The capacity, capabilities and needs of the WHO BioDoseNet member laboratories.

Biodosimetry is an essential tool for providing timely assessments of radiation exposure, particularly when physical dosimetry is unavailable or unreliable. For mass-casualty events involving public exposure to ionising radiation, it is paramount to rapidly provide this dose information for medical management of casualties. The dicentric chromosome assay is currently the most reliable accepted method for biodosimetry; however, in a mass-casualty scenario, the throughput of this assay will be challenged by its time-consuming nature and the specific expertise required. To address this limitation

Study of the tools available in biological dosimetry to estimate the dose in cases of accidental complex overexposure to ionizing radiation: the Lilo accident.

PURPOSE: To compare the efficiency of different cytogenetic tools in estimating the doses received by four people involved in the Lilo accident and to monitor the dose estimate over 4.5 years. MATERIALS AND METHODS: Several young Georgian frontier guards handled at least one of the 12 Caesium sources found in a former Russian military camp. Overexposure lasted from July 1996 to May 1997. The Institute for Radiological Protection and Nuclear Safety (IRSN) obtained blood samples taken at several intervals post-exposure from the four most highly-exposed people. Dose estimation was performed usin

Living with rattlesnakes: Chernobyl and Dounreay.

After the Three Mile Island nuclear plant accident in 1979, a local Pennsylvania resident told The New York Times 'You live with that plant over there for years and years and don't think much about it. But it's like living with a rattlesnake. Sooner or later it's going to bite you. You just don't know when.' As we enter the twenty-first century, many communities find themselves living with rattlesnakes--one at least of which has already bitten. This article considers the official responses to Chernobyl in the first decade after the accident and compares them to the statements surrounding the s

Rethinking the Role of Biodosimetry to Assess Risks for Acute Radiation Syndrome in Very Large Radiation Events: Reconsidering Legacy Concepts.

The development of effective uses of biodosimetry in large-scale events has been hampered by residual, i.e., "legacy" thinking based on strategies that scale up from biodosimetry in small accidents. Consequently, there remain vestiges of unrealistic assumptions about the likely magnitude of victims in "large" radiation events and incomplete analyses of the logistics for making biodosimetry measurements/assessments in the field for primary triage. Elements remain from an unrealistic focus on developing methods to use biodosimetry in the initial stage of triage for a million or more victims. Bas

Medical management of heavily exposed victims: an experience at the Tokaimura criticality accident.

A criticality accident occurred at the uranium conversion plant in Tokaimura, Ibaraki Prefecture, Japan on 30 September 1999. When uranyl nitrate was overloaded to a critical mass level, uncontrolled fission reaction occurred. A procedure was carried out according to the JCO manual, although not an officially approved manual. Three workers were heavily exposed to neutrons andγ-rays produced by nuclear fission, and they subsequently developed acute radiation syndrome (ARS). The average doses to the whole body of the three workers were approximately 25, 9, and 3 GyEq (biologically equivalent dos

Assessment of an Emergency Medicine System for Radiation Accidents in Korea: A State Survey of the Workers Involved the Medical Response to Radiation Accidents.

Radiation emergency medicine systems are operated around the world to provide special care for the injured that require immediate medical attention in accidents. The objective of this survey was to evaluate people's perception of those who design the emergency medical plan for radiation accidents and those who supervise it in Korea. A questionnaire survey was conducted on the people involved in a regulatory system for medical response in a radiation emergency. Of 150 survey recipients, 133 (88.7%) completed the survey, including 92 workers and 41 inspectors. The respondents expressed the view

Dispersion, radiological dose assessment, risk evaluation, and emergency response of radioactive materials based on WRF-HYSPLIT modeling.

Accurate prediction of radionuclide dispersion is essential for radiation hazard prevention, yet predictive studies remain limited. This study employed the weather research and forecasting (WRF) model to generate a high-resolution meteorological data for the hybrid single particle lagrangian integrated trajectory (HYSPLIT) model to simulate the atmospheric transport of iodine-131 (131I). The calculation results of the real-time impact of radioactive emissions were obtained by first solving the gridded standard response distribution, then multiplying the coefficients according to the actual emi

Deposition Measurements From the Full-Scale Radiological Dispersal Device Field Trials.

In 2012, Defence Research and Development Canada led a series of experiments, titled the Full-Scale Radiological Dispersal Device Field Trials, in which short-lived radioactive material was explosively dispersed and the resulting plume and deposition were characterized through a variety of methods. Presented here are the results of a number of measurements that were taken to characterize the radioactive ground deposition. These included in situ gamma measurements, deposition filter samples, and witness plate measurements that were taken in situ with handheld beta survey meters. The results fro

Update and improvement of the global krypton-85 emission inventory.

Krypton-85 is mainly produced in nuclear reactors by fission of uranium and plutonium and released during chopping and dissolution of spent fuel rods in nuclear reprocessing facilities. As noble gas it is suited as a passive tracer for evaluation of atmospheric transport models. Furthermore, research is ongoing to assess its quality as an indicator for clandestine reprocessing activities. This paper continues previous efforts to compile a comprehensive historic emission inventory for krypton-85. Reprocessing facilities are the by far largest emitters of krypton-85. Information on sources and c

A radioiodine speciation, deposition, and dispersion model with uncertainty propagation for the Oak Ridge dose reconstruction.

Between 1944 and 1956, radioactive 131I was released into the atmosphere from operations at the Oak Ridge National Laboratory in Oak Ridge, TN. The releases occurred from stacks and from building vents and openings in three different chemical forms: elemental, organic, and particulate. During their transport in the atmosphere, different forms of iodine react differently with other atmospheric chemicals and moisture, and are removed from the plume at different rates by the processes of dry and wet deposition. A modified Gaussian plume model was developed to address the processes of radioiodine

Assessing radiation exposure of herbaceous plant species at the East-Ural Radioactive Trace.

The East-Ural Radioactive Trace (EURT) is a result of the Mayak Production Association accident that occurred in 1957 in Russia. Radiological assessment improves the interpretation of biological effects of exposure to ionizing radiation. Therefore a modeling approach was used to estimate dose rates on Leonurus quinquelobatus, Silene latifolia, Stellaria graminea and Bromus inermis. Soil-to-organism transfer parameter values are delivered from empirical data of (90)Sr and (137)Cs soil and vegetative plant mass activity concentrations. External and internal whole-body dose rates were calculated

One perspective on stakeholder involvement at Hanford.

The Hanford nuclear site in Washington State had a major role in the production of nuclear weapons materials during the Manhattan Project in World War II and during the Cold War that followed. The production of weapons-grade radionuclides produced a large amount of radioactive byproducts that have been stored since the mid-1900s at the Hanford Site. These by-product radionuclides have leaked from containment facilities into the groundwater, contaminated buildings used for radionuclide processing, and also contaminated the nuclear reactors used to produce weapons-grade uranium and plutonium. Fibrosarcoma of the thyroid in a man exposed to fallout from the Chernobyl accident.

Since the Chernobyl accident in April 1986, a dramatic increase in the incidence of thyroid carcinoma has been described, especially in children. A case of sarcoma of the thyroid in a man exposed to the Chernobyl fallout is presented. Sarcoma of the thyroid is a rare finding. The present patient lived in a region that is known to be one of the most exposed to Chernobyl fallout. The case suggests a high probability of a relationship between the sarcoma of the thyroid and the exposure to radiation.

[Lipid peroxidation and the antioxidant system in chronic cholecystitis in persons located in an area of increased radiation].

The studies covered 35 subjects with chronic cholecystitis who had to work in radionuclide-contaminated territory. There appeared changes in examinees' lipid peroxidation and antioxidant defense systems. These were more pronounced in leukocytes, possibly due to direct action of radiation on cell membrane, and may contribute to disease chronicity and recurrences.

Rapid Radiation Dose Estimation Techniques for Initial Patient Triage.

Emergency responders and health physicists need to make quick decisions on initial analyses and observations when responding to a radiological or nuclear emergency. Health physicists use complicated mathematical equations and dose calculation algorithms to create precise and detailed dose reconstructions, but this is dependent upon the release of information regarding the exposure or contamination scenario. The Radiation Emergency Assistance Center/Training Site employs and teaches certain "rules of thumb" dose estimation techniques for both internal and external exposures, which may help in e

Proposed "Exposure And Symptom Triage" (EAST) Tool to Assess Radiation Exposure After a Nuclear Detonation.

ABSTRACTOne of the biggest medical challenges after the detonation of a nuclear device will be implementing a strategy to assess the severity of radiation exposure among survivors and to triage them appropriately. Those found to be at significant risk for radiation injury can be prioritized to receive potentially lifesaving myeloid cytokines and to be evacuated to other communities with intact health care infrastructure prior to the onset of severe complications of bone marrow suppression. Currently, the most efficient and accessible triage method is the use of sequential complete blood counts

Potassium iodide (KI) to block the thyroid from exposure to I-131: current questions and answers to be discussed.

Thyroid cancer in children and adolescents has to be considered as the most severe health consequence of a nuclear reactor emergency with release of radioiodine into the atmosphere. High doses of potassium iodide are effective to block radioiodine thyroid uptake and to prevent development of thyroid cancer years later. However, there are controversies concerning thyroid cancer risk induced by radioiodine exposure in adults. Further, the interaction of nutritional supply of potassium iodide and radioiodine uptake as well as the interaction of radioiodine with certain drugs has not been addresse

Accidental or intentional exposure to ionizing radiation: biodosimetry and treatment options.

The potential risk of accidental and especially intentional radiation exposure in the form of a terrorist attack is growing. The dangers are potentially devastating. There is an urgent need for building a greater infrastructure for teaching and research in this area. Medical contingency planning and preparedness is also essential. Such planning should include an examination of our current resources, projected medical needs, management guidelines, and personnel training. Exposure to whole-body irradiation can induce acute radiation syndrome, with the resultant damage to hematopoiesis and immune
