As an imaging modality, magnetic resonance (MR) guidance has great potential to direct diagnostic and therapeutic procedures performed in the musculoskeletal region and influence patient management. MR-guided interventional procedures involving bone, soft tissue, intervertebral discs, and joints are safe and sufficiently effective for use in clinical practice. This article discusses and illustrates the procedural characteristics and techniques when performing MR-guided musculoskeletal interventions. Biopsy procedures are similar to other modalities for bone and soft tissue lesions. MR guidance

Following the release of European Directive EU 97/43, radiodiagnostic facilities within the European Union are required to implement a system of patient dose reviews based on comparisons with European, national and local diagnostic reference levels (DRLs). Establishing these levels for typical interventional radiology examinations presents a problem as definition of 'typical' examinations can be difficult, patient numbers are limited and these procedures are often performed at a few specialist centres. This paper uses dose-area product (DAP) gathered over a period of 3 years from 40 fluoroscop

Percutaneous transhepatic obliteration (PTO) and transileocolic vein obliteration (TIO) are techniques of interventional radiology for embolization of collaterals due to portal hypertension 1) We can obtain good results from the precise selection of these techniques in accordance with the patient's hemodynamics and general condition. 2) Endoscopic injection sclerotherapy (EIS) combined with PTO or TIO for esophageal varices proves to be superior in reliability and durability to EIS alone, and the time before retreatment is much longer when this combination therapy is used. 3) In the intractabl

BACKGROUND: Interventional radiology (IR) is an important component of radiology practice in South Africa (SA). However, there is no standard national IR curriculum. Little is known about IR teaching across SA's nine medical schools, of which six offer a 4-year and three offer a 5-year postgraduate radiology training programme. The study assessed procedural performance scope among SA postgraduate radiology trainees across key IR procedures.
METHODS: This retrospective, cross-sectional study evaluated candidates who completed SA postgraduate radiology training and sat the final FC Rad Diag(SA)

OBJECTIVE: To evaluate the current status of interventional radiology in Canada.
METHODS: A questionnaire was sent to 28 Canadian interventional radiologists (defined as a physician who performs any type of interventional procedure, including biopsies, but excluding interventional neuroradiology) practising in both tertiary and community hospitals in the major centres in all provinces except Prince Edward Island.
RESULTS: Twenty-two (79%) of 28 surveys were completed and returned, providing data about 86 interventional radiologists (IRs). IRs were performing almost all of the following proce

PURPOSE: Since the initial edition of this Guideline was published in 2010, it has become increasingly clear that occupational protection involves more than just radiation protection. Additionally, radiation exposure to operators and staff occurs from sources other than fluoroscopy. This Guideline has been updated to incorporate these topics and to reflect new information on occupational protection in interventional radiology.
METHODS: The CIRSE Standards of Practice Committee, in conjunction with SIR, established a writing panel of twelve clinicians, medical physics experts, and others with

In interventional radiology (IR) for malignant bowel obstruction, treatment is broadly classified into drainage and stent placement. In cases such as terminal-stage malignancy, where removal of a nasogastric or ileus tube is unlikely, gastrostomy or stent placement may be performed endoscopically. However, for patients in whom endoscopic gastrostomy is difficult-such as those with anatomical or procedural limitations-IR-guided access for gastrointestinal decompression can be considered. Representative procedures include percutaneous radiologic gastrostomy (PRG), also referred to as radiologica

Transbronchial interventions are rapidly evolving for diagnosis and treatment of peripheral lung nodules. Despite recent advances and lower risk of pleural-related complications, the diagnostic yield of bronchoscopic approaches remains lower than that of image-guided percutaneous approaches. Standardized transbronchial methods use engineered, robotic, and navigation approaches; however, a possible alternative approach familiar to interventional radiology (IR) could use image guidance, image fusion, and IR devices like guidewires and steerable sheaths for transbronchial navigation without the u

Sepsis is a clinical syndrome encompassing physiologic and biologic abnormalities caused by a dysregulated host response to infection. If sepsis progresses into septic shock it is associated with a high mortality, therefore early identification and treatment is critical. Interventional radiologists often perform treatments which may cause or temporarily exacerbate sepsis. Therefore, it is important for the interventional radiologist to recognize early signs of sepsis and understand sepsis treatment protocols. These protocols include source control of infections, administration of systemic broa

Interventional radiologists (IRs) have a massive impact on their patients, communities, and healthcare at large. Yet, IRs have physical and emotional challenges that lead to a high rate of burnout compared with other medical specialties. A Medscape survey in 2013 showed a 37% burnout incidence among radiologists, which increased to 49% in 2015. This ranked radiology 7th out of 26 specialties with respect to burnout. Although the survey did not examine IR specifically, with the increasing demands on those in the profession, this number can only be expected to increase. A survey by Bundy et al d

Interventional procedures in the airway can be performed in interventional radiology suites or the operating room, by radiologists or other specialists. The most common therapeutic interventions carried out by radiologists are balloon dilatation, stenting, and the treatment of certain airway fistulas. These operations can be very challenging for anesthetists in terms of planning, airway management, the identification and treatment of procedural complications and postoperative care. In particular, a multidisciplinary approach to decision-making and planning is important to obtain the best resul

Therapeutic advances in oncology have led to longer survival in many forms of cancer, including those complicated by bone metastases. When a bone metastasis is painful or when there is a risk of fracture, interventional radiology procedures can be carried out for pain control and/or stabilisation. All of these techniques can be performed under local anaesthesia. Cementoplasty and vertebroplasty are stabilisation procedures consisting in the percutaneous injection of acrylic cement into a lytic bone lesion. The effect on pain can be explained by the consolidation of weakened, fractured or pre-f

Postpartum hemorrhage (PPH) is a serious complication of giving birth that can result in death. Interventional radiology allows for the use of uterine artery embolization and transcatheter arterial embolization to prevent and treat PPH. These procedures are minimally invasive options that use the uterine or internal iliac arteries to place either a balloon or embolic material to stop the bleeding. These techniques also can be used as a preventive treatment for women with known placental abnormalities who are at an increased risk of hemorrhaging. This article discusses causes, early recognition

INTRODUCTION: Hepatic arterial venous fistulae are abnormal communications between the hepatic artery and portal or hepatic vein and commonly occur either secondary to iatrogenic causes like liver biopsy, transhepatic biliary drainage, transhepatic cholangiogram and surgery, or following mechanical insult like blunt or penetrating trauma. Congenital fistulae are rare. Treatment is warranted as an emergency management or in the development of portal hypertension/heart failure in chronic cases. Both surgical and endovascular occlusion of the fistula can be attempted with the latter carrying low

OBJECTIVE: To present interventional therapeutic options for patients with thrombosis.
ETIOLOGY: Thrombosis in small animals results from an unbalance in the normal hemostatic mechanisms leading to vessel occlusion. In veterinary medicine, thrombosis is recognized as a common complication of many acquired diseases, including cardiac, endocrine, immunological, inflammatory, and neoplastic disorders.
DIAGNOSIS: Clinical signs are variable depending on the location of the thrombus and various laboratory and imaging modalities can aid in its identification and localization.
THERAPY: Once identi

The author discusses the recent role of interventional radiology to prevent postpartum hemorrhagic complications that represent an important cause of maternal morbidity and mortality all over the world. Hence, hemorrhage control is mandatory. Traditional management and recent minimally invasive radiological procedures by means of inserting occluding balloons into appropriate vessels are analyzed. It is advisable that maternity hospitals have protocols for the management of obstetric hemorrhage by means of interventional radiology, either in their own departments or in association with other la

A 21-year-old patient presented with sudden-onset headache, visual disturbance and left hand incoordination. She was diagnosed with a left vertebral artery dissection of the V3 segment resulting in multiple cerebellar and cerebral infarcts. There were no risk factors for dissection other than recent COVID-19 infection. She was treated initially with antiplatelets, followed by anticoagulation, but experienced recurrent ischaemia. Although guidance suggests endovascular repair may be beneficial for patients with cerebral artery dissection (CAD) who experience recurrent strokes on medical therapy

Interventional radiologists now perform spinal interventions routinely for diagnostic and therapeutic purposes. New technologies for the management of spine pathologies have emerged with promising results in terms of safety and efficacy. Interventional radiology techniques in the spine include percutaneous biopsy and therapies for intervertebral disk herniation or spinal stenosis, facet and sacroiliac joint pathologies, vertebral and sacral fractures, and metastases. These techniques can also be easily combined one with the other or to further therapeutic approaches including systemic therapie
