Vestibulo-Ocular Reflex Dysconjugacy Associated With Central Cause of Dizziness.

OBJECTIVES: Most current video head impulse test (vHIT) systems take monocular recordings, which is sufficient to identify a peripheral vestibulopathy. Given central ocular motor abnormalities can cause dysconjugacy between the eyes, binocular vHIT may help further identify central/dangerous lesions. Here, we examined the utility of binocular vHIT to differentiate central from peripheral vestibulopathy. DESIGN: We enrolled three groups: healthy volunteers, consecutive patients with peripheral vestibulopathy, and consecutive patients with central vestibulopathy, respectively. All subjects unde

Contralesional High-Acceleration Vestibulo-Ocular Reflex Function in Vestibular Schwannoma.

UNLABELLED: Impairment of ipsilesional vestibulo-ocular reflex (VOR) function is well described in vestibular schwannoma (VS) and a correlation between gain of the VOR and tumor size has been suggested. Bilateral VOR impairment may also occur in VS patients, but its mechanisms are poorly understood. We sought to explore the effect of unilateral VS on ipsilesional and contralesional high-acceleration VOR function using video head impulse testing, and evaluate potential factors responsible for contralesional VOR impairment. MATERIALS AND METHODS: Chart review in tertiary referral center of pati

Selective suppression of the vestibulo-ocular reflex during human locomotion.

INTRODUCTION: In lower vertebrates, gaze stabilization during locomotion is at least partially driven by a direct coupling of spinal locomotor commands with extraocular motor signals. To what extent locomotor feed-forward mechanisms contribute to gaze stabilization during human locomotion is yet unknown. In principle, the feasibility of a feed-forward regulation of gaze during locomotion should critically depend on the spatiotemporal coupling between body and head kinematics and hence the internal predictability of head movements (HMP). The present study thus investigated whether changes in ey

Vestibulo-ocular reflex in patients with Meniere's disease between attacks.

In Meniere's disease, spontaneous nystagmus beating toward the affected ear (ipsilateral nystagmus) is frequently observed especially during vertiginous periods. VOR against horizontal rotation was recorded in 19 patients with Meniere's disease exhibiting ispilateral beating nystagmus, and the dynamic aspect of vestibular function during vertiginous periods was examined by determining VOR gain and directional preponderance (DP). The patients sat on a rotation chair and were passively rotated at about 0.3-0.6 Hz. The maximal head velocity ranged from 80 to 120 deg/s and the duration of rotation

Internuclear ophthalmoplegia. II. Pursuit, optokinetic nystagmus, and vestibulo-ocular reflex.

Smooth pursuit, optokinetic nystagmus (OKN) and the vestibulo-ocular reflex (VOR) were studied in four patients with internuclear ophthalmoplegia (two with bilateral and two with unilateral lesions). Horizontal smooth pursuit by an adducting eye on the side of a medial longitudinal fasciculus (MLF) lesion was normal in three of four patients; vertical pursuit was abnormal in all four. The horizontal VOR gain of slow components made by an adducting eye on the side of an MLF lesion was normal in all four patients. The vertical VOR gain was decreased in the two patients tested. It is concluded th

Vestibulo-ocular reflex characteristics in patients with unilateral Ménière's disease.

OBJECTIVE: The pathophysiologic concept of Ménière's disease assumes that endolymphatic hydrops is the cause of the symptoms via increased pressure in the endolymphatic space and/or ionic disorder due to ruptured membrane. The goal of this study was to assess whether the vestibulo-ocular reflex (VOR) properties in patients with Ménière's disease were consistent with the classical theory. MATERIAL AND METHODS: We studied 34 patients (19 women, 15 men) presenting with unilateral Ménière's disease divided into 2 groups according to the duration of the disease: 18 were in the early stage (< 12 mo

Vestibular neuritis: vertigo and the high-acceleration vestibulo-ocular reflex.

Patients after vestibular neuritis (VN) often report persistent dizziness and disequilibrium. We correlated persistent symptoms with sustained impairment of the high-acceleration horizontal vestibulo-ocular reflex as determined by quantitative searchcoil head-impulse testing (qHIT). In 47 patients, qHIT was recorded 0-60 months and symptoms assessed with the Yardley Vertigo Symptom Scale short form > or = 18 months after VN onset. No correlation between the magnitude of high-acceleration vestibular impairment and the severity of vertigo symptoms was observed. The lack of a symptom-qHIT correla

Cortical correlates of vestibulo-ocular reflex modulation: a PET study.

To elucidate cortical correlates of vestibulo-ocular reflex (VOR) modulation, we observed cortical activation during fixation suppression and habituation of caloric vestibular nystagmus in 12 normal subjects, using PET. Significant positive correlation between regional cerebral blood flow (rCBF) and slow phase eye velocity of caloric nystagmus was observed in the middle and posterior insula, inferior parietal lobule, temporal pole, right fusiform gyrus, lingual gyrus, and cerebellar vermis and hemisphere. The rCBF increase in the insular region and the inferior parietal lobule was lateralized

Phase difference of vestibulo-ocular reflex in head autorotation test.

In the head autorotation test (HART) gain is commonly used to determine the vestibulo-ocular reflex (VOR). The purpose of this work was to evaluate the relevance of phase (the phase difference between the eye and corresponding head movement) in describing the VOR. The phase was determined in 5 frequency bands in both degrees and milliseconds (ms) in 4 groups: 30 healthy adults, 15 elderly subjects, 37 vestibular schwannoma (VS) patients, and 35 Meniere's disease (MD) patients. In healthy adults and in MD patients the phase was close to nil in the frequency bands of 3 and 4 Hz. In elderly subje

The relationship between disordered pursuit and vestibulo-ocular reflex suppression.

The performance of the smooth pursuit reflex and the ability to suppress the vestibulo-ocular reflex were assessed in 10 normal subjects and in patients with a variety of diseases of the central nervous system. Pursuit was measured as the maximum velocity of slow phase eye movement in response to a laser target moving sinusoidally at various frequencies up to 1 Hz and with amplitudes stepped up to 35° peak. Suppression of the vestibulo-ocular reflex was assessed with subjects seated in a Barany chair rotating sinusoidally in yaw at matching frequencies. The breakpoint of vestibulo-ocular refle

Adaptations and deficits in the vestibulo-ocular reflex after peripheral ocular motor palsies.

Palsy of a nerve might be expected to lower vestibulo-ocular reflex (VOR) responses in its fields of motion, but effects of peripheral neuromuscular disease were unknown. We recorded the VOR during sinusoidal head rotations in yaw, pitch, and roll at 0.5-2 Hz and static torsional gain in 43 patients with unilateral nerve palsies. Sixth nerve palsy (n = 21) reduced both abduction and adduction VOR gains in darkness. In light, horizontal visually enhanced VOR (VVOR) gains were normal in moderate and mild palsy. In severe palsy, horizontal VVOR gains remained low in the paretic eye when it was fi

The linear vestibulo-ocular reflex, locomotion and falls in neurological disorders.

PURPOSE: During locomotion, head perturbations, consisting of rotations and translations (linear movements), occur with predominant frequencies of 0.5-5.0 Hz. The vestibular reflexes act at short latency to safeguard clear vision and stable posture during locomotion. Much is known about the angular vestibulo-ocular reflex (aVOR) in response to head rotations, which depend on the semicircular canals of the vestibular labyrinth. However, the means to test reliably the linear or translational vestibulo-ocular reflex (tVOR), which depends on the otolithic organs, has only become available more rec

A reinterpretation of the purpose of the translational vestibulo-ocular reflex in human subjects.

In a prior study we reported that the human translational vestibulo-ocular reflex (tVOR) in response to vertical (bob) 2 Hz oscillations generated eye rotations of only 60% of those required to keep the eyes pointed at a stationary visual target, whether located at near (approximately 17 cm) or far (2 m). Best responses occurred in ambient illumination and we concluded that relative image motion between the target and background was an important determinant of tVOR behaviour. To investigate further how visual conditions influenced tVOR, we measured responses as subjects binocularly viewed the

Asymmetry of the pitch vestibulo-ocular reflex in patients with cerebellar disease.

Responses to pitch head impulses were measured in 15 patients with cerebellar degeneration and downbeat nystagmus and in 5 control subjects, using three-axis search coils. For each subject, response gains were calculated as (1) the ratio of instantaneous vertical eye velocity to pitch head velocity (at 30 ms and at 70 ms into the response), and (2) the ratio of peak eye velocity to peak head velocity. Gains varied more widely among patients. When calculated at 70 ms and using peak values, patients had higher gains for downward pitch, and normal subjects had symmetric gains. At 30 ms, gains wer

Transaural linear vestibulo-ocular reflexes from a single utricle.

To clarify the directional sensitivity of a single utricle with respect to the transaural linear vestibulo-ocular reflex (L-VOR) we studied seven patients before and after vestibular neurectomy. Patients were seated upright in an electrically powered car running on a linear track. Transaural acceleration steps of 0.24 g were applied randomly to the left and right in the dark. The slow phase velocity of the L-VOR was measured from the average of the induced compensatory eye movements. L-VOR asymmetry was calculated as (R-L/R + L x 100) and is < 13% in normals. Before surgery, responses were mos

Autorotation test of the horizontal vestibulo-ocular reflex in Menière's disease.

Sixty-four patients with the diagnosis of Menière's disease were tested at the House Ear Clinic with an active head-rotation test system, the Vestibular Autorotation Test (VAT). The VAT is a portable, computerized test that measures the horizontal vestibulo-ocular reflex (VOR) with the use of high-frequency (2 to 6 Hz) active head movements to obtain gain and phase. The purpose of this study was to characterize the horizontal VOR at high frequencies in patients with Menière's disease. At frequencies from 5 to 6 Hz, all patients demonstrated horizontal phase greater than 180 degrees and 85% sho
