The relation between vaccination status and referral to a consultation-liaison psychiatry service for hospitalized patients with COVID-19.

BACKGROUND: Previous studies have shown that patients with Coronavirus Disease 2019 (COVID-19) are likely to be affected by delirium and other psychiatric complications. We aimed to evaluate the relation between COVID-19 vaccination status and referral of patients hospitalized with COVID-19 for consultation-liaison psychiatry services. METHOD: From the medical records used for this retrospective, single hospital-based study, 576 patients were identified who were over 18 years-of-age and hospitalized with a diagnosis of COVID-19 between March 2020 and March 2022. The data of 531 for whom the v

Association of Postpartum Inpatient Consultation Compared With Outpatient Referral With Hepatitis C Virus Treatment Completion.

OBJECTIVE: To evaluate the associations of a traditional postpartum outpatient referral compared with an opportunistic inpatient consultation for hepatitis C virus (HCV) treatment initiation on HCV treatment completion. METHODS: We performed a retrospective evaluation of the HCV perinatal care cascade for women who delivered at Barnes-Jewish Hospital between January 1, 2020, and September 1, 2023. We compared an intervention that provided immediate HCV treatment on discharge after delivery with referral to postpartum outpatient HCV care. Obstetric patients were allocated to either inpatient t

Addressing Child and Adolescent Mental Health Problems in the Community. Evaluation of a Consultation and Advise Team for Assessment, Support and Referral.

INTRODUCTION: Youths with mental health problems are often not identified in primary healthcare, which may prevent or delay appropriate support. In the Netherlands, a Consultation and Advise expert team (CandA team) was implemented to support general practitioners (GPs), youth professionals and youths with mental health problems. This study investigates the team's scope, activities, stakeholders' and users' experiences. METHOD: Interviews and focus groups with policymakers, healthcare professionals, parents and youths were analysed using ATLAS.ti. Demographics and mental health problems of 70

Specialized consultations in a hospital-based referral center for patients suspected of having limb lymphedema: Impact on diagnosis.

Lymphedema, a chronic debilitating disease, is not always easily diagnosed. A total of 254 new patients ((217 women, 37 men), median (Q1-Q3) age 61 (46-72) years) were referred for suspected limb lymphedema to an exclusively lymphedema-dedicated department for a first consultation (January - March 2015) were included; among 118 with upper limb involvement, 100 (84.7%) were diagnosed with post-breast cancer therapy and four with primary lymphedemas; among 136 with lower limb involvement, 31 (22.8%) were diagnosed with primary lymphedemas and 35 (25.7%) with post-cancer lymphedemas. The main aim of this study was to examine the prevalence of antenatal depression and other psychiatric conditions in women referred to a consultation liaison psychiatry service because of positive scores on the Edinburgh Postnatal Depression Scale. The other aim was to review known risk factors in the women and note any significant findings. An audit of all women referred to the psychiatry team because of positive Edinburgh scores during a 2-year period was completed. Informa

This study aims to investigate the multilevel patient characteristics associated with higher frequency of receiving e-consults and increased likelihood of completion. Materials and Methods: University of Colorado's electronic medical record were analyzed to study factors influencing referral types (e-consult vs. standard) and their completion rates from April 2018 to September 2023. Multivariate probit regression assessed the

Pre-referral GP consultations in patients subsequently diagnosed with rarer cancers: a study of patient-reported data.

BACKGROUND: Some patients with cancer experience multiple pre-diagnostic consultations in primary care, leading to longer time intervals to specialist investigations and diagnosis. Patients with rarer cancers are thought to be at higher risk of such events, but concrete evidence of this is lacking. AIM: To examine the frequency and predictors of repeat consultations with GPs in patients with rarer cancers. DESIGN AND SETTING: Patient-reported data on pre-referral consultations from three English national surveys of patients with cancer (2010, 2013, and 2014), pooled to maximise the sample si

Nurse-identified needs, recommendations, and referrals during consultations with family carers of patients diagnosed with high-grade glioma.

BACKGROUND: The aim of this manuscript was to describe the frequency and types of nurse-identified needs, recommendations, and referrals delivered to intervention-arm carers of patients with high-grade glioma (HGG) during monthly nurse-led telephone assessments in the Care-IS randomized controlled trial. METHODS: Primary carers of patients diagnosed with HGG (≤2 months prior) and undergoing active treatment were randomized to: a 12-month Care-IS intervention; or, a control group. Carers randomized to the intervention were included in this sub-analysis. Nurses recorded carers' needs and made r

Patterns of comorbidity in relation to functioning (GAF) among general hospital psychiatric referrals. European Consultation-Liaison Workgroup.

Improvement of services for physically ill patients with concurrent psychiatric problems is a noteworthy issue in general hospitals. Among 1249 general hospital in-patients referred for psychiatric consultation, concurrent mental and behavioural disorders (ICD-10) were diagnosed in 84% of cases. Any concurrent mental and behavioural disorder was associated with dramatically low functioning (GAF = 46), indicating serious psychosocial impairment. This is a striking new finding of clinical importance. The effect on functioning was similar for substance use disorders, other mental disorders, and c

Referral of adults with obstructive sleep apnea for surgical consultation: an American Academy of Sleep Medicine systematic review, meta-analysis, and GRADE assessment.

INTRODUCTION: This systematic review provides supporting evidence for the accompanying clinical practice guideline on the referral of adults with obstructive sleep apnea (OSA) for surgical consultation. METHODS: The American Academy of Sleep Medicine commissioned a task force of experts in sleep medicine. A systematic review was conducted to identify studies that compared the use of upper airway sleep apnea surgery or bariatric surgery to no treatment as well as studies that reported on patient-important and physiologic outcomes pre- and postoperatively. Statistical analyses were performed to

Reducing referral-to-consultation delays in pediatric obstructive sleep apnea through standardized triage: A regional quality improvement initiative.

BACKGROUND: Timely management of pediatric obstructive sleep apnea (OSA) is essential to prevent sequelae. In publicly funded health systems, delays remain common, particularly in non-urban regions with limited specialty capacity. At a regional hospital serving Québec's Bas-Saint-Laurent region, prolonged wait times for adenotonsillectomy were observed. Preliminary review identified referral-to-consultation delay (Wait-1) as the principal access bottleneck, exceeding Pediatric Canadian Access Targets for Surgery. METHODS: Using SQUIRE 2.0 guidelines, we conducted a quality improvement initiat

Agreement to Referral for Consultation and Utilization of Reproductive Endocrinology and Infertility Services among Female Childhood Cancer Survivors from a Regional Childhood Cancer Survivorship Clinic.

Counseling on infertility risk and fertility preservation (FP) is recommended for childhood cancer survivors (CCS). Of 80 eligible CCS without prior FP or post-therapy REI referrals, 7 (9%) agreed to REI referrals, 1 (1%) attended REI consults, and 0 underwent FP. Survivors who agreed and did not agree to REI referrals had similar characteristics and infertility risk. Research to better und
