Mechanism of cognitive impairment in chronic patients with putaminal hemorrhage: A diffusion tensor tractography.

It is not clear whether the fornix and cingulum are involved in cognition after putaminal hemorrhage (PH). We investigated structural changes and differences of the neural tracts, and the relationship between the integrity of the neural tracts and cognition not only at the affected but also at the unaffected side.Sixteen patients with left chronic putaminal hemorrhage and 20 healthy volunteers were enrolled. Using diffusion tensor tractography (DTT), we compared fiber number (FN), fractional anisotropy (FA), and apparent diffusion coefficient (ADC) of the neural tracts between patient and cont

Bilateral Gustatory Disturbance Associated With Unilateral Putaminal Hemorrhage: A Case Report and Literature Review.

Taste disturbance is relatively common after acute stroke but is often overlooked because it is subjective and frequently overshadowed by more prominent neurological deficits. Gustatory dysfunction specifically associated with putaminal hemorrhage has rarely been reported. We describe a 43-year-old right-handed woman who developed acute bilateral loss of taste in close temporal association with a left putaminal hemorrhage, despite clinically preserved olfaction and only mild sensory symptoms. Brain magnetic resonance imaging demonstrated a small left putaminal hemorrhage with surrounding edema

The Relationship Between the Direction of Putaminal Intracerebral Hemorrhage and Anterior Cerebral Artery Predominance.

OBJECTIVE: It is expected that anatomic variations in the circle of Willis have a direct effect on blood flow in the internal carotid artery. Rupture of the lenticulostriate artery of the middle cerebral artery 1st segment (M1) is the most common cause of putaminal intracerebral hemorrhage (ICH), and we hypothesized that this could be related to the anatomic variations of the circle of Willis and the predominance of the anterior cerebral artery 1st segment (A1). METHODS: We retrospectively reviewed the records of 544 patients who were treated for spontaneous ICH between 2013 and 2016 at Yeung

[Hemiballism-hemichorea caused by a putaminal hemorrhage with worsening after smoking--a case report].

A 65-year-old hypertensive man was admitted because of abnormal involuntary movements which had an onset one month prior to the admission. Neurological examination revealed an alert and intelligent male in no acute distress. Cranial nerves appeared intact except for dysarthria. His gait was wide-based. He had hemiballism-hemichorea in his left upper and lower extremities. Cranial CT scans revealed a high density area confined to the right putamen. After admission, his involuntary movements slowly improved without medication. After near complete disappearance of his abnormal movements two weeks

Cerebral blood flow in patients with putaminal hemorrhage after stereotactic aspiration or conservative treatment.

Changes in cerebral blood flow (CBF) were measured and compared in 40 patients with putaminal hemorrhage who underwent either stereotactic hematoma aspiration or conservative treatment. There were no statistically significant differences in mean CBF or hemispheric CBF between the two treatment groups. Although there are claims of the superiority of surgery from the standpoint of integrated cortical function following mild putaminal hemorrhage, the results of this investigation indicate that conservative therapy is adequate in such cases, based on improvement in CBF.

Case report: Near-complete cortical hearing loss caused by sequential development of bilateral putaminal hemorrhage.

Intracerebral hemorrhage with sudden hearing loss as the initial symptom is rare. A right-handed man with a history of right putaminal hemorrhage developed near-complete hearing loss and right hemiplegia and was taken to our hospital by ambulance. Non-contrast computed tomography demonstrated acute intracerebral hemorrhage in the left putamen. A region of old right putaminal hemorrhage involving the right temporal stem was also shown on fluid-attenuated inversion recovery. Standard pure-tone audiometry showed right-dominant bilateral sensorineural hearing loss. More than 2 months after onset,

Endoscopic Surgery Without Decompressive Craniectomy in Large Putaminal Intracerebral Hemorrhage: Assessment of Efficacy and Safety.

BACKGROUND: Decompressive craniectomy (DC) is performed conventionally for large putaminal intracerebral hemorrhage (ICH). However, DC causes local skull defect and leads to post-surgical cranioplasty. The aim of this study is to investigate the effectiveness and safety of an endoscopic procedure to treat large putaminal ICH without DC. METHODS: This retrospective study included 112 large putaminal ICH patients who underwent hematoma evacuations with either an endoscopic procedure (group A) or with DC (group B) between January 2009 and June 2017. The efficacy was evaluated by mean modified Ra

Adjacent Infarction in the Lenticulostriate Artery Territory Associated With Hypertensive Putaminal Hemorrhage: A Case Report.

Acute ischemic lesions are occasionally detected in patients with intracerebral hemorrhage, but they are usually remote from the hematoma. Acute infarction adjacent to hypertensive putaminal hemorrhage is extremely rare. We report a 52-year-old man with untreated hypertension who presented with right upper extremity weakness, gait unsteadiness, and left temporal headache. Computed tomography showed a left putaminal hematoma with surrounding edema and a subtle adjacent hypoattenuating area. Magnetic resonance imaging demonstrated an acute infarction adjacent to the superomedial aspect of the he

Sequential Development of Putaminal Hemorrhage and Corona Radiata Infarction in the Same Lenticulostriate Arterial Territory.

Putaminal hemorrhage is a common type of spontaneous cerebral hemorrhage. However, to our knowledge, there have been no reports of sequential cerebral hemorrhage and infarction in the same perforating arterial territory. Herein, we present the first reported case of the sequential development of putaminal hemorrhage and corona radiata infarction in the same lenticulostriate arterial (LSA) territory. Early intensive blood pressure lowering treatment may have aggravated ischemic damage. If a patient presents with motor weakness that cannot be explained by putaminal hemorrhage, the sequential dev

[Auditory agnosia associated with bilateral putaminal hemorrhage: A case report of clinical course of recovery].

A 45-year-old right-handed man with a past history (10 years) of putaminal hemorrage presented with auditory agnosia associated with left putaminal hemorrhage. It was suspected that the auditory agnosia was due to bilateral damage in the acoustic radiations. Generalized auditory agnosia, verbal and non-verbal (music and environmental), was diagnosed by neuropsychological examinations. It improved 4 months after the onset. However, the clinical assessment of attention remained poor. The cognition for speech sounds improved slowly, but once it started to improve, the progress of improvement was

Distal transsylvian-traninsular approach for the putaminal hypertensive hemorrhages: surgical experience and technical note.

BACKGROUND: Up to now, no systemic studies about the surgical approaches and microsurgical techniques of distal transsylvian-transinsular approach to putaminal hypertensive intracranial hemorrhages (PHHs) were reported. METHODS: A retrospective analysis was performed on 68 consecutive patients with PHH who underwent surgical treatment at the Department of the affiliated Bayi Brain Hospital, the Military General Hospital of Beijing PLA, from May 2009 to December 2011. RESULTS: By using transsylvian-transinsular approach, near-complete (>90%) evacuation was achieved in 51 cases (75%). Glasgow

[Diaschisis in right putaminal hemorrhage: correlation with the degree of the extension to the internal capsule and the volume of hematoma].

Local cerebral lesions may cause depression of function in remote areas of the brain presumably by a transneural mechanism. This has been called "diaschisis". In the present study, regional brain blood flow in hypertensive right putaminal hemorrhage was studied in 33 patients (mean age, 55 year; 22 men, 11 women) to investigate the relationship between the degree in damage of the original lesions and "diaschisis". The hematoma was treated conservatively in 8 patients, aspirated stereotaxically in 9 patients, and evacuated through craniotomy in 16 patients. The regional blood flow in bilateral

Relationship between temporal white matter projections and cognition in patients with putaminal hemorrhage: An observational study.

Cognitive impairment in patients with putaminal hemorrhage (PH) is not well understood, particularly the role of temporal white matter projections. The temporal white matter projections (cingulum, superior longitudinal fasciculus, inferior longitudinal fasciculus, and uncinate fasciculus [UF]) is involved in the cognitive function. Therefore, a brain injury involving the temporal white matter projections results in cognitive impairment. However, few studies have reported the pathophysiological mechanisms associated with cognitive impairment in patients with PH. Hence, in this study, using diff

Long insular artery damage might be a key sign for predicting functional prognosis of putaminal hemorrhage.

OBJECTIVE: Although the putamen is the most common area of spontaneous intracerebral hemorrhage, previous reports about the effects of surgery are limited. We sometimes experience a poor prognosis in patients in whom there is no damage to the internal capsule, but with injury in the long insular artery (LIA) region. The purpose of this study was to confirm the relationship between LIA damage and patient prognosis following surgery for putaminal hemorrhage. METHODS: We retrospectively collected data of 287 surgical cases who presented with putaminal hemorrhage between January 2004 and March 20

Minimally invasive surgical evacuation confers a mortality benefit in patients with moderate-sized putaminal hemorrhages.

OBJECTIVE: Minimally invasive surgical (MIS) evacuation of basal ganglia hemorrhages has not demonstrated efficacy compared with medical management. Prior study from this group has suggested that MIS evacuation of moderate-sized putaminal intracranial hemorrhages (pICHs) using tubular retractors is associated with favorable functional outcomes. The authors hypothesized that postoperative functional outcomes were improved in patients with moderate-sized pICH compared with those of a matched cohort of medically managed patients. METHODS: The authors performed a single-center retrospective revie
