Glucagon-like peptide-1 receptor agonists in rheumatoid arthritis.

PURPOSE OF REVIEW: This article synthesizes current preclinical and clinical literature regarding glucagon-like peptide-1 receptor agonists (GLP-1 RAs) in rheumatoid arthritis (RA), highlights mechanistic hypotheses and explores potential clinical roles and limitations of their use. RECENT FINDINGS: GLP-1 RAs are established therapies for type 2 diabetes mellitus and obesity, with demonstrated benefits on glycemic control and cardiovascular risk reduction. Emerging evidence suggests that GLP-1 RAs may exert anti-inflammatory and immunomodulatory effects relevant to RA. SUMMARY: Present evide

Glucagon-Like Peptide-1 Receptor Agonist-Associated Hepatotoxicity.

PURPOSE: Glucagon-like peptide-1 receptor agonists (GLP-1RAs) have transformed the management of obesity, type 2 diabetes and metabolic dysfunction-associated steatohepatitis. Despite their numerous clinical benefits they are not without risk. Although the product labels contain multiple organ-specific warnings and precautions, the liver is not identified as a target organ of safety concern. Drug-induced liver injury (DILI) is a leading safety-related cause of pipeline attrition, boxed warnings and postmarketing drug withdrawal. This review explores GLP-1RA associated hepatotoxicity. METHODS:

Glucagon receptor agonist aggravates a murine model of Balkan nephropathy.

Combining agonists of glucagon receptors (GCGRA) and GLP1 receptors (GLP1RA) is explored for treatment of obesity, but little is known on kidney protection by GCGRA. To gain further insights, mice with Western diet-induced obesity were first treated for 18 days with a GLP1RA or a GCGRA, and then exposed to tubule-toxic aristolochic acid (AA), a murine model of Balkan nephropathy. Prior to AA application, GLP1RA reduced food intake by ~30% and body weight by ~16% vs. vehicle with no significant changes in non-fasted blood glucose levels or plasma creatinine or urea. GCGRA reduced body weight by

Glucagon-like peptide-1 receptor: mechanisms and advances in therapy.

The glucagon-like peptide-1 (GLP-1) receptor, known as GLP-1R, is a vital component of the G protein-coupled receptor (GPCR) family and is found primarily on the surfaces of various cell types within the human body. This receptor specifically interacts with GLP-1, a key hormone that plays an integral role in regulating blood glucose levels, lipid metabolism, and several other crucial biological functions. In recent years, GLP-1 medications have become a focal point in the medical community due to their innovative treatment mechanisms, significant therapeutic efficacy, and broad development pro

Glucagon Receptor Antagonist for Heart Failure With Preserved Ejection Fraction.

BACKGROUND: Heart failure with preserved ejection fraction (HFpEF) is an emerging major unmet need and one of the most significant clinic challenges in cardiology. The pathogenesis of HFpEF is associated with multiple risk factors. Hypertension and metabolic disorders associated with obesity are the 2 most prominent comorbidities observed in patients with HFpEF. Although hypertension-induced mechanical overload has long been recognized as a potent contributor to heart failure with reduced ejection fraction, the synergistic interaction between mechanical overload and metabolic disorders in the

Glucagon receptor gene mutations with hyperglucagonemia but without the glucagonoma syndrome.

Pancreatic neoplasms producing exclusively glucagon associated with glucagon cell hyperplasia of the islets and not related to hereditary endocrine syndromes have been recently described. They represent a novel entity within the panel of non-syndromic disorders associated with hyperglucagonemia. This case report describes a 36-year-old female with a 10 years history of non-specific abdominal pain. No underlying cause was evident despite extensive diagnostic work-up. More recently she was diagnosed with gall bladder stones. Abdominal ultrasound, computerised tomography and magnetic resonance im

Effect of pinealectomy on liver insulin and glucagon receptor concentrations in the rat.

The studies described here were undertaken to characterize the hepatic insulin and glucagon receptors of control (C), pinealectomized (Pn), and melatonin-treated pinealectomized (Pn + Mel) rats. Compared with C rats, an increase in plasma glucose and glucagon levels and a reduction in circulating concentrations of insulin in Pn animals were observed. Melatonin treatment of Pn rats reverses all three parameters toward the normal values. In liver membranes, insulin binding was lower in Pn than in C rats, and glucagon binding was greater in Pn than in C animals; in Pn + Mel rats both insulin and

The 14-3-3 protein YWHAB inhibits glucagon-induced hepatic gluconeogenesis through interacting with the glucagon receptor and FOXO1.

Glucagon antagonism has been reported as a new therapeutic approach to hyperglycaemia. As the 14-3-3 protein YWHAB has been identified as a regulator of the glucagon receptor (GCGR) by affinity purification and mass spectrometry, we examined the role of YWHAB in vivo. Ywhab knockout mice display impaired blood glucose homeostasis only under pyruvate stimulation. Deletion of Ywhab in mouse primary hepatocytes (MPHs) increases hepatocyte glucose production by magnifying the effect of glucagon. Mechanistic analysis indicates that YWHAB forms a phosphorylation-dependent complex with GCGR and direc

Glucagon receptor deficiency is associated with glucocorticoid receptor activation and proteolytic remodeling in gastrocnemius of male mice.

Glucagon signaling through the glucagon receptor (GCGR) plays a central role in systemic metabolic regulation. However, its role in skeletal muscle protein homeostasis remains poorly understood. In this study, we demonstrated that skeletal muscle from male GCGR-deficient mice exhibited preferential gastrocnemius atrophy accompanied by elevated intramuscular free amino acid levels and hyperaminoacidemia. Transcriptomic and biochemical analyses revealed activation of glucocorticoid receptor (GR) signaling together with induction of proteolytic pathways, as reflected by increased tripartite motif

Regulation of glucagon-like peptide-1 receptor and calcium-sensing receptor signaling by L-histidine.

Receptor-specific agonists of the extracellular calcium-sensing receptor (CaSR) potentiate glucose-induced insulin secretion, an effect similar to that of glucagon-like peptide-1 (GLP-1). We have sequenced the full open reading frame of the CaSR from rat insulinoma (INS-1) cells and find that the predicted amino acid sequence of the receptor is identical with that of the receptor from the parathyroid gland. This receptor couples to both Gq/11 and Gi/o, and this dual coupling may partly explain the varying effects of nonspecific agonists on secretion reported previously. L-Histidine (L-His) inc

Finerenone attenuates downregulation of the kidney GLP-1 receptor and glucagon receptor and cardiac GIP receptor in mice with comorbid diabetes.

BACKGROUND: Several new treatments have recently been shown to have heart and kidney protective benefits in people with diabetes. Because these treatments were developed in parallel, it is unclear how the different molecular pathways affected by the therapies may overlap. Here, we examined the effects of the mineralocorticoid receptor antagonist finerenone in mice with comorbid diabetes, focusing on the regulation of expression of the glucagon-like peptide-1 receptor (GLP-1R), gastric inhibitory polypeptide receptor (GIPR) and glucagon receptor (GCGR), which are targets of approved or investig

Receptor Activity-Modifying Protein 2 (RAMP2) alters glucagon receptor trafficking in hepatocytes with functional effects on receptor signalling.

OBJECTIVES: Receptor Activity-Modifying Protein 2 (RAMP2) is a chaperone protein which allosterically binds to and interacts with the glucagon receptor (GCGR). The aims of this study were to investigate the effects of RAMP2 on GCGR trafficking and signalling in the liver, where glucagon (GCG) is important for carbohydrate and lipid metabolism. METHODS: Subcellular localisation of GCGR in the presence and absence of RAMP2 was investigated using confocal microscopy, trafficking and radioligand binding assays in human embryonic kidney (HEK293T) and human hepatoma (Huh7) cells. Mouse embryonic fi

[Lixisenatide (Lyxumia), a new agonist of glucagon-like peptide-1 receptors with a predominant postprandial action].

Lixisenatide (Lyxumia) is a new agonist of Glucagon-Like Peptide-1 (GLP-1) receptors that is indicated in the treatment of type 2 diabetes, in one single subcutaneous daily injection of 20 microg. It exerts an incretin effect by stimulating insulin secretion after a meal while inhibiting glucagon secretion, both in a glucose-dependent manner, which limits the risk of hypoglycaemia. In addition, it slows down gastric emptying after a meal, which contributes to reduce postprandial hyperglycaemia, especially after breakfast. Lixisenatide is currently reimbursed in Belgium after failure of a dual

An experimental medicine protocol for exploring the haemodynamic effects of dual agonism at the glucagon-like peptide-1 and glucagon receptor in healthy subjects.

AIMS: Glucagon-like peptide-1 (GLP-1) and glucagon dual receptor agonists are in clinical development for a range of metabolic conditions, including type 2 diabetes and obesity. The cardiovascular actions at these receptors are well studied, but less is known about their combination. The aim was to explore the acute haemodynamic effects of dual agonism at the GLP-1 and glucagon receptor. METHODS: Healthy male participants attended randomized, saline-controlled intravenous infusion studies using glucagon (low, 25 ng/kg/min), glucagon (high, 50 ng/kg/min), exenatide (loading dose 50 ng/min for

Association of glucagon-like peptide 1 receptor and the aggressiveness of extrahepatic cholangiocarcinoma.

Previous studies demonstrated that the suppression of GLP-1R expression significantly reduced the aggressiveness of intrahepatic cholangiocarcinoma (iCCA) cells, in vitro and in vivo. However, the clinical significance of GLP-1R in extrahepatic cholangiocarcinoma (eCCA) remains unclear, although this subtype is the most common of bile duct cancer. This study thus aimed to investigate the clinicopathological significance of GLP-1R and to determine its functional roles in eCCA. GLP-1R expression in eCCA tissues was determined using immunohistochemistry. The associations between GLP-1R expression

Glucagon-like peptide-1 receptor agonists and pancreatitis: A reconcilable divorce.

Early clinical trials suggested that patients treated with glucagon-like peptide-1 (GLP-1) receptor agonists had a slightly increased risk of acute pancreatitis. Consequently, clinicians have avoided using GLP-1 receptor agonists in patients with a history of acute pancreatitis. However, recent large meta-analyses of clinical trial data do not support a class-wide risk. Denying these valuable therapeutic medications to patients with a history of pancreatitis seems unwarranted.
