A prospective randomised radiostereometric analysis trial of SmartSet HV and Palacos R bone cements in primary total hip arthroplasty.

BACKGROUND: Introduction of new bone cements into clinical practice should include radiostereometric studies. MATERIALS AND METHODS: A prospective randomised radiostereometric study was performed, comparing SmartSet HV and Palacos R acrylic bone cements (without antibiotics) using third-generation cementing techniques in primary total hip arthroplasty. Thirty-five patients (36 hips) undergoing Charnley total hip arthroplasty were randomised to receive either of the two cements and were followed with repeated clinical, radiographic and radiostereometric examinations over 24 months. Twenty-seve

Two-year radiostereometric analysis evaluation of femoral head penetration in a challenging population of young total hip arthroplasty patients.

This prospective, randomized protocol evaluated femoral head penetration after total hip arthroplasty in a young population. Forty-five patients randomly received either a cross-linked or conventional ultrahigh-molecular-weight polyethylene (UHMWPE) liner in a noncemented hemispheric cup (Trilogy, Zimmer, Warsaw, Ind) with a 28-mm femoral head. Radiostereometric analysis film pairs, Harris hip, UCLA, SF-12, and Western Ontario and McMaster Universities scores were obtained through 2 years. Median femoral head penetration was less among cross-linked compared to conventional liners as follows: 0

Computed tomography-based radiostereometric analysis achieves sufficient precision for femoral implants in total knee arthroplasty: a randomized controlled trial.

BACKGROUND: Conventional radiostereometric analysis (RSA) has been used for several decades to assess implant migration in total knee arthroplasty (TKA). More recently, computed tomography-based radiostereometric analysis (CT-RSA) has been introduced as an alternative. Experimental studies have demonstrated acceptable precision for CT-RSA; however, this has not yet been validated in a clinical setting. With the emergence of new alignment philosophies in TKA, migration analysis of femoral components has become increasingly important. A key criticism of CT-RSA has been the higher radiation doses

5-year experience of highly cross-linked polyethylene in cemented and uncemented sockets: two randomized studies using radiostereometric analysis.

BACKGROUND: Laboratory tests and early clinical studies have shown that highly cross-linked polyethylene (PE) markedly improves wear resistance compared to conventional PE. We evaluated this type of PE in two randomized clinical studies using radiostereometric analysis (RSA). The 2- and 3-year follow-up of these studies have already been reported. We found a lower penetration rate for the highly cross-linked PE than for conventional PE. We now report the outcome after 5 years. PATIENTS AND METHODS: 60 patients (61 hips) with a median age of 55 years were randomized to receive either highly cr

CT-based migration analysis of a tibial component compared to radiostereometric analysis : one-year results of a prospective single-group implant safety study.

AIMS: Radiostereometric analysis (RSA) is considered the gold standard for in vivo migration analysis, but CT-based alternatives show comparable results in the shoulder and hip. We have previously validated a CT-based migration analysis method (CTMA) in a knee phantom compared to RSA. In this study, we validated the method in patients undergoing total knee arthroplasty (TKA). Our primary outcome measure was the difference in maximum total point motion (MTPM) between the differing methods. METHODS: A total of 31 patients were prospectively studied having undergone an uncemented medial pivot kn

Low wear with vitamin E-infused highly cross-linked polyethylene : 14 years' follow-up of a randomized radiostereometric analysis study.

AIMS: Highly cross-linked polyethylene (HXLPE) infused with vitamin E was introduced at the beginning of the 21st century. Vitamin E removes free radicals arising during gamma irradiation in order to cross-link the polyethylene. Removing free radicals was hypothesized to preserve mechanical properties and wear behaviour of the polyethylene in vivo. Excellent polyethylene wear behaviour of other HXLPEs has encouraged surgeons to use larger heads in total hip arthroplasty (THA) to address prosthesis dislocation. It is still unclear whether polyethylene wear remains low in the long term when usin

Evaluation of in vivo wear of vitamin E-diffused highly crosslinked polyethylene at five years: a multicentre radiostereometric analysis study.

AIMS: The primary aim of this study was to compare the wear properties of vitamin E-diffused, highly crosslinked polyethylene (VEPE) and one formulation of moderately crosslinked and mechanically annealed ultra-high molecular weight polyethylene (ModXLPE) in patients five years after primary total hip arthroplasty (THA). The secondary aim was to assess the clinical results of patients treated with VEPE by evaluating patient-reported outcome measures (PROMs), radiological evidence of fixation, and the incidence of mechanical failure. Implant stability and clinical performance of a bicruciate-retaining total knee arthroplasty: A radiostereometric analysis up to 7.5 years follow-up.

BACKGROUND: A previous randomized controlled trial showed higher tibial migration and more device-related complications in bicruciate-retaining compared to cruciate-retaining total knee arthroplasty, raising concerns about long-term implant stability and prompting this follow-up study to assess migration patterns and clinical outcomes up to 7.5 years postoperatively. METHODS: In this follow-up study, the bicruciate-retaining group from an initial single-centre randomized controlled trial was monitored at 5 and 7.5 years postoperatively. Implant migration was measured through model-based radio

First-time revision using impacted morsellised allograft bone with a cemented Exeter stem: radiostereometric analysis of stem migration over nine years.

Previously, radiostereometric analysis following hip revision performed using impacted morsellised allograft bone and a cemented Exeter stem has shown continuous subsidence of the stem for up to five years. It is not known whether the subsidence continues thereafter. In our study, 17 of 25 consecutive osteo-arthritic patients with aseptically loose stems who underwent first-time revision using impacted morsellised allograft bone and a cemented Exeter stem were followed by yearly radiostereometric examinations for nine years. The mean subsidence at six weeks was 1.1 mm (0.1 to 2.3), from six we

Stable migration pattern of an ultra-short anatomical uncemented hip stem: a prospective study with 2 years radiostereometric analysis follow-up.

INTRODUCTION: Shorter, anatomically shaped and proximally loading stems have been developed to achieve better stress distribution and be more bone preserving. The purpose of this prospective study was to evaluate the migration pattern of the Proxima™ ultra-short uncemented stem using radiostereometric analysis (RSA), and to review the literature regarding the migration of short stemmed hip arthroplasty. METHODS: 25 patients (28 hips) with hip osteoarthritis received a Proxima stem during total hip arthroplasty (THA). To measure stem migration, repeated RSA examinations were done during a 2 ye

Radiostereometric analysis of distal radial fracture displacement during treatment: a randomized study comparing Norian SRS and external fixation in 23 patients.

In a randomized study, we included 23 osteoporotic patients with a distal radial fracture and loss of reduction after 1 week. The facture was re-reduced. In one group, a self-setting hydroxyapatite, Norian SRS, was injected into the fracture and the wrist was immobilized for 2 weeks with a dorsal splint (n 12). In the other group, the fracture was immobilized for 5 weeks with an external fixator (n 11). During the operation, the fracture fragments were marked with tantalum markers, so that loss of reduction during the immobilization and after mobilization could be studied with radiostereometri

The Effect of Coronal Alignment on Tibial Component Migration Following Total Knee Arthroplasty: A Cohort Study with Long-Term Radiostereometric Analysis Results.

BACKGROUND: Recent short-term studies of total knee arthroplasty (TKA) have claimed improved clinical outcomes and implant survival when aiming to restore constitutional joint kinematics, as compared with neutral mechanical axis alignment. However, implant durability may be compromised when aligned in varus or valgus. With use of data pooled from 3 long-term radiostereometric analysis (RSA) studies, the aim of the present study was to assess the effects of coronal alignment on tibial component migration. METHODS: Coronal alignment parameters from full-leg radiographs were measured and the con

The seven-year wear of highly cross-linked polyethylene in total hip arthroplasty: a double-blind, randomized controlled trial using radiostereometric analysis.

BACKGROUND: The use of highly cross-linked polyethylene is now commonplace in total hip arthroplasty. Hip simulator studies and short-term in vivo measurements have suggested that the wear rate of highly cross-linked polyethylene is significantly less than that of conventional ultra-high molecular weight polyethylene. However, long-term data to support its use are limited. The aim of this study was to compare the intermediate-term steady-state wear of highly cross-linked polyethylene compared with that of conventional ultra-high molecular weight polyethylene acetabular liners in a prospective,

The combination of radiostereometric analysis and the telos stress device results in poor precision for knee laxity measurements after anterior cruciate ligament reconstruction.

PURPOSE: Several devices for measuring knee laxity following anterior cruciate ligament ACL reconstruction exist, but the precision of the methods has never been optimal. Therefore, a new standardized protocol (NSP) was made, aiming at ensuring a reliable positioning of the Telos Stress Device (TSD) which theoretically could result in precise knee laxity measurements when using radiostereometric analysis (RSA) in combination with TSD. METHOD: The TSD was applied to the knee of 30 healthy persons, using both the NSP and the official company instructions. The position of the stress arms of the

Long-term migration of monoblock vs modular design in uncemented total knee arthroplasty: a secondary report of a randomized trial using radiostereometric analysis.

BACKGROUND AND PURPOSE: Backside wear of the polyethylene insert in total knee arthroplasty (TKA) has been described to produce clinically significant levels of polyethylene debris, which can lead to aseptic loosening and osteolysis. Monoblock design eliminates backside wear of the polyethylene and therefore could improve long-term fixation. This randomized clinical trial (RCT) using radiostereometric analysis (RSA) compares micromotion of monoblock and modular polyethylene inserts with 7 years' follow-up. METHODS: 65 patients (mean age 61 years) were randomized to receive either monoblock (n

Radiostereometric measurement of implant migration in robotically assisted vs conventional bi-cruciate stabilized cemented total knee arthroplasty: secondary analysis of a randomized controlled trial.

BACKGROUND AND PURPOSE: Robotically assisted computer navigation (robotic) has been developed to improve the positioning in total knee arthroplasties (TKAs), attempting to achieve better functional results and longevity of the prostheses. However, the benefit of robotics is still controversial. The aim of our study was to compare migration between robotic and conventional techniques in cemented bi-cruciate stabilized TKAs, using radiostereometric analysis (RSA) based on a secondary analysis of a randomized controlled trial (RCT). METHODS: We enrolled 60 TKA patients from one hospital (2020-20
