There is no doubt about the effectiveness of radiotherapy as total skin electron irradiation in the therapy of mycosis fungoides and the Sézary syndrome. Dose distribution in skin and body surface (measured on the Alderson phantom) as well as the therapy scheme by electron therapy are presented. The Stanford group observed that 84% of their patients achieved a complete remission by aggressive (greater than 3,000 rad) total skin electron irradiation. The overall survival rate was 46% at 10 years with the major prognostic factor being the initial extent of skin involvement.

A technique for rotational total skin electron irradiation is presented in which the patient stands on a slowly rotating platform (SSD = 285 cm) in a large uniform linear accelerator electron field (Eo = 3.5 MeV). The beam is scattered by the transmission ionization chamber and by a special lead/aluminum scattering filter, and then degraded by a sheet of Lucite. Paget's disease of the breast is a rare form of breast cancer. Patients with nipple-areolar disease alone and no palpable mass have an excellent prognosis. Although there is some argument about the treatment of this type of Paget's disease, only two reports have been published on the role of conservative treatment. This report discusses a case of limited Paget's disease of the nipple and areola of the breast which was treated by radiation alone without limited surgery.

Doses in the build-up region have been compared for regular fields with and without a perspex accessory tray and for two irregular fields defined by either a Philips multi-leaf collimator (MLC) or lead blocks. The results show an increase in doses within the build-up region, by up to a factor of two, primarily due to the presence of the accessory tray. A retrospective analysis was carried out to evaluate the effectiveness of small field megavolt electron irradiation for localized mycosis fungoides. The most effective treatment of late mycosis fungoides is total skin electron beam therapy. The beam at the Hammersmith Hospital in London has been adapted to treat these patients. We report the case of a 57-year-old woman who developed bullous pemphigoid 3 weeks after electron beam radiotherapy. The bullae were confined to the area of the radiation field and responded to topical clobetasol propionate 0.05% cream (Dermovate). This is the second case of bullous pemphigoid confined to an area of previous radiotherapy. Radiotherapy may be a rare trigger for the development of bullous pemphigoid.
