having a lumbar puncture (LP). AIM: To evaluate whether these criteria were being used by the general medicine department of Auckland City Hospital when requesting CT scans prior to LPs in patients with suspected meningitis. METHODS: A total of 37 patients admitted under general medicine with suspected meningitis from 1 January to 31 July 2016 were retrospectively audit

Epidemiology of lumbar punctures in hospitalized patients in the United States.

OBJECTIVES: Lumbar puncture (LP) is an important technique for assessing and treating neurological symptoms. The objective of this study was to describe the characteristics of diagnostic lumbar punctures performed on hospitalized patients in the United States. METHODS: We analyzed data from the 2010 National Inpatient Sample (NIS) and the National Emergency Department Survey (NEDS). We included patients treated in the Emergency Department (ED) as well as those admitted to an inpatient bed through the ED. We identified patients undergoing LPs from ICD-9 procedural code 03.31 and CPT code 62270

Correction of Cerebrospinal Fluid Protein in Infants With Traumatic Lumbar Punctures.

In our multicenter cohort of infants ≤60 days of age, we identified 2646 infants with a traumatic lumbar puncture, of which 31 (1.2%) had bacterial meningitis. For every 1000 cerebrospinal fluid red blood cells/mm, cerebrospinal (cerebrospinal fluid) protein increased 1.1 mg/dL (95% confidence interval: 1.0-1.2 mg/dL).

Tips and Tricks for Simple and Complex Below-the-Ankle Punctures.

Lower limb complex arterial obstructions can be difficult to treat by standard endovascular means. Different technical strategies, such as the retrograde puncture of below-the-knee (BTK) vessels, the plantar loop technique, or the transcollateral approach, have been described to approach BTK or below-the-ankle (BTA) chronic total occlusions after a failed antegrade recanalization attempt. However, when the disease progresses into the foot or the plantar arch cannot be crossed for a bidirectional approach, the necessity for mastering extreme distal punctures increases. This review is to summari

Repeated therapeutic lumbar punctures in cryptococcal meningitis - necessity and/or opportunity?

PURPOSE OF REVIEW: Overall, 50-70% of patients with cryptococcal meningitis have raised intracranial pressure (ICP). Multiple international treatment guidelines recommend repeated therapeutic lumbar punctures as adjunctive management. Here, we review the recent evidence for the role of repeated lumbar punctures on clinical outcome in cryptococcal meningitis and also review the increasing body of data utilizing these repeated cerebrospinal fluid (CSF) samples as a window into understanding immunopathogenesis of cryptococcal meningitis. RECENT FINDINGS: Adjunctive dexamethasone led to higher ad

Malignant mixed müllerian tumor of the ovary and false negative punctures.

Malignant mixed müllerian tumour (MMMT) of the ovary is a rare and aggressive tumour with a poor prognosis. We present a case of a 57-year-old woman with a large pelvic mass, omental cake, ascites and pleural effusions, clinically highly suspect of an ovarian neoplasm. Paracentesis and ultrasound-guided biopsy of the ovary were negative for malignant disease. Therefore a CT-guided true cut biopsy was performed. The latter gave a histopathologic diagnosis of an endometrioid adenocarcinoma of the ovary. However after cytoreductive surgery anatomopathologic examination revealed a malignant mixed

Epidermoid tumours associated with lumbar punctures performed in early neonatal life.

Three children (ages 5, 7, and 12 years) with epidermoid tumours in the spinal canal, all of whom had a lumbar puncture during the early neonatal period, are reported. A considerable delay occurred from the first symptoms until the diagnosis was made. MRI of the lumbar spine was the method of choice in the diagnostic work-up. All three cases were successfully surgically treated. The link between lumbar punctures and epidermoid tumours and the possible risk factors involved are explored. Because of the variable clinical presentation, the possibility of the existence of these tumours should be c

Lumbar punctures in suspected bacterial meningitis: too many or too few?

Children aged 1 month to 14 years admitted to the Royal Alexandria Hospital for Children during a 10 month period with suspected meningitis were studied prospectively. The aims were to determine how often lumbar puncture (LP) was delayed or never done, in relation to the outcome of all children, in order to determine the risks of LP and the risks of not doing LP. Of 218 children with suspected meningitis, LP was performed immediately in 195 (89.4%). Meningitis was diagnosed in 49 of these (bacterial 18, viral 31). No child developed cerebral herniation due to immediate LP. There were 11 trauma

[Diagnostic puncture and puncture treatment of liver echinococcosis--"yes" or "no"].

The authors present as problematic and debatable the article about the diagnostic punction and punction treatment of the liver echinococcosis. Of many years scientific disputation about this problem exist. The risks of per cutaneum punction leads to many serious complications. This assess the revaluation of this method which must apply only in very limited evidence--small surface cysts without additional cysts and for diagnostic plan must be abandon.

The risk of traumatic lumbar punctures in children with acute lymphoblastic leukaemia.

BACKGROUND: Traumatic lumbar punctures with blasts (TLP+) in children with acute lymphoblastic leukaemia (ALL) obscure central nervous system status and are associated with a poorer event-free survival (EFS). METHODS: We conducted a retrospective cohort study of all lumbar punctures (LPs) for children with ALL diagnosed at our institution from 2005 to 2009. We utilised random-effects and fixed-effects repeated-measures logistic regression analyses to identify risk factors for TLPs. Fixed-effects models use each patient as his or her own control. We used survival analysis to describe outcomes

The effect of therapeutic lumbar punctures on acute mortality from cryptococcal meningitis.

INTRODUCTION: Cryptococcal meningitis is the most common cause of adult meningitis in sub-Saharan Africa. Raised intracranial pressure (ICP) is common in cryptococcosis. Prior studies suggest elevated ICP is associated with mortality, and guidelines recommend frequent lumbar punctures (LPs) to control ICP. However, the magnitude of the impact of LPs on cryptococcal-related mortality is unknown. METHODS: In sum, 248 individuals with human immunodeficiency virus (HIV)-associated cryptococcal meningitis, screened for the Cryptococcal Optimal ART Timing (COAT) trial in Uganda and South Africa, we

[CT-guided punctures: fine-needle biopsy vs. automated co-axial cutting biopsy].

PURPOSE: Comparison between fine needle aspiration (FNAB) and automated coaxial core biopsy (ACCB) of CT-guided thoracal and abdominal biopsies. METHOD: 98 patients had CT-guided biopsies of the liver (n = 26), lung (n = 36), pancreas (n = 8), presacral space (n = 16), retroperitoneum (n = 8), groin (n = 1) and mediastinum (n = 3). In 50 patients fine needle aspiration, in 48 patients ACCB was performed. Nine physicians, two of them with advanced experience, performed the procedure. RESULTS: Histologically satisfying material was achieved in 76% of the biopsies with FNAB, in 100% with ACCB.

[Massive hemorrhage following multiple epidural punctures as a late complication in thrombocythemia].

We present a female patient in whom numerous lumbar punctures for epidural anaesthesia were followed by a delayed development of a large subcutaneous haematoma in the lumbar region. The outcome was fatal. The patient's basic disease was found to be chronic myeloic leukaemia that had caused an excessive elevation of platelet count. We, therefore, suggest that lumbar punctures for epidural anaesthesia should be approached with extreme care in cases of thrombocytaemia. If possible, repeated punctures for the placement of epidural catheters should be avoided.

To Lumbar Puncture or Not to Lumbar Puncture: A Case of Lyme Neuroborreliosis.

Lyme disease is a zoonotic infection increasing in prevalence across the United States. While the recognition of its classic clinical signs is sufficient to establish the diagnosis in the early stages, the diagnosis of Lyme neuroborreliosis (LNB) may be challenging and the diagnostic approaches may have to be tailored. We report a rare case of early disseminated LNB presenting with features of Banwarth syndrome in the form of painful radiculoneuritis, motor weakness, and facial palsy in a middle-aged female who presented to an Upstate New York Hospital during summer. Lyme antibody testing was
