Interventional neuroradiology : journal of peritherapeutic neuroradiology, surgical procedures and related neurosciences2018eng
There are fewer than 20 published case reports of bradycardia or asystole during intracranial embolisation procedures. These are well described in open neurosurgical procedures, particularly involving the skull base. We present a case of a 59-year-old male patient who presented for elective embolisation of a dural arteriovenous fistula. During the injection of Onyx, the patient experience sudden asystole, which recurred after a second Onyx injection. Following successful treatment, a third injection proceeded without incident.

BACKGROUND: Trigeminocardiac reflex (TCR) is a well-known phenomenon that causes bradycardia, hypotension, and cardiac arrest through mechanical stimulation of the trigeminal nerve distribution. Although TCR has been reported during posterior fossa surgeries, cardiac arrest during supratentorial aneurysm clipping is exceptionally rare.
OBSERVATIONS: An 88-year-old woman with a ruptured internal carotid-posterior communicating artery aneurysm underwent craniotomy via a pterional approach. During clip application using a slightly curved Sugita no. 65 clip inserted along the sphenoid ridge dura,

BACKGROUND: A sudden attack of an asystolic phenomenon is an extremely rare event during transsphenoidal surgery (TSS). It may be caused by an extreme type of trigeminocardiac reflex (TCR) during the manipulation of the trigeminal nerve or its innervated structures.
CASE DESCRIPTION: We report two cases of sudden asystole and a case of severe bradycardia due to TCR during TSS. All patients were managed successfully by cessation of the surgical manipulation or with the injection of an anticholinergic agent.
CONCLUSIONS: Although TCR occurs rarely and usually is self-limiting, surgeons should

Trigeminocardiac reflex (TCR) is a physiological response that occurs due to stimulation of the trigeminal nerve, resulting in vagus nerve activation, parasympathetic dysrhythmia, and sympathetic hypotension. The pediatric population is vulnerable to TCR due to higher resting vagal tone, which can result in ischemia and death. This study reports two cases of TCR during adenotonsillectomy. Placement and rapid opening of a Crowe-Davis retractor led to immediate bradycardia and hypotension in two children undergoing adenotonsillectomy, which resolved upon release of the retractor. Early intraoper

This paper reports the incidence of tri-geminocardiac reflex (TCR) in endovascular treatment of dural arteriovenous fistulas (DAVFs) with Onyx. The consecutive case histories of 45 patients with DAVFs, treated with Onyx transarterially and transvenously, from February 2005 to February 2008 at Beijing Tiantan Hospital, China, were retrospectively reviewed. The time period was limited as the anesthetic and intravascular procedure was performed under the same standardized anesthetic protocol and by the same team. The TCR rate was subsequently calculated. Of the 45 patients, five showed evidence o

The trigeminocardiac reflex (TCR) occurs when branches of the trigeminal nerve are stimulated by physical and/or chemical triggers, leading to cardiac changes. It is commonly associated with midface manipulation, especially in the orbital region and similar incidents with mandibular procedures are not frequently reported. A girl in her late teens with no significant medical history or concurrent medications developed asystole during a bilateral sagittal split osteotomy (BSSO) under general anaesthesia. Prompt cardiopulmonary resuscitation led to the return of spontaneous circulation. The surge

INTRODUCTION AND IMPORTANCE: The trigeminocardiac reflex (TCR) is an uncommon but significant complication that can arise during orthognathic surgery.
CASE PRESENTATION: We present two cases of TCR-induced bradycardia and hypotension during such procedures. In the first case, bradycardia was noted following a controlled downward fracture of the maxilla. In the second case, both bradycardia and hypotension occurred while the left mandibular ramus was split with a separator.
CLINICAL DISCUSSION: In both instances, interruption of the surgical procedure led to normalization of heart rate and bl

BACKGROUND: Percutaneous compression of the trigeminal ganglion (PCTG) is an alternative surgical treatment for trigeminal neuralgia (TN). Manipulation of PCTG can lead to significant hemodynamic changes, which may increase the risk of cardiovascular complications. However, to our knowledge, few studies have focused on anesthesia experience during PCTG as treatment for TN so far. It was our primary focus on how to ensure the stability of hemodynamics during our clinical anesthesia experience. This study aimed to compare the study group (using sodium nitroprusside [SNP] as soon as the puncture

In the present study we have extended our previous findings about the effects of 10 minutes of passive mandibular extension in anesthetized Wistar rats. By prolonging the observation time to 3 hours, we showed that 10 minutes mandibular extension caused a significant reduction of the mean arterial blood pressure and heart rate respect to baseline values, which persisted up to 160 minutes after mandibular extension. These effects were accompanied by a characteristic biphasic response of pial arterioles: during mandibular extension, pial arterioles constricted and after mandibular extension dila

The trigeminocardiac reflex (TCR) is an autonomic reflex elicited by stimulation of the trigeminal nerve and is most commonly characterized by bradycardia and hypotension. In oral and maxillofacial surgery, peripheral TCR may occur during manipulation of the maxillary or mandibular divisions, occasionally presenting with atypical cardiovascular responses. We report a rare case of peripheral TCR manifesting as sudden severe hypertension with ventricular bigeminy during wide local excision and marginal mandibulectomy in a 69-year-old male with carcinoma of the lower alveolus. The hemodynamic dis

RATIONALE: Trigeminocardiac reflex (TCR) sometimes occurs during the percutaneous balloon compression (PBC) procedure to treat trigeminal neuralgia (TN), and it manifests as transient bradycardia or sinus arrest. However, recurrent intraoperative TCR cases are rarely reported. Meanwhile, the treatment for recurrent TCR is still unclear.
PATIENT CONCERNS: A 74-year-old male with a 2-year TN history could no longer tolerate the side effects of carbamazepine and came to seek PBC treatment.
DIAGNOSES: Bradycardia or sinus arrest occurred repeatedly during the operation, and the heart rate (HR) r

BACKGROUND/PURPOSE: Trigeminocardiac reflex (TCR) is a unique clinical incident of acute change in hemodynamic balance, which may lead to hypotension, bradycardia, and even clinical crisis. Up to date, no study so far considers the impact of non-surgical root canal treatment (NSRCT) of irreversible pulpitis teeth under either local infiltration or block anesthesia on hemodynamic change possibly related to TCR.
METHODS: This study enrolled 111 patients with 138 irreversible pulpitis teeth that were treated by two sessions of NSRCT. The first session involved mainly the removal of vital pulp ti

BACKGROUND: The occurrence of trigeminocardiac reflex (TCR) is known to be a negative prognostic factor for hearing preservation in cerebellopontine angle tumor surgery. Our study was conducted to investigate brainstem auditory evoked potential (BAEP) changes after this reflex in cerebellopontine angle tumor surgery and to evaluate their impact on postoperative hearing function.
METHODS: Five of 102 consecutive patients had an intraoperative TCR (4.9%) and were retrospectively evaluated for the intraoperative BAEP changes after TCR and postoperative auditory function (7 to 10 d after surgery)

The trigeminocardiac reflex (TCR) is a rare brainstem reflex characterized by bradycardia and hemodynamic instability secondary to stimulation of branches of the trigeminal nerve. In maxillofacial surgery, the phenomenon has been reported in orthognathic surgery, foreign body removal, and fixation of midface fractures. Despite reasonably extensive reporting of the reflex in head and neck surgery in general, the mechanism and predisposing factors are poorly understood. The primary aim of this systematic review was to explore potential contributory factors and summarize outcomes of the TCR in pa

PURPOSE: Temporomandibular joint (TMJ) surgery is being increasingly performed globally with considerable success leading to improved quality of life for patients affected with disabling temporomandibular disorders. One of the most unusual phenomena noted during maxillofacial surgery due to stimulation of the trigeminal nerves is the Trigeminocardiac reflex (TCR), which causes sudden bradycardia and hypotension causing alarm and distress to the surgical and anesthetic team. The purpose of this systematic review is to identify the frequency and discuss the pathophysiology of the TCR especially

BACKGROUND: The trigeminocardiac reflex (TCR) is characterized by bradycardia, decrease of mean arterial blood pressure, and sometimes, asystole during surgery. We critically reviewed TCR studies and devised a novel classification scheme for assessing the reflex.
METHODS: A comprehensive systematic literature review was performed using PubMed, MEDLINE, Web of Science, EMBASE, and Scielo databases. Eligible studies were extracted based on stringent inclusion and exclusion criteria. Statistical analyses were used to assess cardiovascular variables. TCR was classified according to morphophysiolo

The trigeminocardiac reflex is a rare occurrence in patients undergoing maxillofacial surgery, with a reported incidence of 1-2%. Clinical signs and symptoms include bradycardia, nausea, with further stimulation potentially leading to cardiac dysrhythmias, ectopic beats, atrioventricular blocks and asystole. Most maxillofacial procedures, including temporomandibular joint procedures, are considered low risk. We report the first case of a tender temporomandibular joint synovial cyst whose management was complicated by severe trigeminocardiac reflex resulting in asystole. We suggest that in such

Stimulation of the trigeminal nerve evokes a dramatic decrease in heart rate and blood pressure, and this reflex has generally been termed the trigeminocardiac reflex. A subset of the trigeminocardiac reflex is the diving reflex in which the nasal mucosa is stimulated with water or air-borne chemical irritants. Activation of the diving reflex evokes a pronounced bradycardia, mediated by increased parasympathetic cardiac activity, and is the most powerful autonomic reflex. However, exaggeration of this protective response could be detrimental and has been implicated in Sudden Infant Death Syndr

OBJECT: Surgical manipulation of the fifth cranial nerve during its intra- or extracranial course may lead to bradycardia or even asystole as well as arterial hypotension, a phenomenon described as the trigeminocardiac reflex (TCR). The authors studied the impact of this reflex on postoperative auditory function in patients undergoing vestibular schwannoma (VS) surgery.
METHODS: One hundred patients scheduled for VS surgery were studied prospectively for parameters influencing the postoperative auditory function. The evaluation included sex, age, pre- and postoperative auditory function, preo
