Fluctuations in Subjective Tinnitus Ratings Over Time: Implications for Clinical Research.

OBJECTIVE: Patients with chronic, subjective tinnitus are often administered a battery of audiometric tests to characterize their tinnitus percept. Even a comprehensive battery, if applied just once, cannot capture fluctuations in tinnitus strength or quality over time. Moreover, subjects experience a learning curve when reporting the detailed characteristics of their tinnitus percept, such that a single assessment will reflect a lack of familiarity with test requirements. We addressed these challenges by programming an automated software platform for at-home tinnitus characterization over a 2

The Belmont Report. Ethical principles and guidelines for the protection of human subjects of research.

On July 12, 1974, the National Research Act (Pub. L. 93-348) was signed into law, thereby creating the National Commission for the Protection of Human Subjects of Biomedical and Behavioral Research. One of the charges to the Commission was to identify the basic ethical principles that should underlie the conduct of biomedical and behavioral research involving human subjects and to develop guidelines which should be followed to assure that such research is conducted in accordance with those principles. In carrying out the above, the Commission was directed to consider: (a) the boundaries betwee

International human subject research: taking stock in the wake of the Guatemala affair.

The recent unearthing of a US-funded study wherein unknowing Guatemalans were exposed to syphilis, gonorrhea, or chancroid sent shock waves throughout the scientific community. Prompted by this revelation, the Presidential Commission for the Study of Bioethical Issues has been convened to determine if current "standards adequately guard the health and well-being of participants in scientific studies supported by the Federal Government." This communication revisits the statutory and regulatory framework undergirding federally sponsored human subject research and explores its continued relevance

[Results and observations on the research of circulating immunocomplexes in subjects with suspected autoimmune pathology].

The authors have investigated the level of circulating immune complexes (CIC) in 409 patients (some hospitalized) suffering from disease in which the presence of CIC had been suspected. The 10% (forty-one) was positive, and clinical manifestations related have shown, in the majority of patients, a close relation with diseases in which there was, or it was possible to presume, an immunological pathogenic mechanism. Besides the authors examine a case with close association between high level of CIC and leptospirosis.

From Youth Advisory Boards to subject matter research consultants: transforming youth participation in research advising and practice.

BACKGROUND: Youth Advisory Boards and Community Advisory Boards are widely used to promote public involvement in research involving adolescents and young adults. These structures can improve the relevance, acceptability, and ethical grounding of research; however, advisory models often remain constrained by structural limitations, including inconsistent or inadequate compensation models, limited decision-making authority, grant-dependent instability, and late-stage engagement. These constraints may unintentionally position youth as episodic consultants rather than sustained research partners.

Gender differences in outpatient research subjects with affective disorders: a comparison of descriptive variables.

BACKGROUND: Gender may play an important role in the etiopathophysiology of psychiatric illness and has become a subject of increasing interest because of its possible effects on biological markers, treatment outcome, and prognosis. Intrigued by this issue and as part of our attempt to further characterize research subjects in San Diego, we evaluated male and female research subjects from our affective disorders clinical research center on a variety of measures. Based on epidemiologic data, we postulated that female and male subjects would be similar to epidemiologic samples and would differ i

Trends and hotspots in subjective idiopathic tinnitus and anxiety disorder research based on Bibliometrics.

OBJECTIVE: Subjective idiopathic tinnitus is characterized by the subjective perception of sound in the absence of an external source, significantly impairing quality of life. Studies have found that a considerable number of tinnitus patients experience comorbid negative emotional states such as anxiety disorders, while emotional dysregulation can, in turn, exacerbate the perceived tinnitus, indicating a close interrelationship. A systematic bibliometric summary in this field is currently lacking. This study aims to employ bibliometric methods to reveal the research dynamics, collaborative net

Objective measures of subjective experience: the use of therapist notes in process-outcome research.

Computerized linguistic measures of emotional engagement, and reflective and affective language, previously applied to session transcripts, were applied to notes of 14 treatments by candidates under supervision at the New York Psychoanalytic Society and Institute, covering the five decades from the 1950s to the 1990s. The findings indicate a strong relationship between candidates' subjective experience as represented unintentionally in the linguistic style of their case notes and the effectiveness of their clinical work. The treatments were evaluated for effectiveness by experienced clinicians

[Subjective memory loss--a sign of cognitive impairment in the elderly? An overview of the status of research].

According to the growing clinical interest in early indicators of dementia, numerous studies have examined the association between subjective memory complaints and cognitive performance in old age. Their results are contradictory. In this paper, studies carried out over the last 10 years are compared with regard to the study design and the assessment instruments used. The results are discussed with particular reference to the diagnostic validity of subjective memory complaints. The majority of case-control studies and cross-sectional studies of non-representative samples could not demonstrate

Views of potential subjects toward proposed regulations for clinical research with adults unable to consent.

OBJECTIVE: The authors' goal was to assess healthy individuals' attitudes toward five of the most prominent proposed safeguards regarding the consent process for research with adults unable to consent. METHOD: Telephone interviews were conducted with 246 individuals with a family history of Alzheimer's disease who had participated in clinical research. RESULTS: The majority of respondents said that they were willing to participate in research if they lost the ability to consent. Few completed a research advance directive. Many had discussed their preferences with their families, and the majo

From protection to entitlement: selecting research subjects for early phase clinical trials involving breakthrough therapies.

Our goals are to (1) set forth and defend a multiprinciple system for selecting individuals who meet trial eligibility criteria to participate in early phase clinical trials testing chimeric antigen receptor (CAR T-cell) for acute lymphoblastic leukaemia when demand for participation exceeds spaces available in a trial; (2) show the relevance of these selection criteria to other breakthrough experimental therapies; (3) argue that distinct distributive justice criteria apply to breakthrough experimental therapies, standard research and healthcare and (4) argue that as evidence of benefit increa

Predictors of Clinical Progression of Subjective Memory Impairment in Elderly Subjects: Data from the Clinical Research Centers for Dementia of South Korea (CREDOS).

BACKGROUND/AIMS: The aims of this study were to determine baseline factors related to the progression of subjective memory impairment (SMI) in elderly subjects and to develop a new modeling scale to predict progression. METHODS: Elderly subjects with SMI were recruited from the nationwide Clinical Research Centers for Dementia of South Korea (CREDOS) multicenter cohort and divided into two groups: (1) progressed to mild cognitive impairment or Alzheimer's disease or (2) stable without progression. Baseline clinical characteristics were compared between the groups, and the most relevant predic

Vagus nerve stimulation in Parkinson's disease: a scoping review of animal studies and human subjects research.

Parkinson's Disease (PD) is a prevalent, progressive neurodegenerative disease with motor and non-motor symptoms. Vagus Nerve Stimulation (VNS) has emerged as a potential therapeutic approach for PD, but published research on this topic varies widely. This scoping review maps existing literature on VNS for PD, highlighting stimulation methods, operational parameters, safety profiles, neurophysiological mechanisms, and clinical outcomes in human and animal models. Online databases were used to identify 788 papers published between 2013 and 2023, from which 17 publications on invasive and non-in

Subject eligibility criteria can substantially influence the results of alcohol-treatment outcome research.

OBJECTIVE: Most alcohol-treatment studies exclude some patients from participation based on particular criteria (e.g., comorbid illegal drug abuse, homelessness). The current study evaluated whether such eligibility criteria can change the outcome results a study obtains. METHOD: Five widely used treatment research eligibility criteria--(1) psychiatric problems, (2) medical problems, (3) social-residential instability, (4) low motivation/noncompliance, and (5) drug problems--were applied to two samples of real-world alcohol patients whose outcomes were known. Comparing outcomes of the samples

The subjective effects of nicotine: methodological issues, a review of experimental studies, and recommendations for future research.

This paper reviews findings from placebo-controlled human experimental studies of the effects of nicotine on subjective experience. Studies are grouped according to whether participants were smokers (significantly nicotine deprived, minimally nicotine deprived) or non-smokers. Within each category, studies are also grouped according to method of nicotine administration (e.g., smoked tobacco, nasal spray) and nicotine dose. This review of studies is preceded by a discussion of several methodological issues in studies of nicotine and mood. The principal findings of this review are: (1) there is

Effects of researcher follow-up of distressed subjects: tradeoff between validity and ethical responsibility.

Researchers studying depression often encounter research participants in serious preexisting distress. Examining investigators' ethical responsibilities to these subjects, Stanton and New (1988) found that depression researchers reported actions that ranged from doing nothing to contacting both the distressed subject and a significant other. By experimentally manipulating consent form information regarding potential treatment referral, we examined whether subjects (n=357) adjusted their responses on depression measures as a function of the level of follow-up they expected to receive. Results r
