Influence of Cooling duration on Efficacy in Cardiac Arrest Patients (ICECAP): study protocol for a multicenter, randomized, adaptive allocation clinical trial to identify the optimal duration of induced hypothermia for neuroprotection in comatose, adult survivors of after out-of-hospital cardiac arrest.

BACKGROUND: Cardiac arrest is a common and devastating emergency of both the heart and brain. More than 380,000 patients suffer out-of-hospital cardiac arrest annually in the United States. Induced cooling of comatose patients markedly improved neurological and functional outcomes in pivotal randomized clinical trials, but the optimal duration of therapeutic hypothermia has not yet been established. METHODS: This study is a multi-center randomized, response-adaptive, duration (dose) finding, comparative effectiveness clinical trial with blinded outcome assessment. We investigate two populatio

Swedish trial on embolization of middle meningeal artery versus surgical evacuation in chronic subdural hematoma (SWEMMA)-a national 12-month multi-center randomized controlled superiority trial with parallel group assignment, open treatment allocation and blinded clinical outcome assessment.

BACKGROUND: Chronic subdural hematoma (cSDH) is one of the most common neurosurgical disorders and the incidence is rising. The routine treatment is neurosurgical hematoma evacuation, which is associated with recurrence rates up to 10-25%. In recent years, endovascular embolization of the middle meningeal artery (eMMA) has garnered much attention due to recurrence rates as low as < 5%. Several randomized controlled trials are planned or ongoing. In most of these trials, conventional neurosurgical treatment with or without adjunctive endovascular embolization is compared. The proposed trial aim

A Novel Method for Generating Spatially Resolved Synthetic Populations for Health Impact Assessments in Vulnerable Populations.

The spatial resolution of environmental exposure and sociodemographic population data is often mismatched given limited publicly available population data that complies with privacy requirements for individuals. To address this limitation, we developed a novel matching algorithm to construct a synthetic population at the address-level. To demonstrate how our approach can improve environmental justice (EJ) analyses and health impact assessments (HIAs), we examined sociodemographic patterns of residential proximity to major roadways in Greater Boston (Massachusetts) and HIA results, comparing ou

Assessing the performance of international pre-exposure prophylaxis (PrEP) eligibility guidelines in a cohort of Chinese MSM, Beijing, China 2009 to 2016.

INTRODUCTION: PrEP is a powerful HIV prevention tool, and locally relevant eligibility criteria are necessary to optimize the prevention impact of PrEP. We assessed performance of existing national and international PrEP eligibility criteria to predict future HIV seroconversion among MSM in Beijing, China. METHODS: Participants were MSM aged ≥18 years who enrolled in a cohort study between July 2009 and March 2016. Participants completed HIV testing, syphilis testing, and a questionnaire on recent sexual health behaviours at each follow-up visit and were followed until HIV seroconversion or d

Assessing the effects of limited vaccine supply on risk-based vaccination strategies in regions of endemic foot-and-mouth disease.

Foot-and-mouth disease (FMD) is an important livestock disease that is endemic in many regions of the world, many of which struggle to supply enough vaccine doses to cover the entire population of animals at risk. We use a previously fitted stochastic metapopulation model of FMD spread, utilising high-quality data from Erzurum in Turkey, to simulate four plausible vaccine allocation strategies in the context of limited vaccine supply and endemic disease. We find that random allocation outperforms other strategies when the objective is to minimise average prevalence, and random allocation also

Sex allocation of hermaphrodites in metapopulations with frequent population extinction and recolonization.

Competition among related males, male gametes, or pollen grains to fertilize a limited number of females, eggs, or ovules (local mate competition) is expected to favor female-biased progeny sex ratios in dioecious species. In hermaphrodites, such 'local mate competition' should similarly promote female-biased sex allocation, with reduced investment in sperm or pollen relative to eggs, ovules, or seeds. Because inbreeding, especially self-fertilization, tends to generate local mate competition, female-biased sex allocation should be associated with positive inbreeding, for example, as measured

Interpreting Randomized Controlled Trials.

This article describes rationales and limitations for making inferences based on data from randomized controlled trials (RCTs). We argue that obtaining a representative random sample from a patient population is impossible for a clinical trial because patients are accrued sequentially over time and thus comprise a convenience sample, subject only to protocol entry criteria. Consequently, the trial's sample is unlikely to represent a definable patient population. We use causal diagrams to illustrate the difference between random allocation of interventions within a clinical trial sample and tru

Randomness and inductions from streaks: "gambler's fallacy" versus "hot hand".

Sometimes people believe that a run of similar independent events will be broken (belief in the gambler's fallacy) but, other times, that such a run will continue (belief in the hot hand). Both of these opposite inductions have been explained as being due to belief in a law of small numbers. We argue that one factor that distinguishes these phenomena is people's beliefs about the randomness of the underlying process generating the events. We gave participants information about a streak of events but varied the scenarios in such a way that the mechanism generating the events should vary in how

Reporting of allocation method and statistical analyses that deal with bilaterally affected wrists in clinical trials for carpal tunnel syndrome.

The authors aimed to describe how often the allocation method and the statistical analyses that deal with bilateral involvement are reported in clinical trials for carpal tunnel syndrome and to determine whether reporting has improved over time. Forty-two trials identified from recently published systematic reviews were assessed. Information about allocation method and statistical analyses was obtained from published reports and trialists. Only 15 trialists (36%) reported the method of random sequence generation used, and 6 trialists (14%) reported the method of allocation concealment used. Of

Antimicrobial-impregnated central venous catheters for prevention of neonatal bloodstream infection (PREVAIL): an open-label, parallel-group, pragmatic, randomised controlled trial.

BACKGROUND: Bloodstream infection is associated with high mortality and serious morbidity in preterm babies. Evidence from clinical trials shows that antimicrobial-impregnated central venous catheters (CVCs) reduce catheter-related bloodstream infection in adults and children receiving intensive care, but there is a paucity of similar evidence for babies receiving neonatal intensive care. METHODS: This open-label, parallel-group, pragmatic, randomised controlled trial was done in 18 neonatal intensive care units in England.
