Renal transplant artery stenosis.

Renal transplant artery stenosis is a relatively frequent complication after transplantation, with an incidence of up to 23% being reported. The gold standard for the diagnosis still remains renal arteriography. Several imaging techniques are available to confirm the diagnosis (duplex-Doppler, nuclear magnetic resonance, spiral computerized tomography), and their use depends, in part, on the centre's experience. The treatment can either be conservative (providing graft perfusion is not jeopardized) or by revascularization (surgical or percutaneous transluminal angioplasty). There are several u

Traumatic renal artery thrombosis.

We report the case of a unilateral traumatic thrombosis of the renal artery. Revascularization was not performed because the diagnosis was made more than 24 h after the injury. Early diagnosis and prompt surgical repair are the keys to successful management of this complication.

[Spontaneous renal artery dissection].

The clinical clues of sudden arterial hypertension, acute pain in the side, proteinuria, hematuria, abdominal bruits and renal failure are suspicious for spontaneous dissection of the renal arteries, even in the absence of one or more of the above-mentioned symptoms. To confirm the diagnosis and to provide appropriate therapy, immediate renal arteriography is mandatory, otherwise acute loss of renal tissue may occur. We report on one patient with spontaneous dissection of the renal arteries.

Traumatic renal artery thrombosis.

Traumatic bilateral renal artery thrombosis is a rare injury that may be under-diagnosed. The clinical hallmarks include epigastric or flank pain and proteinuria, or hematuria following blunt trauma to the back or abdomen. Traditional practice suggests that revascularization should not be attempted more than 20 hours after injury. A patient is described in whom the retroperitoneum was explored 48 hours after injury and was found to have bilateral renal artery thrombosis. Although no repair was attempted, renal function spontaneously improved over several weeks. The clinical course of this pati

Spontaneous renal artery dissection.

Focal lesions in the renal artery caused by fibromuscular dysplasia (FMD) are susceptible to spontaneous intramural dissection, and carry a serious risk of malignant hypertension and loss of renal function. Of four patients with arterial dissection and severe hypertension, nephrectomy was required in two, an aortorenal bypass was constructed in one, and spontaneous re-entry with resolution of malignant hypertension occurred in one man. All except the patient with re-entry became normotensive. Isolated dissections in the renal artery wall in association with FMD apparently occur more often than

Renal artery stenting for atherosclerotic renal artery stenosis: A review of contemporary data.

Renal artery stenting has been less frequently performed following the results of the Cardiovascular Outcomes in Renal Atherosclerotic Lesions trial. In this trial, stenting demonstrated no benefit over medical therapy in a composite outcome including mortality as well as cardiovascular and renal events. However, these findings must be interpreted in the context of the study population, which included many patients less likely to benefit from intervention due to relatively mild renal artery stenosis, as well as less severe hypertension and renal dysfunction. More recent investigations have foc

Renal artery reconstruction and kidney autotransplantation for Takayasu arteritis-induced renal artery stenosis.

A young woman with Takayasu arteritis and complex renal artery stenosis in a solitary functional kidney underwent an ex vivo revascularization with autologous saphenous vein graft and renal autotransplantation. Before surgery, she had resistant hypertension and recurrent episodes of acute kidney injury. Two years later, her blood pressure is 123/77 mm Hg, and there have been no acute kidney injury episodes. Computed tomography scan demonstrates no abnormal thickening of the graft despite proximal progression of disease to involve the superior mesenteric artery. As Takayasu arteritis is a progr

Percutaneous renal artery stent implantation in the treatment of atherosclerotic renal artery stenosis.

We aimed to investigate the clinical curative effect of percutaneous renal artery stent implantation (PTRAS) in the treatment of atherosclerotic renal artery stenosis (ARAS), and to analyze the factors influencing the curative effect of PTRAS. A total of 230 patients with unilateral or bilateral renal artery stenosis were retrospectively analyzed. According to whether adverse cardiogenic or nephrogenic events occurred, 230 patients were divided into two groups to analyze the risk factors of adverse cardiogenic or nephrogenic events. The blood pressure of patients at each time-point after opera

Renal artery stenting for renal artery stenosis: interventional approach and initial clinical results.

BACKGROUND: Significant renal artery stenosis is being increasingly diagnosed and interventional cardiologists are taking on the unfamiliar role of deploying renal stents. We describe our technique for renal artery stenting using techniques and equipment similar to that used in coronary stenting and present early results. METHODS: Thirty-two patients with significant renal artery stenosis (diameter stenosis 60-95%) and difficult to control hypertension or renal impairment were referred for renal stenting. Renal DTPA scans were performed before intervention. All the procedures were attempted f

[Effectiveness of percutaneous transluminal renal artery stenting in treating atherosclerotic renal artery stenosis].

OBJECTIVE: To investigate the effectiveness of percutaneous transluminal renal artery stenting (PTRAS) in treating atherosclerotic renal artery stenosis (ARAS). METHODS: A total of 69 patients with severe ARAS were treated with PTRAS between January 2002 and December 2008. There were 47 males and 22 females with an average age of 66.2 years (range, 42-88 years), including 66 cases of unilateral ARAS (single functional kidney, 1 case) and 3 cases of bilateral ARAS. Renal angiography revealed that the degree of renal artery stenosis was 70%-99%. Concomitant diseases included hypertension (67 ca

Mycotic renal artery degeneration and systemic sepsis caused by infected renal artery stent.

A case of Staphylococcus aureus renal artery stent infection was studied. Fourteen days after the procedure, the patient had a fever, hypotension, and an elevated white blood cell (WBC) count. Blood cultures were positive for S. aureus on admission and during the patient's hospitalization, despite intravenous vancomycin therapy. Evaluation included serial CT scans, revealing increasing persistent inflammation with development of multiple renal intraparenchymal abscesses, and arteriography, showing marked degeneration of the renal artery. Therapy required resection of the renal artery/stent and

Renal denervation in multiple renal arteries.

BACKGROUND: In most previous studies investigating efficacy of renal denervation (RDN), patients with multiple renal arteries are generally excluded from treatment. This study was designed to determine the prevalence of multiple renal arteries in patients referred for RDN, to propose a classification for anatomical eligibility and to investigate the relation between the presence of multiple arteries and blood pressure (BP)-lowering effect. MATERIALS AND METHODS: Patients referred for RDN who underwent noninvasive imaging of the renal arteries before treatment were included in present analysis

Outcome following renal autotransplantation in renal artery stenosis.

INTRODUCTION: With significant advances in the area of interventional radiology, angioplasty and stenting have become preferred first-line treatment in patients with significant renal artery stenosis. However, not all patients have favorable anatomy to undergo minimally invasive treatments, and reconstruction of the renal artery is an option. In select cases, either improved renal function or maintenance of existing function and sometimes resolution of hypertension can follow surgical treatment. MATERIAL AND METHODS: This was a prospective observational study conducted from August 2010 to Jun

Symptomatic Renal Artery Stenosis and Infra-renal AAA.

OBJECTIVES: To identify evidence to guide the vascular surgeon as to the relevance of renal artery stenting in a patient with symptomatic renal artery stenosis undergoing elective endovascular aortic aneurysm repair (EVAR). METHODS: A comprehensive literature search of MEDLINE was performed without time limits. The following terms were used in the first instance: renal artery stenting and renal artery stenosis, and any other analogous terms identified during the search. Selection criteria were set to randomised control trials. RESULTS: Despite several large, randomised controlled trials inve

Spontaneous renal artery dissection complicating with renal infarction.

Spontaneous renal artery dissection (SRAD) is a rare entity. We reported a 30-year-old healthy man presenting with sudden onset of left flank pain. Abdominal plain film and sonography were unremarkable. The contrast-enhanced abdominal computed tomographic (CT) scan demonstrated a dissecting intimal flap of the left distal renal artery (RA) complicating infarction. Selective angiography of the renal artery disclosed a long dissection of left distal RA with a patent true lumen and occlusion of left accessory RA. Conservative treatment with control of blood pressure and antiplatelet agent was pre

Renal artery stenosis with erythrocytosis after renal transplantation.

We present here results of studies on four patients (three men, one woman) who had had cadaver renal transplants and in whom renal artery stenosis and hypertension developed. Erythropoietin-dependent erythrocytosis developed in association with these changes in the three men. All patients had stable renal function and the hypertension was well controlled. Absolute erythrocytosis thought to be secondary to local renal hypoxia due to decreased renal blood flow developed in two of the men. Erythrocytosis developed in the other man but his RBC mass was at the upper limit of normal. In these patien

Renal infarction resulting from traumatic renal artery dissection.

Renal artery dissection may be caused by iatrogenic injury, trauma, underlying arterial diseases such as fibromuscular disease, atherosclerotic disease, or connective tissue disease. Radiological imaging may be helpful in detecting renal artery pathology, such as renal artery dissection. For patients with acute, isolated renal artery dissection, surgical treatment, endovascular management, or medical treatment have been considered effective measures to preserve renal function. We report a case of renal infarction that came about as a consequence of renal artery dissection.

Segmental renal artery embolization in solitary renal carcinoma.

Segmental renal artery embolization was performed as an alternative method to treat solitary renal carcinoma in 2 patients who were poor surgical candidates. In the first patient, a recurrent renal carcinoma in a solitary kidney was selectively embolized. Tumor aggression was observed eighteen months later. Recanalization of the segmental artery and collateral from the capsular artery required a second embolization. In the second patient with a transitional cell carcinoma involving the lower half of the only functioning kidney, segmental renal artery embolization successfully controlled his pe
