Phenotypic Characterization and Sorting of Circulating Tumor Cells (CTCs) Using a Digital Image-Based Sorter.

Circulating tumor cells (CTCs) are rare cells found in the bloodstream of patients with solid tumors, originating from the primary tumor and migrating toward distant anatomical sites. Recognized as key agents in the metastatic process, CTCs are essential to study in order to understand the underlying mechanisms of metastasis. However, CTC analysis is challenging due to their extreme rarity in the bloodstream (approximately one CTC per 106-107 white blood cells). Dielectrophoretic (DEP)-based digital imaging and sorting technology enables the visualization and isolation of single, labeled cells

Centenarians and supercentenarians: a black swan. Emerging social, medical and surgical problems.

The Black Swan Theory was described by Nassim Nicholas Taleb in his book "The Black Swan". This theory refers to "high-impact, hard-to-predict, and rare events beyond the realm of normal expectations". According to Taleb's criteria, a Black Swan Event is a surprise, it has a major impact and after the fact, the event is rationalized by hindsight, as if it had been expected. For most of human history centenarians were a rare and unpredictable phenomenon. The improvements of the social-environmental conditions, of medical care, and the quality of life caused a general improvement of the health s

Distractions in Forensic Evaluations.

Although test manuals and professional guidelines universally specify that forensic assessments should take place in quiet, private, and distraction-free environments, such absences of distractions are rare in jail and prison settings. In this article, four aspects of this problem are examined. First, compelling examples of noise and distractions are described to give a sense of the nature of the problem. Second, the portions of guidelines from the American Academy of Psychiatry and the Law and the American Psychological Association regarding distractions are presented, along with associated e

Double true umbilical cord knots coexisting with a nuchal cord with successful fetal outcome: A case report.

Umbilical nodes and cords play a crucial role in fetal development and are essential for the transfer of nutrients and oxygen between the mother and the fetus. Sonographic diagnosis of umbilical nodes and cords has become an integral part of prenatal care, allowing for the early detection of abnormalities and potential complications. The umbilical cord is a vital structure connecting the fetus to the placenta, providing essential nutrients and oxygen for fetal growth and development. Sonographic examination of the umbilical cord and its associated nodes has become an indispensable tool in pren

Confident but unsupported: auditing large language models against supplied evidence boundaries in drug-induced liver injury assessment.

INTRODUCTION: Large language models (LLMs) are entering drug-safety and regulatory workflows, yet their behavior at the boundary between unavailable evidence and risk reassurance remains poorly characterized. Drug-induced liver injury (DILI) is a stringent setting because evidence is fragmented across labels, case reports, mechanistic studies, and curated knowledge bases, while unsupported low-risk reassurance can be consequential. METHODS: We evaluated five LLMs on a fixed 47-drug DILI risk-assessment panel using 1,410 parsed responses from closed-book answering and an evidence-gated protoco

The Road to Nowhere: Being Rooted in a Region Does Little to Predict Support for Populist Parties or Opposition to Migration and Social Change.

In his influential book The road to somewhere David Goodhart claims that a split between a majority of people who are rooted in particular places or groups (the "Somewheres") and a minority of people who are or can be at home anywhere in the world (the "Anywheres") is at the heart of many contemporary political conflicts. According to him, the Anywheres dominate public discourse and political decisions, while the preferences of the Somewheres are neglected. Goodhart's arguments have been widely cited but rarely systematically tested. We conduct such a test for the following three central claim

Book-embedded hair reveals mineral-rich diets among urban commoners in early modern to modern Japan.

Urban populations in early modern to modern Japan depended strongly on rice and marine fish, raising the question of whether such apparently simple diets provided sufficient mineral availability in everyday life. Here, we analyze human hair preserved in historical book covers to reconstruct elemental exposure among urban commoners from the seventeenth to nineteenth centuries. Screening approximately 70,000 volumes allowed individual hair samples to be linked to specific cities and years through colophon records, enabling a rare population-scale reconstruction from abundant and well-dated mater

Laryngeal paragangliomas - controversies in diagnosis and management.

Paragangliomas of the larynx are rare neuroendocrine tumors with difficulties in diagnosis and management. A review of the literature was carried out, using Medline and other available databases. Electronic links and related books were also included. The most reliable diagnostic imaging procedures are MRI, CT scan, and octreotide scintigraphy. Complete surgical excision with the maximal possible preservation of the laryngeal function is the treatment of choice. Many different surgical techniques have been reported but open surgical procedures seem to provide better results with lower recurrenc

Repurposing of a gill gene regulatory program for outer-ear evolution.

How new structures emerge during evolution has long fascinated biologists. An example is how the diminutive bones of the mammalian middle ear arose from ancestral fish jawbones1. By contrast, the evolutionary origin of the outer ear, another mammalian innovation, remains a mystery, partly because it is supported by non-mineralized elastic cartilage, which is rarely recovered in fossils. Whether the outer ear arose de novo or through the reuse of ancestral developmental programs has remained unknown. Here we show that the outer ear shares gene regulatory programs with the gills of fishes and am

Update from the 4th Edition of the World Health Organization Classification of Head and Neck Tumours: What Is New in the 2017 WHO Blue Book for Tumors and Tumor-Like Lesions of the Neck and Lymph Nodes.

The World Health Organization (WHO) 2017 Classification of Head and Neck Tumors ("Blue Book") will now include a new chapter on tumors and tumor-like lesions of the neck and lymph nodes, which was not included in the previous edition. Tumors and tumor-like lesions, including a variety of cysts and metastases, can arise in any component in the neck, including soft tissue, lymph nodes, and developmental remnants. The pathology and clinical features of metastatic carcinoma of unknown primary in the head and neck has changed dramatically in the last several years. Many of these tumors which were p

Gametophytic Apomixis: An Introduction.

Apomixis, clonal reproduction through seed, is divided into sporophytic (nucellar embryony) and gametophytic apomixis. This book focuses on the latter type. Gametophytic apomixis, while rare, is taxonomically widespread, notably among grasses and Asteraceae. Detection methods have advanced significantly, employing flow cytometry and advanced microscopy techniques. The genetic, molecular, and developmental mechanisms behind apomixis involve modifications preventing meiosis and enabling parthenogenesis. Understanding these mechanisms and applying approaches such as forward genetics and mutagenes

Dr Ayub Khan Ommaya (1930-2008): The eventful life of a revolutionary neurosurgeon.

Dr Ayub Khan Ommaya (1930-2008) was a pioneering figure in the field of neurosurgery, with a particular focus on traumatic brain injury. As history books have held, he was a man of great intellect and vision, possessing a rare combination of scientific rigour and compassionate empathy. One of Dr Ommaya's most notable contributions was his development of the Ommaya reservoir, a device used to deliver drugs directly into the brain. This groundbreaking technology transformed the treatment of brain tumours and other neurological disorders, enabling clinicians to administer medications with unprece

Rare disease monitoring plans: a case study within a clinical decision support system.

INTRODUCTION: Clinical decision support systems (CDSS) have emerged as valuable tools for enhancing healthcare for rare diseases. Nonetheless, most tools focus on diagnosis, while few support patient monitoring. We aim to report the methods to develop an evidence-based CDSS for monitoring rare diseases, using Bardet-Biedl Syndrome (BBS) as a case study. METHODS: We assembled a multidisciplinary team of over 40 healthcare providers from 11 specialities to develop rare disease monitoring plans. We conducted a scoping review to map the existing literature on BBS monitoring, followed by the syste

A Primary Urothelial Carcinoma Presenting as a Mid-Bulbar Urethral Stricture in a 30-Year-Old Male.

The bulbar urethra is the most common site of anterior urethral strictures. In this case report, we present a 30-year-old male who was referred to us as a case of mid-bulbar urethral stricture. Urethroplasty was booked and a papillary lesion was found on routine diagnostic cystoscopy. An open biopsy was taken which showed invasive high-grade papillary urothelial carcinoma with squamous differentiation. This case is rare in terms of a young age of incidence, a lack of risk factors, an absence of suspicious symptoms, and a short duration of signs and symptoms. Urologists should consider workup f

Spontaneous liver rupture in pre-eclampsia.

Spontaneous liver rupture is a rare complication of pre-eclampsia. A booked, 30-year old woman with pre-eclampsia and twin gestation developed severe abdominal pains 10 hours after a supervised, vaginal delivery. On examination she was in hypovolemic shock with abdominal distension from hemoperitoneum. Uterine rupture was suspected and she had a laparotomy after resuscitation. But at surgery the uterus was intact and instead liver rupture was found which was managed by omental packing after evacuating the clots. Postoperatively, the patient developed acute renal failure that responded well to
