Postmastectomy radiation therapy and immediate autologous breast reconstruction: integrating perspectives from surgical oncology, radiation oncology, and plastic and reconstructive surgery.

BACKGROUND AND OBJECTIVES: The effect of postmastectomy radiation therapy (PMRT) on immediately reconstructed abdominal wall-based tissue remains imprecisely defined. We evaluated evidence from all fields involved in care of the breast cancer patient in order to advance a unified recommendation regarding this therapeutic sequence. METHODS: We performed a MEDLINE and manual reference search to identify studies of PMRT with immediate autologous breast reconstruction. Inclusion criteria required studies to describe patients, flaps, and complication rates. Analyses were based on a random effects

Key factors for establishing and sustaining a successful palliative radiation oncology program: a survey of the Society for Palliative Radiation Oncology.

BACKGROUND: Dedicated palliative radiation oncology programs (PROPs) within radiation oncology (RO) practices have been shown to improve quality and decrease costs of radiation therapy (RT) in advanced cancer patients. Despite this, relatively few PROPs currently exist, highlighting an unmet need to understand characteristics of the few existing PROPs and the potential barriers and facilitators that exist in starting and maintaining a successful PROP. We sought to assess the attributes of existing PROPs, the facilitators and barriers to establishing these programs, and the resources needed to

Advances in Radiation Oncology in Soft Tissue Sarcoma.

PURPOSE: To review recent advances with radiation therapy (RT) for soft tissue sarcomas (STS). RECENT FINDINGS: Newer data showcases hypofractionated preoperative RT for soft tissue sarcomas treated with surgery to be safe and effective, however, long-term follow up data is pending. Hypofractionated and dose-escalated RT in patients with unresectable STS is also being studied, for which we remain optimistic given advances in RT planning approaches. SFRT may also be considered in select cases to improve tumor control outcomes. Finally, for patients requiring high doses of RT adjacent to critic

H-Index and Promotion in Academic Radiation Oncology.

OBJECTIVES: Academic promotion in radiation oncology is influenced by multiple factors, including scholarly productivity and demographic characteristics. While citation-based metrics such as the h-index are increasingly used as objective measures of academic output, the impact of demographic factors such as sex and underrepresented minority (URM) status remains inadequately defined. This study represents the first evaluation of the predictive value of h-index, sex, and URM status on academic promotion. METHODS: A retrospective cohort of 554 radiation oncologists from 51 NCI-designated Compreh

Radiomics for clinical decision support in radiation oncology.

Radiomics is a promising tool for the development of quantitative biomarkers to support clinical decision-making. It has been shown to improve the prediction of response to treatment and outcome in different settings, particularly in the field of radiation oncology by optimising the dose delivery solutions and reducing the rate of radiation-induced side effects, leading to a fully personalised approach. Despite the promising results offered by radiomics at each of these stages, standardised methodologies, reproducibility and interpretability of results are still lacking, limiting the potential

[Practice-changing clinical trials in gastrointestinal radiation oncology].

Current events in radiotherapy oncology are marked by the results of strategic trials, particularly for esophageal and rectal cancers. For resectable esophageal adenocarcinoma, results of the ESOPEC study showed a benefit in overall survival from the perioperative chemotherapy with fluorouracil plus leucovorin, oxaliplatin and docetaxel compared to chemoradiotherapy (41.4Gy radiotherapy and carboplatin/paclitaxel chemotherapy). In definitive setting, the CONCORDE study did not show any benefit from dose escalation and the standard dose remains 50Gy. For resectable pancreatic cancer, the NRG/RT

Reliability of oral examinations: Radiation oncology certifying examination.

PURPOSE: Oral examinations are used in certifying examinations by many medical specialty boards. They represent daily clinical practice situations more realistically than do written tests or computer-based tests. However, there are repeated concerns in the literature regarding objectivity, fairness, and extraneous factors from interpersonal interactions, item bias, reliability, and validity. In this study, the reliability of oral examination on the radiation oncology certifying examination, which was administered in May of 2010, was analyzed. METHODS AND MATERIALS: One hundred fifty-two candi
