[Function of the respiratory muscles in acute cardiac decompensation].

In anesthetized dogs the performance of the respiratory muscles during cardiogenic shock and their influence on its outcome was studied. Shock was induced by cardiac tamponade. Cardiac output during shock amounted to 25-35% of control and was kept constant. Minute ventilation (VE) initially increased and then progressively decreased until respiratory arrest. The latter was due to impairment of respiratory muscle contractility, the pressure generated by the diaphragm (Pdi) decreasing despite a marked increase in the diaphragmatic electromyogram and of the phrenogram. Mechanical ventilation duri

Nonhomogeneous alveolar pressure swings: effect of different respiratory muscles.

We tested the hypothesis in dogs that during isovolumic inspiratory efforts the degree of nonhomogeneity of alveolar pressure changes is influenced by regional differences in respiratory muscle activity. Pressure distal to an occluded right lower lobe bronchus (PRLL) was compared with pressure in the occluded airway opening (PaO) during spontaneous inspiratory efforts (S), during right (RP), left (LP), and bilateral (BP) phrenic nerve stimulation, and during spontaneous inspiratory efforts after cutting both phrenic nerves (SCP), Upper and lower rib cage and abdominal dimensional changes were

Respiratory Muscle Testing.

Disorders of the respiratory muscles (RM) are common, yet are often overlooked. Physical examination findings may suggest the presence of RM weakness but are nonspecific. Several tests of RM function are available, including measurements of upright and supine vital capacity, inspiratory and expiratory muscle pressures, sniff nasal inspiratory pressure, sniff esophageal and transdiaphragmatic pressures, and peak cough flow. These tests are widely available but are effort dependent. Electrodiagnostic studies, such as electromyography, electrical, or magnetic phrenic nerve stimulation, are effort

Respiratory muscle failure.

The diseases which are commonly complicated by hypercapnic respiratory failure also compromise the respiratory muscles in several ways. Increased work of breathing, mechanical disadvantage, neuromuscular disease, impaired nutritional status, shock, hypoxemia, acidosis, and deficiency of potassium, magnesium, and inorganic phosphorus are the major non-neurologic factors which contribute to respiratory muscle fatigue and failure. Respiratory muscle fatigue has two components. High frequency fatigue occurs rapidly with intense contractile efforts but is usually not severe. It also recovers rapidl

Respiratory muscle rest.

The respiratory muscles have great reserves, and under normal circumstances, in a fit person, it is probably impossible to fatigue them by activity. However, respiratory muscle fatigue can be induced experimentally, and with the stress of pulmonary disability, or with impairment of muscle function due to neuromuscular disease or skeletal deformity. Respiratory muscle fatigue contributes to respiratory failure and clinical deterioration. Resting the respiratory muscles can allow time for recovery but requires complete or partial artificial ventilation. Whilst positive pressure ventilation has b

Increased load on the respiratory muscles in obstructive sleep apnea.

We wished to quantify, in patients with obstructive sleep apnoea (OSA), the activity of the respiratory muscles in relation to upper airway occlusion and patency in sleep. We hypothesized that particular levels of neuromuscular activation are directly associated with upper airway patency. 21 patients with previously diagnosed OSA and 21 healthy control subjects underwent respiratory muscle testing and polysomnography. Neural respiratory drive, as measured by the electromyogram of the diaphragm (EMG(di)) was elevated in the obese OSA patients, awake and supine (13.1(5.6)%max), compared to norma

A Case of Progressive Ossifying Fibrodysplasia of Tracheobronchial Respiratory Muscles.

The authors report a case of progressive ossifying myositis (POM) in a 13-year-old boy, revealed by dry cough and dyspnea. Conventional chest x-rays and whole-body CT showed extraskeletal ossification that seems to affect the left bronchial strain and trachea. This lesional topography, if established, not yet described to our knowledge, contrasts with the observations of all the authors, including Munchmeyer, for whom smooth muscles and muscles attached to the skeleton by a single end are spared by the heterotopic ossifications characteristic of the disease. Therefore, this observation raises

[Inflammatory myopathy with initial respiratory muscles involvement and rheumatoid arthritis].

Inflammatory myopathies comprise a heterogeneous group of subacute, chronic and sometimes acute acquired muscle diseases. The most common inflammatory myopathies seen in practice can be separated into four distinct subsets: polymyositis, dermatomyositis, necrotizing autoimmune myositis and inclusion body myositis. These disorders present as proximal and symmetric muscle weakness but rarely respiratory muscles may also be affected. We report the case of a 39 year-old female with inflammatory myopathy with acute respiratory failure due to alveolar hypoventilation secondary to respiratory muscle

Corticospinal control of respiratory muscles in chronic obstructive pulmonary disease.

Patients with chronic obstructive pulmonary disease (COPD) face an increased respiratory load and in consequence have an elevated respiratory drive. We used transcranial magnetic stimulation (TMS) to investigate associated changes in corticospinal excitability both at rest and during voluntary facilitation at different levels of inspiratory effort. Diaphragm and abdominal motor thresholds were significantly lower in COPD than healthy controls, but the quadriceps response was the same. In patients there was a significant increase in diaphragm response from rest during 20% inspiratory efforts bu

Effects of surgery on the function of the respiratory muscles.

The function of the respiratory muscles (RM) is affected positively or negatively by a variety of surgical procedures. Cardiac, thoracic and upper abdominal surgery impair the RM function and lead to postoperative complications such as hypoxia, atelectasis, aspiration and infections. Preoperative assessment of RM function is cardinal to avoid or attenuate these complications. Three types of surgical procedures, lung transplantation, lung volume reduction surgery and surgery for obesity have been shown to improve RM function. A mechanism by which these types of operation have shown beneficial e

Glutathione metabolic responses to loaded breathing: variation among respiratory muscles.

Several studies have shown that loaded breathing elicits an oxidation of reduced glutathione (GSH) to oxidized glutathione (GSSG) within the diaphragm, but the effects of loaded breathing on GSH and GSSG levels in other respiratory muscles have not been examined. The present experiment examined this issue by using decerebrate unanesthetized rats in which a large inspiratory resistive load was applied until respiratory arrest. Subsequently, muscle samples from the triangularis sterni, diaphragm (Dia), parasternal intercostal (PI), upper rib cage lateral intercostal, lower rib cage lateral inter

Role of the respiratory muscles in acute respiratory failure of COPD: lessons from weaning failure.

Diabetes mellitus (DM) is associated with characteristic structural and functional changes of the myocardium, termed diabetic cardiomyopathy. As a distinct entity independent of coronary atherosclerosis, diabetic cardiomyopathy is an increasingly recognized cause of heart failure. A detailed understanding of diabetic cardiac dysfunction, using relevant animal models, is required for the effective prevention and treatment of cardiovascular complications in diabetic patients. We investigated and compared cardiac performance in rat models of type 1 DM (streptozotocin induced) and type 2 DM (Zucke

Immediate effect of respiratory muscle stretch gymnastics and diaphragmatic breathing on respiratory pattern. Respiratory Muscle Conditioning Group.

We investigated the immediate effect of respiratory muscle stretch gymnastics (RMSG) and diaphragmatic breathing (DB) on the respiratory pattern in patients with chronic obstructive pulmonary disease (COPD). Sixteen patients with COPD (age, 71.3 +/- 3.9 year) were entered in the study. DB was performed for 10 minutes in supine position. For RMSG, 5 patterns were repeated 10 times each. Respiratory variables were measured in sitting position before and after both RMSG and DB, performed in random order, patient to patient, with a 20-minute washout period in-between. After RMSG, but not after DB,

Respiratory muscle training improves aerobic capacity and respiratory muscle strength in youth wrestlers.

BACKGROUND: Respiratory muscle training (RMT) has shown potential for enhancing athletic performance, but its effectiveness, in youth wrestlers, remains unclear. This study aimed to investigate the effects of RMT on respiratory muscle strength and aerobic endurance in youth wrestlers. METHODS: A parallel-grouperal was conducted across 22 male youth wrestlers aged 14.8 ± 0.4 years. Participants were assigned to an experimental(E) group (n = 11), which received RMT in addition to their regular wrestling training, or a control(C) group (n = 11), which continued with standard wrestling training o

Microstimulators and intramuscular hook electrodes for the stimulation of respiratory muscles.

BACKGROUND/OBJECTIVES: We determined the feasibility of stimulating the major muscles of respiration with different types of electrodes. Intramuscular hook electrodes, model microstimulators (M-Micro) developed in our laboratory, and commercial radiofrequency microstimulators (RFM) (Alfred Mann Foundation, Valencia, CA), were employed in this investigation. METHODS: In 8 anesthetized dogs, M-Micro were placed bilaterally on the diaphragm and in the abdominal muscles, and hook electrodes were placed in the 3rd and 5th intercostal regions adjacent to the intercostal nerves known to support insp
