Epigenetic and Transcriptomic Regulation Landscape in HPV+ Cancers: Biological and Clinical Implications.

Human Papillomavirus (HPV) is an oncogenic virus that causes the highest number of viral-associated cancer cases and deaths worldwide, with more than 690,000 new cases per year and 342,000 deaths only for cervical cancer (CC). Although the incidence and mortality rates for CC are declining in countries where screening and vaccination programs have been implemented, other types of cancer in which HPV is involved, such as oropharyngeal cancer, are increasing, particularly in men. Mutational and transcriptional profiles of various HPV-associated neoplasms have been described, and accumulated evid

USP6 RNA Chromogenic In Situ Hybridization Is Useful in the Diagnosis of Nodular Fasciitis and Aneurysmal Bone Cyst.

Nodular fasciitis (NF) and aneurysmal bone cyst (ABC) are uncommon entities that can mimic malignant neoplasms and be challenging to diagnose. Molecularly, the majority of NF and ABC cases are characterized by USP6 rearrangements, which result in a promoter swap and aberrant upregulation of USP6 RNA. While fluorescence in situ hybridization (FISH) can be used to confirm USP6 rearrangement, it often requires multiple days to complete. Here, we examine if USP6 RNA chromogenic in situ hybridization (CISH) could aid in the diagnosis of NF and ABC. We found that USP6 RNA CISH was positive in 89% (1

Case report: Hepatic inflammatory pseudotumor-like follicular dendritic cell sarcoma: A rare case and minireview of the literature.

Inflammatory pseudotumor (IPT)-like follicular dendritic cell sarcoma (FDCS) is a rare neoplasm referred to as the FDCS variant. Here we report a 66-year-old female patient suffering from hepatic IPT-like FDCS and summarize IPT-like FDCS reported in the literature. The patient presented with obvious abdominal pain without significant laboratory abnormalities and subsequently underwent surgical resection of a hepatic lesion. Postoperative pathological results demonstrated a vascular tissue-rich neoplasm (7.0-cm maximum diameter). The tumor cells expressed CD21 and CD35, and in situ hybridizatio

OGT-rearranged mesenchymal neoplasm of the palm with a novel C3orf62/MAPS partner gene: expanding the molecular spectrum of this rare entity.

OGT-rearranged mesenchymal neoplasm is a rare, molecularly defined soft tissue tumor with an acral predilection and recurrent fusions involving FOXO1, FOXO3, FOXO4, or CITED1. We report a tumor of the left ulnar palm in a 72-year-old woman presenting as a scaly papule. Shave biopsy demonstrated a bland dermal mesenchymal neoplasm composed of epithelioid to spindled cells arranged in cords and small nests within a hyalinized to fibromyxoid stroma. The lesional cells expressed CD34 diffusely and EMA focally, whereas SOX10 and SMA were negative. Targeted RNA sequencing identified a novel in-frame

GLI1-rearranged mesenchymal tumor of the ovary with a novel ACTG1::GLI1 fusion.

Here, we report an exceedingly rare case of a GLI1-rearranged mesenchymal neoplasm primarily occurring in the ovary, a highly unusual site for this tumor type, which typically arises in the gastrointestinal tract or soft tissues. The patient, a 17-year-old female, presented with acute abdominal pain, and imaging revealed an ovarian mass. Histopathological examination confirmed a GLI1-altered mesenchymal tumor, and RNA sequencing identified a novel ACTG1::GLI1 fusion, representing the first documented case of ACTG1 as a fusion partner for GLI1. By reviewing the literature, we further characteri

Uterine tumour resembling ovarian sex cord tumour (utrosct) associated with ctnnb1, pten and other gene mutations: Expanding the molecular landscape.

Uterine tumour resembling ovarian sex cord tumour (UTROSCT) is an uncommon neoplasm associated with recurrent gene fusions involving NCOA1-3, with ESR1 and GREB1 being the most frequent fusion partners. Fusions are not present in all cases and there have been few reports of gene mutations in these neoplasms. We report an index case of a UTROSCT (classic morphology with supportive immunophenotype) in a 58-year-old woman with no demonstrable fusions on RNA next generation sequencing (NGS) but with CTNNB1 and PTEN gene mutations. We reviewed a large series (n = 28) of UTROSCT from a single instit

Myeloid Neoplasm with PCM1-PDGFRB Transcript Responded to Low-Dose Imatinib: One Case Report with Literature Review.

Through an RNA-seq analysis of an adult patient with unclassifiable myelodysplastic/myeloproliferative neoplasms (MDS/MPN-U), we identified a rare PDGFRB fusion partner gene, PCM1. Conventional chromosome karyotype analysis showed abnormal clones of t(5;8)(q32;p22), and fluorescence in situ hybridization (FISH) confirmed rearrangement of the PDGFRB gene. Reverse transcription PCR (RT-PCR) and Sanger sequencing further confirmed that exon 30 of the PCM1 gene was fused with exon 11 of PDGFRB in frame, and the fusion event was accompanied by a 14 bp deletion of exon 11 of PDGFRB. After low-dose i

Diagnostic, Prognostic, and Predictive Utility of Recurrent Somatic Mutations in Myeloid Neoplasms.

The classification and risk stratification of myeloid neoplasms, including acute myeloid leukemia, myelodysplastic syndromes, myelodysplastic syndromes/myeloproliferative neoplasms, and myeloproliferative neoplasms, have increasingly been guided by molecular genetic abnormalities. Gene expression analysis and next-generation sequencing have led to the ever increasing discovery of somatic gene mutations in myeloid neoplasms. Mutations have been identified in genes involved in epigenetic modification, RNA splicing, transcription factors, DNA repair, and the cohesin complex. These new somatic/acq

Emerging roles for heterogeneous ribonuclear proteins in normal and malignant B cells.

Heterogeneous nuclear ribonucleoproteins (hnRNPs) are among the most abundantly expressed RNA binding proteins in the cell and play major roles in all facets of RNA metabolism. hnRNPs are increasingly appreciated as essential for mammalian B cell development by regulating the carefully ordered expression of specific genes. Due to this tight regulation of the hnRNP-RNA network, it is no surprise that a growing number of genes encoding hnRNPs have been causally associated with the onset or progression of many cancers, including B cell neoplasms. Here we discuss our current understanding of hnRNP

Transient overexpression of exogenous APOBEC3A causes C-to-U RNA editing of thousands of genes.

APOBEC3A cytidine deaminase induces site-specific C-to-U RNA editing of hundreds of genes in monocytes exposed to hypoxia and/or interferons and in pro-inflammatory macrophages. To examine the impact of APOBEC3A overexpression, we transiently expressed APOBEC3A in HEK293T cell line and performed RNA sequencing. APOBEC3A overexpression induces C-to-U editing at more than 4,200 sites in transcripts of 3,078 genes resulting in protein recoding of 1,110 genes. We validate recoding RNA editing of genes associated with breast cancer, hematologic neoplasms, amyotrophic lateral sclerosis, Alzheimer di

USP8-Rearranged Mesenchymal Tumors With Myofibroblastic Phenotype: A Comprehensive Clinicopathologic, Genetic, and Epigenetic Characterization.

USP8 is one of the members of ubiquitin-specific proteases deconjugating ubiquitin from target proteins. Besides USP6, it can be involved in tumorigenesis of mesenchymal neoplasms by binding to an activating fusion partner. Until now, USP8 fusion genes have been reported in calcified chondroid mesenchymal neoplasms, an inflammatory myofibroblastic tumor, a cardiac neoplasm, and a retroperitoneal sarcoma. In this study, we describe the clinicopathologic and genetic/epigenetic features of 7 USP8-associated tumors. The cohort included 5 male patients aged between 2 and 11 years, and 2 female pati

A Cryptic BCR-PDGFRB Fusion Resulting in a Chronic Myeloid Neoplasm With Monocytosis and Eosinophilia: A Novel Finding With Treatment Implications.

RNA-seq was used to identify the partner gene and confirm the presence of a BCR-PDGFRB fusion. Identification of this fusion product resulted in successful treatment and long-term remission of this myeloid neoplasm. Based on our results, we suggest that despite current WHO recommendations, screening for PDGFRB rearrangement in cases of leukocytosis with eosinophilia and no other etiologic explanation is necessary, even if the karyotype is normal.

Uterine Sarcoma With FGFR1-TACC1 Gene Fusion: A Case Report and Review of the Literature.

With the growing availability of RNA sequencing technology in the pathology laboratory, new gene fusion-associated malignancies are increasingly being characterized. In this article, we describe the second ever reported case of a uterine sarcoma harboring a FGFR1-TACC1 gene fusion. The patient, a 53-yr-old perimenopausal woman, was found to have a 6 cm mass spanning the lower uterine segment and endocervix. Histologically, this was a spindle cell neoplasm with coagulative necrosis, moderate cytologic atypia, and increased mitotic activity. By immunohistochemistry, the neoplastic cells coexpres

CCDC88C::PDGFRB-rearranged myeloid neoplasm with predominant neutrophilia and rapid response to imatinib: a molecularly defined case report.

Myeloid/lymphoid neoplasms with PDGFRB rearrangements are rare hematologic malignancies usually associated with eosinophilia and sensitivity to tyrosine kinase inhibitors, whereas atypical presentations remain poorly defined. We report a 60-year-old man with persistent leukocytosis and thrombocytopenia detected during a routine health examination. Bone marrow examination revealed marked granulocytic proliferation without increased blasts, and peripheral blood showed predominant neutrophilia with only mild eosinophilia. Cytogenetic analysis identified a t(5;14) translocation. RNA sequencing det

MicroRNAs in Myeloid Hematological Malignancies.

MicroRNAs are 19-24 nucleotides noncoding RNAs which silence modulate the expression of target genes by binding to the messenger RNAs. Myeloid malignancies include a broad spectrum of acute and chronic disorders originating from from the clonal transformation of a hematopoietic stem cell. Specific genetic abnormalities may define myeloid malignancies, such as translocation t(9;22) that represent the hallmark of chronic myeloid leukemia. Although next-generation sequencing pro-vided new insights in the genetic characterization and pathogenesis of myeloid neoplasms, the molecular mechanisms unde
