Anesthesia for Pediatric Interventional Radiology.

Pediatric Interventional Radiology (IR) is progressively expanding and gaining popularity as it provides minimally invasive procedures and treatments, with very good outcomes and low adverse event rates. The image-guided body procedures include central venous access, gastrointestinal procedures, such as gastrostomy tubes, biopsies of solid organs and tumors, pleural and abdominal drain placements, endovascular procedures, and embolizations. Pediatric patients who require IR are often medically complex and have significant pathophysiological derangements. Pediatric IR intersects with multiple s

Interventional Procedures in Pediatric Musculoskeletal Diseases.

Interventional radiology (IR) procedures are becoming increasingly relevant in the management of musculoskeletal disorders due to their high precision and low invasiveness. However, specific considerations need to be made when applied to children and adolescents, especially in terms of anesthetic care and radioprotection. The aim of this article is to provide a description of the main IR procedures available for the treatment of benign and malignant bone tumors, septic arthritis, and rheumatic diseases in the pediatric population (including radiofrequency ablation, high-intensity focused ultra

Practical Imaging Strategies for Intussusception in Children.

OBJECTIVE. Intussusception is the most common cause of intestinal obstruction in young children. Radiology has a key role in its diagnosis and treatment. This systematic review summarizes the currently available evidence for best practices in radiologic management of pediatric intussusception. CONCLUSION. High diagnostic accuracy and lack of ionizing radiation make ultrasound (US) the preferred imaging modality for diagnosing intussusception. For intussusception reduction, fluoroscopy-guided pneumatic enema and US-guided hydrostatic enema are equally dependable and safe techniques. The areas t

Role of high magnification specimen radiography in surgical and core biopsies of the breast.

The clinical relevance of a high magnification specimen radiography (HMSR) system in breast biopsies was evaluated and compared with conventional specimen radiography with a mammography system (SRM). 100 surgical biopsies of 72 patients and 248 core biopsies of 30 patients were examined in (a) maximal 20-fold HMSR in combination with storage phosphors and (b) 1.8-fold SRM using a film-screen system. Detection of calcifications/soft tissue lesions and the impact on management were evaluated. In surgical biopsies, SRM could detect only 22% of individual microcalcifications, 39% of calcified lesi

Surgical Tools for the Interventional Radiologist.

Many of the instruments used in interventional radiology originated in surgical practice and have been adapted for minimally invasive procedures. Formal training in the design, handling, and optimal use of these instruments is not typically included in interventional radiology education. The purpose of this article is to provide a basic overview of these tools, including their names, variants, uses, and other potentially valuable details, using a pictorial approach. Included are scalpels, pickups, scissors, needle drivers, clamps, retractors, electrocautery, and sutures.

Precision Meets Pain Relief: Image-guided Nerve Blocks in Interventional Radiology.

Nerve blocks are a valuable opioid-sparing analgesic technique that play a critical role in multimodal pain management. As interventional radiology continues to expand its scope, proficiency in nerve block techniques has become increasingly relevant. Unlike general anesthesia, nerve blocks provide targeted pain relief, facilitating procedural success while minimizing systemic opioid use and associated complications as well as decreasing health care costs. The authors provide an in-depth discussion of nerve block mechanisms, indications, procedural techniques, imaging guidance, and safety consi

Paradigm shift in the application of patient contact shielding: a balancing act between tradition and progress.

OBJECTIVES: Despite recommendations and guidelines on patient contact shielding in X-ray imaging, substantial uncertainties remain in clinical practice, particularly concerning computed tomography (CT) examinations and vulnerable groups such as pediatric and pregnant patients. This study identifies gaps in existing recommendations and offers a comprehensive statement of the actual risks and benefits associated with patient shielding. MATERIALS AND METHODS: A systematic literature search was conducted using Google Scholar and PubMed, alongside current national and international guidelines. Our

Ultrasonography of the pediatric elbow.

Ultrasound of the elbow can be incorporated into routine pediatric practice in the radiology department, emergency department, orthopedic clinic, and interventional suite. Ultrasound is complementary to radiography and magnetic resonance imaging for the evaluation of elbow pain in athletes with overhead activities or valgus stress, focusing on the ulnar collateral ligament medially and capitellum laterally. As a primary imaging modality, ultrasound can be used for a variety of indications including inflammatory arthritis, fracture diagnosis, and ulnar neuritis/subluxation. Ultrasound is also w

Diagnostic approach and management of acute abdominal pain.

The incidence of acute abdominal pain ranges between 5-10% of all visits at emergency department. Abdominal emergencies of hospital visits may include surgical and non-surgical emergencies. The most common causes of acute abdomen are appendicitis, biliary colic, cholecystitis, diverticulitis, bowel obstruction, visceral perforation, pancreatitis, peritonitis, salpingitis, mesenteric adenitis and renal colic. Good skills in early diagnosis require a sound knowledge of basic anatomy and physiology of gastrointestinal tract, which are reflected during history taking and particularly, physical exa

Effect of triage-based use of the Ottawa foot and ankle rules on the number of orders for radiographic imaging.

CONTEXT: Reducing unnecessary testing lessens the cost burden of medical care, but decreasing use depends on consistently following evidence-based clinical decision rules. The Ottawa foot and ankle rules (OFARs) are validated, longstanding evidence-based guidelines to predict fractures. Frequently, radiography is automatically ordered for acute ankle injuries despite findings from OFARs suggesting no fracture. OBJECTIVES: First, to determine whether implementation of protocol-driven use of the OFARs at triage would decrease the number of radiography orders and length of stay (LOS) in the emer

Pneumothorax detection with thoracic ultrasound as the method of choice in interventional pulmonology - A retrospective single-center analysis and experience.

BACKGROUND: Recent studies have shown that thoracic ultrasound (TUS) is not inferior to chest radiography (CR) in detecting pneumothorax (PTX). It is unclear if adopting TUS can reduce the number of CR in the daily clinical routine. This retrospective study investigates the utilization of post-interventional CR and TUS for PTX detection after the introduction of TUS as the method of choice in an interventional pulmonology unit. METHODS: All interventions with CR or TUS for ruling out PTX performed in the Pneumology Department of the University Hospital Halle (Germany) 2014 to 2020 were includ

Uptake of validated clinical practice guidelines: experience with implementing the Ottawa Ankle Rules.

This study examined whether emergency physicians (EPs) exposed to multiple dissemination strategies for the Ottawa Ankle Rules (OARs) would reduce extremity radiography use. We conducted a prospective cohort study comparing intervention (n = 2) with control (n = 2) hospitals over a 2-year period. All EPs received the paper-based rules during the run-in phase; EPs in the intervention hospitals were also subjected in sequence to valid dissemination approaches. Provincewide dissemination of the OARs did not decrease radiography during the run-in period (92% vs. 93%; P =.36). Sequential directed e

Case Report: Successful management of acute kidney injury following interventional heartworm extraction in a dog with caval syndrome.

A 12-year-old, 7 kg, castrated male, mixed-breed dog presented with lethargy, vomiting, and hemoglobinuria. Heartworm infection was diagnosed through a positive Dirofilaria immitis antigen test, thoracic radiography, and echocardiography, which revealed an extensive worm burden extending into the right atrium, right ventricle, main pulmonary artery, and caudal vena cava, indicative of caval syndrome. Interventional heartworm extraction was successfully performed via jugular venotomy, based on clinical and imaging assessments. However, within 48 h postoperatively, the dog developed acute kidney

5. Cervical facet pain.

More than 50% of patients presenting to a pain clinic with neck pain may suffer from facet-related pain. The most common symptom is unilateral pain without radiation to the arm. Rotation and retroflexion are frequently painful or limited. The history should exclude risk factors for serious underlying pathology (red flags). Radiculopathy may be excluded with neurologic testing. Direct correlation between degenerative changes observed with plain radiography, computerized tomography, and magnetic resonance imaging and pain has not been proven. Conservative treatment options for cervical facet pai

Estimation of effective dose in some digital angiographic and interventional procedures.

In general, effective dose values for similar interventional vascular radiology (IVR) procedures are different. This is due to problems with the classification of radiological procedures, which make comparisons difficult. Patient size, examination technique and clinical condition as well as the skill of the medical radiologists also affect effective dose. Currently, there is a broad agreement on the classification of similar procedures so that effective dose estimates can be made from measurements of the dose area product (DAP). Thus, reference dose values may be established and comparative st

Lateral decubitus chest radiography or chest ultrasound to predict pleural adhesions before medical thoracoscopy: a prospective study.

BACKGROUND: Medical thoracoscopy (MT) is the gold-standard to investigate unexplained pleural exudates. However, the major prerequisite is an easy pleural access obtained by creating an artificial pneumothorax at the beginning of the procedure which can be a challenge in case of pleural adhesions and make the procedure unsafe. The detection of pleural adhesions prior to MT is necessary. Nowadays chest ultrasonography (CUS) is considered the best procedure to detect pleural adhesions. However, this technique is not available in all countries where the assessment of the pleural cavity is only ba
