Value-based health care (VBHC) is an evolving paradigm in healthcare that prioritises patient outcomes relative to the costs incurred. VBHC emphasises efficiency, quality, and patient satisfaction as metrics and determinants of healthcare. A VBHC approach is becoming increasingly significant as healthcare systems worldwide grapple with rising costs, staff shortages, and the ongoing need for improved patient care. Online adaptive radiotherapy (oART) is a promising innovation in radiation oncology, allowing for the adaptation of radiation treatment plans to account for daily anatomical changes.

PURPOSE: In a recent article, our group proposed a fast direct aperture optimization (DAO) algorithm for fixed-gantry intensity-modulated radiation therapy (IMRT) called fast inverse direct aperture optimization (FIDAO). When tested on fixed-gantry IMRT plans, we observed up to a 200-fold increase in the optimization speed. Compared to IMRT, rotational volumetric-modulated arc therapy (VMAT) is a much larger optimization problem and has many more delivery constraints. The purpose of this work is to extend and evaluate FIDAO for inverse planning of VMAT plans.
METHODS: A prototype FIDAO algori

More than 50% of cancer patients are treated with radiotherapy, either exclusively or in combination with other methods. The planning and delivery of radiotherapy treatment is a complex process, but can now be greatly facilitated by artificial intelligence technology. Deep learning is the fastest-growing field in artificial intelligence and has been successfully used in recent years in many domains, including medicine. In this article, we first explain the concept of deep learning, addressing it in the broader context of machine learning. The most common network architectures are presented, wi

PURPOSE: The purpose of this work is to explore a new adaptive radiation therapy (ART) strategy, combined "online and offline" ART, that can fully account for interfraction variations similar to the existing online ART but with substantially reduced online effort.
METHODS AND MATERIALS: The concept for the combined ART is to perform online ART only for the fractions with obvious interfraction variations and to deliver the ART plan for that online fraction as well as the subsequent fractions until the next online fraction needs to be adapted. To demonstrate the idea, the daily computed tomogra

Through geometrical simulation, we evaluated the effect of rotational error in patient setup on geometrical coverage and calculated the maximum distance between the isocenter and target, where the clinical PTV margin secures geometrical coverage with a single-isocenter technique. We used simulated spherical GTVs with diameters of 1.0 (GTV 1), 1.5 (GTV 2), 2.0 (GTV 3), and 3.0 cm (GTV 4). The location of the target center was set such that the distance between the target and isocenter ranged from 0 to 15 cm. We created geometrical coverage vectors so that each target was entirely covered by 100

PURPOSE: Deformable dose accumulation (DDA) has uncertainties which impede the implementation of DDA-based adaptive radiotherapy (ART) in clinic. The purpose of this study is to develop a multi-layer quality assurance (MLQA) program to evaluate uncertainties in DDA.
METHODS: A computer program is developed to generate a pseudo-inverse displacement vector field (DVF) for each deformable image registration (DIR) performed in Accuray's PreciseART. The pseudo-inverse DVF is first used to calculate a pseudo-inverse consistency error (PICE) and then implemented in an energy and mass congruent mappi

Positron emission tomography/Computed tomography (PET/CT) is an essential part of modern radiotherapy for patients with lymphoma. PET/CT can be used to adapt treatment algorithms in Hodgkin lymphoma, reserving consolidation radiotherapy for patients with residual fluoro-D-glucose (FDG) avidity after treatment with intensive chemotherapy such as escalated BEACOPP and limiting the need for radiotherapy for some patients with complete metabolic response on PET if radiotherapy may be associated with increased toxicity. More importantly, PET/CT is now mandatory to define sites of initial disease fo

BACKGROUND: The degree of freedom has been recognized as a key factor in particle beam therapy, such as improving the dose conformity and linear energy transfer distributions. Therefore, proton arc therapy has drawn significant interest in the Society of Radiation Oncology as a potentially efficient and optimal treatment option for cancer patients. However, state-of-the-art spot-scanning proton arc therapy (SPArc) still relies on single or dual- co-planar arc trajectories, which limits its capacity to advance the treatment outcome further. There is an urgent need to explore particle beam thera

The history of optimization in radiation oncology is closely intertwined with the history ofPhysics in Medicine & Biology(PMB). Over the past four decades, many of the conceptual advances that transformed radiotherapy-from forward planning to inverse planning, from physical to biological optimization, and from treatment planning to treatment strategy optimization-were first reported in the pages of this journal. Looking back from the perspective of PMB's seventieth anniversary, one can trace a remarkably consistent intellectual trajectory: the progressive expansion of optimization from the des

Radiometric surveys in radiotherapy bunkers have been carried out in Brazil for many years, both by the same radiotherapy facility for verification of shielding as by the regulatory agency for licensing and control purposes. In recent years, the Intensity Modulated Radiation Therapy (IMRT) technique has been gradually incorporated into many facilities. Therefore, it has been necessary to consider the increased leakage component that has an important impact on the secondary walls. For that, a radiometric survey method has been used that considers an increased 'time of beam-on' for the secondary

PURPOSE: Minibeam radiation therapy (MBRT) is characterized by the delivery of submillimeter-wide regions of high "peak" and low "valley" doses throughout a tumor. Preclinical studies have long shown the promise of this technique, and we report here the first clinical implementation of MBRT.
METHODS AND MATERIALS: A clinical orthovoltage unit was commissioned for MBRT patient treatments using 3-, 4-, 5-, 8-, and 10-cm diameter cones. The 180 kVp output was spatially separated into minibeams using a tungsten collimator with 0.5 mm wide slits spaced 1.1 mm on center. Percentage depth dose (PDD)

In radiation therapy for glottic cancer, respiratory motion of larynx may change the dose variation in the target. The purpose of this study is to measure the respiratory motion of the larynx, and quantify the impact of the motion on the dose variation. This study included 10 patients treated by opposing portal irradiation for glottic cancer. We acquired fluoroscopy and respiratory waveform of the patients simultaneously and formulated the relationship between the displacement of larynx and the respiratory phase. We divided one field into 39 sub-fields on the basis of control points. Dose dist

This study aims to determine the optimal beam design among various combinations of field numbers and beam trajectories for intensity-modulated radiation therapy (IMRT) with simultaneous integrated boost (SIB) technique for the treatment of nasopharyngeal cancer (NPC). We used 10 fields with gantry angles of 155°, 130°, 75°, 25°, 0° L, 0° R, 335°, 285°, 230°, and 205° denoted as F10. To decrease doses in the spinal cord, the F10 technique was designed by featuring 2 pairs of split-opposed beam fields at 155° to 335° and 205° to 25°, as well as one pair of manually split beam fields at 0°. PURPOSE: As a challenging but important optimization problem, the inverse planning for volumetric modulated arc therapy (VMAT) has attracted much research attention. The column generation (CG) type method is so far one of the most effective solution schemes. However, it often relies on simplifications leading to significant gaps between the output and the actual feasible plan. This paper presents a novel column generation (NCG) approach to push the planning results substantially closer to practice.
METHODS: The proposed NCG algorithm is equipped with multiple new quality-enhancing and computa

A new acrylic version of the familiar Fletcher intracavitary applicator, the Ann Arbor (AA) applicator, has been developed. This new device eliminates the problem of "streak" artifacts on CT images, but unlike other plastic applicators the ability to shield portions of the bladder and rectum is retained through the use of tungsten alloy shields which are afterloaded with the radioactive sources. To minimize changes in placement geometry and to take advantage of the wide clinical experience with the Fletcher system, the new applicator nearly duplicates the physical dimensions of the Fletcher ap

PURPOSE: Ethos allows online adaption of radiotherapy treatment plans. Dose is calculated on synthetic computed tomographies (sCT), CT-like images generated by deforming planning CTs (pCT) onto daily cone beam CTs (CBCT) acquired during treatment sessions. Errors in sCT density distribution may lead to dose calculation errors. sCT correctness was investigated for bolus-covered surfaces.
METHODS: pCTs were recorded of a slab phantom covered with bolus of different thicknesses and with air gaps introduced by spacer rings of variable diameters and heights. Treatment plans were irradiated followi

BACKGROUND: To improve organ at risk (OAR) sparing, dynamic trajectory radiotherapy (DTRT) extends VMAT by dynamic table and collimator rotation during beam-on. However, comprehensive investigations regarding the impact of the gantry-table (GT) rotation gradient on the DTRT plan quality have not been conducted.
PURPOSE: To investigate the impact of a user-defined GT rotation gradient on plan quality of DTRT plans in terms of dosimetric plan quality, dosimetric robustness, deliverability, and delivery time.
METHODS: The dynamic trajectories of DTRT are described by GT and gantry-collimator pa

BACKGROUND: Intensity-modulated radiation therapy (IMRT) requires delivery quality assurance (DQA) to ensure treatment accuracy and safety. Irradiation techniques such as helical tomotherapy (HT) have become increasingly complex, rendering conventional verification methods insufficient. This study aims to develop a novel DQA system to simultaneously verify dose distribution and multi-leaf collimator (MLC) opening during HT.
METHODS: We developed a prototype detector consisting of a cylindrical plastic scintillator (PS) and a cooled charge-coupled device (CCD) camera. Scintillation light was r
