Pyometra is a rare condition in which purulent material becomes entrapped within the uterine cavity. If unrecognized in a timely fashion, life-threatening complications can arise. The following is a case report of a 50-year-old female who presented to the emergency department with abdominal pain and heavy vaginal bleeding. She was diagnosed with a pyometra based on imaging and treated conservatively with antibiotics. The patient ultimately had an uncomplicated hysterectomy with resolution of normal female pelvic anatomy prior to surgery. Pyometra should be considered when women present with di

BACKGROUND: Emphysematous pyometra is a rare canine disease characterized by gas-forming bacteria infecting the uterus and causing an accumulation of both gas and infectious exudate in the uterine lumen. While radiological features of emphysematous pyometra have been previously described in dogs, the ultrasonographic appearance has not been reported.
CASE PRESENTATION: A 7-year-old intact female Labrador Retriever was presented because of a 1 day history of vomiting, anorexia, mild polyuria/polydipsia and signs of fatigue. On physical examination the dog had a swollen vulva with a sparse amou

We describe a Herlyn-Werner-Wunderlich syndrome (HWWS) patient with previous history of infertility who got pregnant without treatment and presented a pyometra in the contralateral uterus throughout the gestational period, despite multiple antibiotic treatments. Due to the uterus' congenital anomaly and the possibility of ascending infection with subsequent abortion, this pregnancy was classified as high-risk. We believe that the partial horizontal septum in the vagina may have contributed to the closure of the gravid uterus cervix, thus ensuring that the pregnancy came to term, with an uneven

Here we describe a case of pyometra coexisting with gestation in a 4.5 year-old miniature short-haired Dachshund. The dog exhibited depression, vaginal discharge, polydipsia and dehydration. Ultrasound examination revealed the presence of low to moderate anechoic fluid collection in the left uterine horn. Blood analysis revealed mild neutrophilia with a left shift. Based on these findings a presumptive diagnosis of pyometra was made and the bitch was treated using amoxicillin-clavulanate with dopaminergic agonist (cabergoline). A second ultrasound scan revealed the presence of two gestational

The objective of this study was to assess the frequency of gait abnormalities and lameness (GAL) in bitches with pyometra, and their association with clinical and laboratory findings. The study included 79 bitches diagnosed with pyometra and 35 negative control intact bitches presented with other soft tissue surgical disorders. Dogs with a history of chronic lameness due to orthopaedic or neurological origin were excluded. A history of GAL was more frequent in the pyometra group (47 per cent) compared with the control group (20 per cent) (P=0.007). In the pyometra group, bitches presenting GAL

A 13-year-old female domestic shorthair cat was examined because of weight loss over 4 months. The history included being neutered when a kitten and no signs of estrus recognized by the owner since this surgery. A mid-abdominal mass was palpated. Ultrasonographic abdominal evaluation revealed a tubular fluid-filled mass. The cat underwent surgery to remove the mass, which was one horn of the uterus. The patient made a full recovery.

Cystic endometrial hyperplasia-pyometra complex (CEH/P) is a challenge in canine reproduction. Present study aimed to assess fertility after medical treatment. One-hundred-seventy-four bitches affected by CEH/P received aglepristone on days 1, 2, 8, then every 7 days until blood progesterone < 1.2 ng/mL; cloprostenol was administered on days 3 to 5. Records were grouped according to bodyweight (BW): small (< 10 kg, n = 33), medium (10 ≥ BW < 25 kg, n = 44), large (25 ≥ BW < 40 kg, n = 52), and giant bitches (BW ≥ 40 kg, n = 45). Age; success rate; aglepristone treatments number; relapse, pregn

The treatment options for pyometra include antibiotic administration, cervical dilation with drainage, and surgical intervention. Surgical management options include laparotomy with lavage and/or total hysterectomy. However, reports on the percutaneous drainage of pyometra are rare. We present a case report and discussion of pyometra in a woman in her 70s with pyometra caused by cervical invasion of rectal cancer and fibrosis following radiation therapy. Antibiotic treatment was ineffective, and computed tomography (CT)-guided transabdominal uterine drainage was performed. After drainage, the

BACKGROUND: Pyometra is a life-threatening disease characterized by persistent endometrial inflammation and bacterial colonization, which occur through enzymatic and non-enzymatic antioxidant mechanisms. Minerals and trace elements have effective roles in numerous metal-dependent enzyme and protein activities. The aim of this study is to evaluate the monocyte to lymphocyte ratio (MLR), neutrophil to lymphocyte ratio (NLR), white blood cell (WBC), C-reactive protein (CRP), albumin (ALB), progesterone (P4) levels, serum and uterine tissue iron (Fe), copper (Cu), selenium (Se), zinc (Zn), magnesi

Pyometra, characterized by the accumulation of purulent exudate in the uterus, is the most prevalent reproductive disease in canines. While the disease often begins with mild local symptoms, it can escalate into peritonitis, sepsis, and multi-organ dysfunction, thereby posing a significant threat to life. Despite the high incidence and recognized significance of canine pyometra, gaps persist in our understanding of its epidemiology, etiology, and pathogenesis. Recent studies have, however, broadened our comprehension of this disease, shedding light on potential new infection sources, etiologie

Staphylococcus pseudintermedius is a commensal bacterium residing on the mucosae and skin surfaces of dogs. Under favorable conditions, the organism may cause serious infections, primarily involving the skin, but it is also isolated in eye and ear infections. This report describes pathologic findings of pyometra associated with S pseudintermedius in two bitches. One bitch was a 6-year-old Pomeranian bitch who died after three days of diarrhea, vomiting, and depression. During necropsy, the uterus of the bitch was diffusely enlarged and filled with 30 mL of thick, yellow, creamy pus. Histology

Uterine inflammation is the most common and clinically relevant genital change in bitches. The aim of this study was to evaluate the association between uterine hyperplasic lesions and uterine inflammation. Uteri and ovaries from 200 bitches were sampled after elective or pathological ovariohysterectomy and histologically evaluated. Endometrial hyperplastic lesions were classified as cystic endometrial hyperplasia (CEH) or pseudoplacentational endometrial hyperplasia (PEH), whereas cases of uterine inflammation were subdivided into endometritis or pyometra. The frequency of PEH (79.4%) was sig

RATIONALE: The incidence of spontaneous perforations in pyometra occurs rarely, only 0.01% to 0.5% in gynecological patients, with high mortality and morbidity. The clinical manifestation of perforated uterine pus is similar to that of gastrointestinal perforation, but the gynecological symptoms are not so obvious, which makes preoperative diagnosis difficult. Here, we report a rare case of peritonitis with laparotomy of pyometra.
PATIENT CONCERNS: An acute abdominal pain and purulent vaginal discharge developed in a 72-year-old woman who underwent an emergency laparotomy because of signs of

We report herein a case of spontaneous perforation of pyometra leading to diffuse peritonitis. This pathological condition is rare, and preoperative diagnosis is difficult. We were able to make the correct diagnosis preoperatively using contrast-enhanced abdominopelvic computed tomography. Both intrauterine and extrauterine fluid collections and defects of the uterine fundus were key findings, and reconstructed sagittal and coronal images were useful. In elderly female patients presenting with acute abdomen, ruptured pyometra should be considered as a possibility.

A 4-year-old miniature horse experienced intermittent episodes of pyrexia, lethargy, and purulent vaginal discharge following dsytocia. Vaginal endoscopy and transrectal ultrasonography revealed a blind-ending vaginal cavity and distended uterus. Surgical treatment was declined and the mare was euthanized. Post-mortem examination confirmed the diagnosis of pyometra and vaginal adhesions.

Cystic endometrial hyperplasia-pyometra complex is the most frequent and important endometrial disorder encountered in bitches. The pathogenesis of the disease is related to the activity of progesterone [Feldman and Nelson, Canine and Feline Endocrinology and Reproduction (1996) W.B. Saunders, Philadelphia]. Cystic endometrial hyperplasia (CEH) is an abnormal response of the bitch's uterus to ovarian hormones [De Bosschere et al. Theriogenology (2001) 55, 1509]. CEH is considered by many authors to be an exaggerated response of the uterus to chronic progestational stimulation during the luteal

Incidence of pyometra observed in our colony-raised beagle dogs over a 12-year period is described. Pyometra was observed in 25 female dogs of more than 4 years old, frequently at 8-11 years, with the average age of onset 9.36 +/- 0.38 years. The incidence of the disease was 15.2% of the female dogs (n = 165) more than 4 years old. Clinical findings useful in the diagnosis of this disease included an excretion of pus from the cervix, rapid increase in leukocyte count, and enlarged uterus as revealed by radiography. The relationships of delivery, estrus cycles to this disease are discussed. Ova
