Rapid-onset respiratory failure following diesel aspiration at 4,200 m: a case report.

BACKGROUND: Oral siphoning of diesel fuel carries substantial risks of chemical inhalation injury; however, documented cases of diesel aspiration pneumonia, especially at altitudes >2,500 m, remain rare. The synergistic effects of hydrocarbon toxicity and hypobaric hypoxia in such environments are inadequately characterized. CASE PRESENTATION: A 23-year-old man accidentally aspirated approximately 10 mL of diesel while siphoning a fuel line at 4,200 m altitude. Within 5 h, he developed nausea, vomiting, and dry cough, progressing to right-sided pleuritic chest pain, fever (38.2 °C), and hypox

Aspiration pneumonia induces muscle atrophy in the respiratory, skeletal, and swallowing systems.

BACKGROUND: Repetition of the onset of aspiration pneumonia in aged patients is common and causes chronic inflammation. The inflammation induces proinflammatory cytokine production and atrophy in the muscles. The proinflammatory cytokines induce muscle proteolysis by activating calpains and caspase-3, followed by further degradation by the ubiquitin-proteasome system. Autophagy is another pathway of muscle atrophy. However, little is known about the relationship between aspiration pneumonia and muscle. For swallowing muscles, it is not clear whether they produce cytokines. The main objective o

[Clinical study of respiratory infection due to Branhamella catarrhalis using transtracheal aspiration].

We clinically investigated 40 cases of respiratory infection due to B. catarrhalis by transtracheal aspiration (TTA). The cases consisted of acute bronchitis (13 cases), pneumonia (14 cases), and chronic lower respiratory tract infection (13 cases). The infection was monomicrobial in 17 cases and polymicrobial in 23 cases with respect to the organisms isolated from TTA. The principal organisms isolated in combination with B. catarrhalis in cases of polymicrobial infection were S. pneumoniae and H. influenzae. All of the 6 cases of death were patients with pneumonia who had malnutrition and imm

Early pneumococcal sepsis after pulmonary aspiration and the adult respiratory distress syndrome.

Bacterial pneumonia often complicates recovery several days after aspiration of gastric and oropharyngeal contents. Two children aspirated vomitus resulting in adult respiratory distress syndrome (ARDS) and suffered life-threatening pneumonia and septicemia caused by Streptococcus pneumoniae on the 2nd day after aspiration. We presume that inoculation of oropharyngeal bacteria into severely injured lung resulted in unusually rapid progression of infection. A high index of suspicion is necessary to detect this early complication because clinical signs of infection were obscured by ARDS.

The Respiratory-Swallow Coordination may be Related to Aspiration in Infratentorial Stroke Patients.

To investigate the characteristics of respiratory-swallow coordination (RSC) in poststroke dysphagia (PSD) patients following infratentorial stroke (IS) and further explore their association with aspiration. PSD patients after IS and age-matched healthy controls were recruited. Nasal airflow and sound signals were recorded using a nasal cannula-type flow sensor and contact microphone, which were synchronized with videofluoroscopic swallowing studies (VFSS). For healthy controls, only nasal airflow and sound signals recordings were conducted. A 5 ml thickened liquid was utilized during these as

Lamellar body counts on gastric aspirates for prediction of respiratory distress syndrome.

AIM: To develop a rapid method for diagnosing lung maturity at birth with the purpose of administering surfactant early to infants with immature lungs and to spare infants with mature lungs from this treatment. METHODS: Lamellar body counts (LBC) on gastric aspirates from 191 newborns were counted in the platelet window in automatic blood cell counters. A preliminary study was performed on 108 aspirates from 2000 in infants with <32 weeks' gestation. Furthermore, 83 aspirates from 2004 to 2005 in infants with <30 weeks' gestation were analysed. RESULTS: Lamellar bodies in gastric aspirate we

Failure of tracheal aspirate cultures to define the cause of respiratory deteriorations in neonates.

The spectrum of organisms responsible for lower respiratory tract infection in chronically ventilated neonates is poorly defined. During an 18-month period 63 infants with a respiratory deterioration defined as an increase in fractional inspired O2 concentration > or = 20% and/or mean airway pressure > or = 3 cm H2O were evaluated for pulmonary infection. These infants were compared with 58 stable control ventilated infants. Tracheal aspirates for culture and Gram stain were taken from both groups and were cultured for bacteria, viruses, Chlamydia trachomatis, Ureaplasma urealyticum and Mycopl

Phosphatidylcholine composition of endotracheal tube aspirates of neonates and subsequent respiratory disease.

The phosphatidylcholine (PC) content of the initial endotracheal tube aspirate was measured in 105 infants intubated for resuscitation or for ventilation for respiratory distress syndrome, using high performance liquid chromatography and postcolumn fluorescence derivitization with diphenyl-1,3,5-hexatriene. Sixty eight had measurable PC. Of the infants who developed respiratory distress syndrome, with or without subsequent chronic lung disease, neither the percentage of dipalmitoylphosphatidylcholine (DPPC) nor the ratio of DPPC to palmitoyloleoylphosphatidylcholine (POPC), showed any correlat

Torque Teno Virus in Nasopharyngeal Aspirate of Children With Viral Respiratory Infections.

BACKGROUND: Torque teno virus (TTV) is a ubiquitous anellovirus responsible for persistent infections and is considered a marker of immune function. The role of TTV as a facilitator of respiratory infections (RIs) is unknown. OBJECTIVES: Our aim was to estimate, in a prospective study, the prevalence of TTV in the nasopharyngeal aspirate (NPA) of hospitalized children <5 years old, with RIs and correlate them with outcomes and immune response. PATIENTS AND METHODS: NPA was taken for testing of 16 respiratory viruses by reverse transcription-polymerase chain reaction (PCR), TTV PCR, and immun

Correction to: Abnormalities of Aspiration and Swallowing Function in Survivors of Acute Respiratory Failure.

The mechanisms responsible for aspiration are relatively unknown in patients recovering from acute respiratory failure (ARF) who required mechanical ventilation. Though many conditions may contribute to swallowing dysfunction, alterations in laryngeal structure and swallowing function likely play a role in the development of aspiration. At four university-based tertiary medical centers, we conducted a prospective cohort study of ARF patients who required intensive care and mechanical ventilation for at least 48 h. Within 72 h after extubation, a Flexible Endoscopic Evaluation of Swallowing (FE

Stable microbubble test on tracheal aspirate for the diagnosis of respiratory distress syndrome.

BACKGROUND: Exogenous surfactant should be used as early as possible in the presence of respiratory distress syndrome (RDS), but diagnosis may only become clear late in the course of the disease. The stable microbubble test (SMT) in the tracheal aspirates could help in the decision to give early surfactant to preterm babies with respiratory distress. OBJECTIVES: The objective of this study was to evaluate the accuracy of the SMT on tracheal aspirate for the diagnosis of RDS in newborns requiring mechanical ventilation. METHODS: The test was performed on specimens obtained from 74 infants req
