COVID-19 PICU guidelines: for high- and limited-resource settings.

BACKGROUND: Fewer children than adults have been affected by the COVID-19 pandemic, and the clinical manifestations are distinct from those of adults. Some children particularly those with acute or chronic co-morbidities are likely to develop critical illness. Recently, a multisystem inflammatory syndrome (MIS-C) has been described in children with some of these patients requiring care in the pediatric ICU. METHODS: An international collaboration was formed to review the available evidence and develop evidence-based guidelines for the care of critically ill children with SARS-CoV-2 infection.

Point-of-Care Ultrasound in Resource-Limited Settings: Common Applications.

Point-of-care ultrasound (POCUS) is used increasingly in resource-limited settings (RLSs), particularly as inexpensive ultrasound machines and evidence-based protocols become more available. POCUS often is the only imaging modality available in such settings, and it has the potential to significantly affect patient care. This article discusses four case-based reviews of POCUS in RLSs for several common diseases: pediatric pneumonia; extrapulmonary tuberculosis; ectopic pregnancy; and tropical parasitic diseases, including echinococcus, schistosomiasis, and amebiasis. It concludes with a discus

A Diagnostic Scoring Model for Leptospirosis in Resource Limited Settings.

BACKGROUND: Leptospirosis is a zoonotic infection with significant morbidity and mortality. The clinical presentation of leptospirosis is known to mimic the clinical profile of other prevalent tropical fevers. Laboratory confirmation of leptospirosis is based on the reference standard microscopic agglutination test (MAT), direct demonstration of the organism, and isolation by culture and DNA detection by polymerase chain reaction (PCR) amplification. However these methods of confirmation are not widely available in resource limited settings where the infection is prevalent, and reliance is pla

Strategies to optimize HIV treatment outcomes in resource-limited settings.

Although the availability of antiretroviral therapy has increased rapidly to reach over three million people in low- and middle-income countries, coverage remains low as only 31% of people in need were receiving antiretroviral therapy in sub-Saharan Africa. Antiretroviral therapy scale-up needs to continue to grow exponentially to meet the need for universal access and keep pace with or exceed the new HIV infections. This calls for strategies that will have the greatest impact on the reduction of opportunistic infections, toxicities, and early mortality after antiretroviral therapy initiation

Beyond Atrial Fibrillation: Prioritizing Stroke Evaluation in Resource-Limited Settings.

Atrial fibrillation (AF) accounts for the majority of cardioembolic strokes. In addition to AF screening, echocardiography is recommended to evaluate structural heart diseases including congenital anomalies. In resource-limited settings, in-hospital echocardiography for all stroke patients may not be feasible. However, careful assessment of readily available clinical parameters, such as bedside examination, electrocardiography, and chest X-ray, can offer essential clues to guide prioritization of echocardiography in selected patients and facilitate timely management. We present a case of recur

Parental manual ventilation in resource-limited settings: an ethical controversy.

Lower respiratory tract infections are a leading cause of paediatric morbidity and mortality worldwide. Children in low-income countries are disproportionately affected. This is in large part due to limitations in healthcare resources and medical technologies. Mechanical ventilation can be a life-saving therapy for many children with acute respiratory failure. The scarcity of functioning ventilators in low-income countries results in countless preventable deaths. Some hospitals have attempted to adapt to this scarcity by using hand-bag ventilation, as either a bridge to a mechanical ventilator

Stakeholders' engagement with Ebola therapy research in resource limited settings.

BACKGROUND: The current Ebola Virus Disease (EVD) outbreak in West Africa is the largest in history. As of February 18(th) 2015, 23,258 cases of EVD have been cumulatively reported from Nigeria, Senegal, Guinea, Liberia, Mali, Sierra Leone, Spain, the United Kingdom and the United States of America resulting in more than 9,000 deaths. It is therefore exigent to develop prevention and treatment therapies for EVD. DISCUSSION: Several new EVD treatments are in clinical development at this time. Based on lessons learned, four critical processes need to be implemented before clinical trials begin.

Flow-through, viral co-infection assay for resource-limited settings.

Here we present a new and rapid immunofiltration assay for simultaneous detection of HIV p24 and hepatitis B virus antigens. The assay platform is composed of a 13 mm nitrocellulose filter spotted with capturing bioprobes and inserted in a Swinnex(®) syringe filter holder. Samples and reagents are flown through the nitrocellulose filter by manual pressure on the syringe. A colorimetric detection allows for naked eye results interpretation. The assay provides sensitivity in the picomolar range in just 5 min, even using low volumes of sample in complex matrix. Probe deposition by spotting allows

Cost-effective diagnostic checklists for meningitis in resource-limited settings.

BACKGROUND: Checklists can standardize patient care, reduce errors, and improve health outcomes. For meningitis in resource-limited settings, with high patient loads and limited financial resources, central nervous system diagnostic algorithms may be useful to guide diagnosis and treatment. However, the cost effectiveness of such algorithms is unknown. METHODS: We used decision analysis methodology to evaluate the costs, diagnostic yield, and cost effectiveness of diagnostic strategies for adults with suspected meningitis in resource-limited settings with moderate/high HIV prevalence. We cons

Advances in Patient Monitoring Systems for Prehospital and Resource-Limited Settings.

INTRODUCTION: Vital sign monitoring is essential to the management of critically ill and injured patients. Recent advances in patient monitoring systems have the potential to improve outcomes by providing real-time data and predictive insights, which are particularly valuable in prehospital and resource-limited settings. We conducted a systematic review of the literature to assess the capabilities, performance, and clinical impact of patient monitoring technologies designed for these environments. METHODS: In accordance with Preferred Reporting Items for Systematic Reviews and Meta-Analyses g

Metronomic chemotherapy in resource-limited settings: evidence, opportunities, and future directions.

Metronomic chemotherapy-frequent, continuous delivery of low-dose chemotherapeutic drugs without drug-free breaks-is a promising treatment option for resource-limited situations. This narrative review analyzes the scientific justification, clinical evidence, cost-effectiveness, implementation challenges, and prospective developments of metronomic chemotherapy in low- and middle-income countries (LMICs), which account for 80% of childhood cancers and a rising incidence of adult malignancies. Metronomic regimens provide accessible and cost-effective therapy alternatives by targeting tumor vascul

Challenges in managing pediatric heart failure, especially in resource-limited settings.

Pediatric heart failure (HF) is a complex syndrome with high morbidity, characterized by distinct epidemiological profiles globally. While congenital heart disease and cardiomyopathies predominate in developed regions, rheumatic heart disease and delayed surgical interventions drive the burden in low- and middle-income countries. This review elucidates the unique pathophysiology of pediatric HF, which differs significantly from that of adult HF in terms of developmental cardiac physiology, molecular signaling, and maladaptive ventricular remodeling. Diagnosis remains challenging due to nonspec

Reinforcement learning to optimize tuberculosis screening strategies in resource-limited settings.

Still, tuberculosis (TB) is a big health problem all over the world. It is very hard to find cases quickly in places that don't have many resources or fair access to care. Screening methods from the past, such as x-rays, symptom-based assessments, and molecular diagnostics, have limits on how much they cost, how many people they can help, and how well they work. This means we need new ideas on how to use our tools better and find cases more quickly. A strong way to improve how TB is screened is to use reinforcement learning (RL), which can show how to make choices in a certain order when there

Management of neonates with respiratory distress syndrome in resource-limited settings.

In South Africa, prematurity stands as one of the foremost causes of neonatal mortality. A significant proportion of these deaths occur because of respiratory distress syndrome of prematurity. The implementation of non-invasive respiratory support, such as continuous positive airway pressure (CPAP), has demonstrated both safety and efficacy in reducing mortality rates and decreasing the need for mechanical ventilation. Given the absence of blood gas analysers and limited radiological services in many district hospitals, the severity of respiratory distress is often assessed through observation

Hypoxic-ischaemic encephalopathy code: A systematic review for resource-limited settings.

It is estimated that 96% of infants with hypoxic-ischaemic encephalopathy (HIE) are born in resource-limited settings with no capacity to provide the standard of care that has been established for nearly 15 years in high-resource countries, which includes therapeutic hypothermia (TH), continuous electroencephalographic monitoring and magnetic resonance imaging (MRI) in addition to close vital signs and haemodynamic monitoring. This situation does not seem to be changing; however, even with these limitations, currently available knowledge can help improve the care of HIE patients in resource-li

Smartphone for retinal imaging - Case series in resource-limited rural settings.

PURPOSE: There is a clinical need for a cost-effective, reliable, easy-to-use, and portable retinal photography. The use of smartphone fundus photography for documentation of retinal changes in resource-limited settings, where retinal imaging was not previously possible, is studied here. The introduction of smartphone-based retinal imaging has resulted in the increase in available technologies for fundus photography. On account of the cost, fundus cameras are not readily available in ophthalmic practice in developing countries. Because smartphones are readily available, easy to use, and also p
