Diagnosis and management of patients with pleural effusions.

Pleural effusions occur when fluid accumulates between the visceral and parietal pleura in the chest cavity, preventing the lung from expanding fully during inspiration. The reduction in lung volume, depression of the diaphragm and reduced chest wall compliance cause dyspnoea, and occasionally pain or cough. Pleural effusion is a complex problem caused by a variety of conditions requiring different treatment depending on the underlying diagnosis. This article discusses the causes and treatment of pleural effusions, referencing guidelines produced by the British Thoracic Society.

Rabbit respiratory system: clinical anatomy, physiology and disease.

Rabbits are obligate nose breathers due to their epiglottis positioned rostrally to the soft palate. Any obstruction within the nasal cavity will produce a respiratory wheeze with increased respiratory effort. Respiratory diseases are a major cause of morbidity and mortality in rabbits. This article focuses on these diseases and their causative pathogens.

Introducing breathlessness as a significant animal welfare issue.

Breathlessness is a negative affective experience relating to respiration, the animal welfare significance of which has largely been underestimated in the veterinary and animal welfare sciences. In this review, we draw attention to the negative impact that breathlessness can have on the welfare of individual animals and to the wide range of situations in which mammals may experience breathlessness. At least three qualitatively distinct sensations of breathlessness are recognised in human medicine--respiratory effort, air hunger and chest tightness--and each of these reflects comparison by cere

[Problems of regulation of breathing].

Knowledge about the regulation of breathing has pronounced clinical consequences. In the review the concepts of control and regulation of breathing are defined. Regulation based on the chemical feed back is well documented. Apart from that, attention is paid to unproven, hypothetical regulations. Among them, the regulation of the breathing pattern and that of functional residual lung capacity are of especial interest. Not less important is the cortical or volitional regulation of breathing, which is so far almost unexplored. An important problem represents abnormalities of the regulation of br

The psychobiology of strained breathing and its cardiovascular implications: a functional system review.

Strained breathing is a natural respiratory pattern, with cardiovascular implications. It is associated with social factors, attention, expectation, and anxiety and with defense behavior in animals. An inhibition of active behavior is characteristic. Strained breathing is based on the functional heterogeneity of the medullary postinspiratory neurons. In stressful circumstances, muscle tension and laryngeal reflexes induce a strong reduction of airflow in the glottis, resulting in a prolonged Stage I of expiration and an elevated intrathoracic pressure. The resulting elevations of blood pressur

Why are infants prone to wheeze? Physiological aspects of wheezing disorders in infants.

Wheezing in infants is common and increasing in prevalence. Infants are particularly prone to wheezing due to developmental differences in airway mechanics compared to adults. These effects are enhanced in the presence of airway inflammation. Wheezing in infants is related to flow limitation which is a function of airway calibre and airway wall compliance. This review discusses the factors contributing to flow limitation and hence, wheezing. It tries to make the link between risk factors influencing airway structure, and thus function, with particular emphasis on the special physiological pecu

New perspectives concerning feedback influences on cardiorespiratory control during rhythmic exercise and on exercise performance.

The cardioaccelerator and ventilatory responses to rhythmic exercise in the human are commonly viewed as being mediated predominantly via feedforward 'central command' mechanisms, with contributions from locomotor muscle afferents to the sympathetically mediated pressor response. We have assessed the relative contributions of three types of feedback afferents on the cardiorespiratory response to voluntary, rhythmic exercise by inhibiting their normal 'tonic' activity in healthy animals and humans and in chronic heart failure. Transient inhibition of the carotid chemoreceptors during moderate i

The physiological challenges of the 1952 Copenhagen poliomyelitis epidemic and a renaissance in clinical respiratory physiology.

The 1952 Copenhagen poliomyelitis epidemic provided extraordinary challenges in applied physiology. Over 300 patients developed respiratory paralysis within a few weeks, and the ventilator facilities at the infectious disease hospital were completely overwhelmed. The heroic solution was to call upon 200 medical students to provide round-the-clock manual ventilation using a rubber bag attached to a tracheostomy tube. Some patients were ventilated in this way for several weeks. A second challenge was to understand the gas exchange and acid-base status of these patients. At the onset of the epide

Available ventilation monitoring methods during pre-hospital cardiopulmonary resuscitation.

High quality cardiopulmonary resuscitation (CPR) in the pre-hospital setting has been associated with improved survival rates during cardiopulmonary arrest (CPA). Recent documentation of hyperventilation associated deterioration in hemodynamics during CPR, suggests that guided or controlled ventilation strategies may contribute to improved hemodynamics and increased survival. This article briefly reviews the mechanical methods, advantages, and disadvantages of the available ventilation monitoring methods currently available for clinical use, with an emphasis on pre-hospital implementation. Variations in the cerebrospinal fluid dynamics of the American alligator (Alligator mississippiensis).

BACKGROUND: Studies of mammalian CSF dynamics have been focused on three things: paravascular flow, pressure and pulsatility, and "bulk" flow; and three (respective) potential motive forces have been identified: vasomotor, cardiac, and ventilatory. There are unresolved questions in each area, and few links between the different areas. The American alligator (Alligator mississippiensis) has pronounced plasticity in its ventilatory and cardiovascular systems. This study was designed to test the hypothesis that the greater cardiovascular and ventilatory plasticity of A. mississippiensis would res

Anatomical basis of hypoxic and hyperoxic injuries to the centres of cardiorespiratory regulation.

The aim of the present paper is to briefly review the changes occurring in the nucleus tractus solitarii and carotid body in response to hypoxic and hyperoxic injuries. Selective alterations of dendrites and Fos-immunoreactivity of neurons have been observed in the subnucleus gelatinosus of the nucleus tractus solitarii of adult subjects dying after hypoxic-ischaemic injury. The selective vulnerability of this portion of the nucleus tractus solitarii may be explained mainly with reference to the vascularization of medullary tegmentum. In the carotid body, chronic hypoxia and hyperoxia cause a

Physiological and clinical aspects of apnea diving.

Apnea diving is a fascinating example of applied physiology. The record for apnea diving as an extreme sport is 171 meters, 8:58 minutes. The short time beneath the surface induces profound cardiovascular and respiratory effects. Variations of blood-gas tensions result from the interaction of metabolism and the rapid sequence of compression and decompression. Decompression sickness is possible. Apnea divers can reach depths beyond the theoretic physiologic limit by using the lung-packing maneuver. Apnea divers exhibit a fall in heart rate, which can be trained and is an oxygen-conserving effec

The laryngeal mask airway--anatomical and physiological implications.

The laryngeal mask airway (LMA) is a relatively new general purpose airway which fills a niche between the face mask and tracheal tube, both in terms of anatomical location and degree of invasiveness. It sits in the hypopharynx at the interface between the gastrointestinal and respiratory tracts, where it forms a circumferential low pressure seal around the glottis. This has advantages in terms of gas flow through the upper airway and allows direct access to the glottis without loss of airway control. LMA insertion can be considered in the context of swallowing and combines the ability to inse

Neural control of airway pressures in rabbits.

In twenty anaesthetized and spontaneously breathing rabbits airway pressures were measured above and below the larynx during tidal respiration through the larynx. Peak inspiratory and expiratory pressures at both sites were recorded in control conditions and then compared to values obtained in the course of progressive denervation of the airways. The two methods of denervation consisted of (1) bilateral section of superior and recurrent laryngeal nerves and of the midcervical vagotomy (horizontal method); (2) right-sided sections of the three nerves followed by left-sided sections (vertical me

Ventilatory and circulatory adjustments in the European eel (Anguilla anguilla L.) exposed to short term hypoxia.

Respiratory and circulatory (measured and calculated) variables were obtained at the same time in resting eels, during normoxia and after 1 h exposure to environmental hypoxia (water PO2 of 40 torr). In normoxia, values of respiratory and circulatory variables appeared less than those reported for most other fish. These differences could be partly explained by a lower level of standard metabolism and a greater uptake of O2 through the skin. Hypoxia caused a marked decrease in heart rate (40%), cardiac output (37%), ventral and dorsal arterial blood pressures (22% and 32%), associated with a co
