Compensatory contribution of the contralateral pyramidal tract after experimental cerebral ischemia.

Many people escape sudden death from ischemic brain stroke, but suffer from severe disabilities such as aphasia and/or paralysis. These survivors of focal brain injury need chronic care to recover from and/or compensate for the impaired sensory and motor functions previously controlled by the focal ischemic core. Functional compensation not only involves the remaining brain areas around the infarction but also the areas contralateral to the stroke lesion, with the need for remodeling of neuronal circuits in some cases. In this review, recent human and animal studies are presented to aid in the

Ipsilateral hemiparesis after putaminal hemorrhage due to uncrossed pyramidal tract.

OBJECTIVE: Previous case reports supported the presence of the uncrossed pyramidal tract in exceptional patients. However, most of these case reports have not fully discussed involvement of the motor cortex controlling the ipsilateral limbs. DESIGN AND METHOD: The authors investigated a 62-year-old man who developed right hemiparesis after right putaminal hemorrhage by using MRI, transcranial magnetic stimulation, functional MRI (fMRI), and sensory evoked potentials. He had moderate weakness including the face, spasticity with brisk deep tendon reflexes and Babinski sign, and impaired vibrati

Wallerian degeneration of the pyramidal tract does not affect stroke rehabilitation outcome.

OBJECTIVE: To test whether Wallerian degeneration (WD) of the pyramidal tract as signaled by MRI affects rehabilitation outcome in patients with subcortical infarction (internal capsule or corona radiata). BACKGROUND: Recent radiologic evidence suggests that WD occurs no earlier than 3 months after a subcortical infarction. METHODS: A total of 77 consecutive patients with pure motor hemiparesis due to an initial subcortical infarction were assessed on admission and discharge with the Functional Independence Measure (FIM) for disability and Stroke Impairment Assessment Set (SIAS, full = 25) f

A rare case of ispilateral hemiparesis in a patient with uncrossed pyramidal tract shown by tractography.

This article presents a unique case of ipsilateral hemiparesis in a 66-year-old individual, contrary to the conventional understanding of supratentorial strokes causing contralateral neurological deficits. The patient exhibited persistent weakness and sensory abnormalities on the left side of the body following a left occipital infarct. Neuroimaging revealed a chronic stroke in the left occipital lobe, with diffusion tensor imaging demonstrating uncrossed pyramidal tracts at the level of the medulla. The discussion encompasses the anatomical basis of corticospinal tract crossing, historical pe

Intraoperative visualization of the pyramidal tract by diffusion-tensor-imaging-based fiber tracking.

Functional neuronavigation allows intraoperative visualization of cortical eloquent brain areas. Major white matter tracts, such as the pyramidal tract, can be delineated by diffusion-tensor-imaging based fiber tracking. These tractography data were integrated into 3-D datasets applied for neuronavigation by rigid registration of the diffusion images with standard anatomical image data so that their course could be superimposed onto the surgical field during resection of gliomas. Intraoperative high-field magnetic resonance imaging was used to compensate for the effects of brain shift, which a

Early, evolving Wallerian degeneration of the pyramidal tract in cerebrovascular diseases: MR study.

OBJECTIVE: The purpose of this study was to demonstrate the early features of the Wallerian degenerative process and their relationship with the severity of motor deficit. MATERIALS AND METHODS: Fourteen patients with infarction or hemorrhage of the basal ganglia were studied by MRI of the same coronal slice along the pyramidal line. RESULTS: The earliest sign of abnormal signal intensity was detected 0.7 month after the initial insult. On proton-density (PD) imaging, low signal intensity was observed on 11 (73.3%) of 15 occasions from 0.7 to 2.0 months, and abnormal signal intensity was not

Immunocytochemical localization of cell adhesion molecule L1 in developing rat pyramidal tract.

L1 is a representative of a family of carbohydrate neural cell adhesion molecules. The expression of L1 was studied during postnatal development of the rat pyramidal tract by immunohistology using polyclonal antibodies to L1 in spinal cord cervical intumescences. On postnatal day 1 (P1), L1 immunoreactivity was present in the entire dorsal funiculus, consisting of the ascending fasciculus gracilis and fasciculus cuneatus and the descending pyramidal tract. At that time the cervical pyramidal tract contains the first outgrowing corticospinal axons. At P4 both the fasciculus gracilis and the pyr

Prolonged inactivation of cortical pyramidal tract neurones in cats by distension of the carotid sinus.

1. We have investigated the effects of stimulating carotid sinus baroreceptors upon the activity of single cortical pyramidal tract cells (PT-cells) in anaesthetized cats.2. Extracellular potentials were recorded from PT-cells, which were driven orthodromically (1/sec) by stimulating thalamic nuclei (N. ventralis lateralis, N. ventralis posterolateralis) or afferent nerves in the contra-lateral forepaw. Baroreceptors were stimulated by inflating small balloons placed in the bifurcations of one or both common carotid arteries.3. Distension of the carotid sinus caused a prolonged depression of t

Changes in red nucleus after pyramidal tract injury in patients with cerebral infarct.

We investigated red nucleus (RN) changes in patients with a cerebral infarct, using diffusion tensor imaging (DTI). Forty-nine consecutive stroke patients with pyramidal tract(PT) injury (mean age, 62.1 years; range, 41 to 75) and 46 age-matched normal healthy control subjects (mean age, 58.9 years; range, 41 to 75) with no history of neurologic disease were recruited. DTI was performed using a 1.5-T system during the early stage, after cerebral infarct (8-l21 days after onset). DTIs were acquired using a sensitivity-encoding head coil at 1.5 T. We measured fractional anisotropies (FAs) and ap

Pyramidal tract degeneration in multiple system atrophy: the relevance of magnetization transfer imaging.

The clinical features of multiple system atrophy (MSA) include four domains: autonomic failure/urinary dysfunction, Parkinsonism, cerebellar ataxia, and corticospinal tract dysfunction. Although the diagnosis of definite MSA requires pathological confirmation, magnetic resonance imaging (MRI) studies have been shown to contribute to the diagnosis of MSA. Although pyramidal tract dysfunction is frequent in MSA patients, signs of pyramidal tract involvement are controversially demonstrated by MRI. We evaluated the pyramidal involvement in 10 patients (7 women) with clinically probable MSA, detec

Pyramidal tract side effects induced by deep brain stimulation of the subthalamic nucleus.

OBJECTIVE: To study the pyramidal tract side effects (PTSEs) induced by the spread of current from the subthalamic nucleus (STN) to the pyramidal tract (PT), in patients with parkinsonism undergoing STN stimulation. METHODS: 14 patients bilaterally implanted with tetrapolar electrodes were assessed. For each side separately, the threshold of adverse effects induced by monopolar stimulation delivered by the chronically used contact was detected. The voltage was progressively increased until the patient experienced discomfort. All the PTSEs induced at 130 Hz (high-frequency stimulation (HFS)) a

Pontine infarction in a dog with unilateral involvement of the trigeminal motor nucleus and pyramidal tract.

An unusual case of unilateral trigeminal neuronopathy in a dog is reported, in which the motor nucleus of the trigeminal nerve and the ipsilateral corticospinal tract were destroyed, apparently by a cerebrovascular accident (stroke). Hemiplegia did not occur. Neuropathological changes are described, including remyelination by Schwann cells in the central nervous system. The case illustrates the importance of central nervous system post-mortem examination when establishing causes of cranial nerve paralysis.

Pyramidal tract damage correlates with motor dysfunction in bilateral periventricular leukomalacia (PVL).

In children with periventricular leukomalacia (PVL), motor dysfunction is thought to be related to involvement of pyramidal tract fibres in the periventricular white matter. The purpose of the present study was to test this hypothesis. Thirteen former preterm adolescents with PVL, ten of whom were suffering from bilateral spastic cerebral palsy, were studied by MRI. The severity of pyramidal tract damage was assessed on semicoronal MRI reconstructions along anatomical landmarks of somatotopy in the precentral gyrus and the internal capsule; for comparison, the overall volume of cerebral white

Diffusion tensor tractography of pyramidal tracts in patients with brainstem and intramedullary spinal cord tumors: Relationship with motor deficits and intraoperative MEP changes.

PURPOSE: To evaluate whether pyramidal tracts course alterations observed in diffusion tensor tractography (DTT) in cases of brainstem and intramedullary spinal cord tumors reflect patient clinical status and prognosis. MATERIALS AND METHODS: For this purpose, we assessed in 17 patients relationships between pyramidal tracts course alterations observed in DTT (classified into four categories: unaffected; displaced or interspaced; partially disintegrated and completely disintegrated) performed on a 1.5 Tesla scanner and the presence of preoperative motor deficits, changes observed in motor evo

Symptoms of degeneration of the pyramidal tracts in conventional magnetic resonance imaging and diffusion tensor imaging in a young woman with primary lateral sclerosis.

Primary lateral sclerosis (PLS) is one of the forms of motor neuron disease (MND), affecting only upper motor neurons. The diagnosis of PLS should be made on different diagnostic criteria, for example, Pringle or Gordon, but it is usually a diagnosis of exclusion. There are no characteristic findings in standard laboratory and electrophysiological assessment. We present details of a 31-year-old woman who had suffered from progressive paraparesis with right-side predominance. Conventional MRI of brain and spinal cord and diffusion tensor imaging (DTI) studies showed Hyperintense lesions in the

Different subtypes of motor cortex pyramidal tract neurons projects to red and pontine nuclei.

INTRODUCTION: Pyramidal tract neurons (PTNs) are fundamental elements for motor control. However, it is largely unknown if PTNs are segregated into different subtypes with distinct characteristics. METHODS: Using anatomical and electrophysiological tools, we analyzed in mice motor cortex PTNs projecting to red and pontine midbrain nuclei, which are important hubs connecting cerebral cortex and cerebellum playing a critical role in the regulation of movement. RESULTS: We reveal that the vast majority of M1 neurons projecting to the red and pontine nuclei constitutes different populations. Cor
