Reframing Pandemic Preparedness: Why Positive-Pressure Exposure Control Merits Formal Public Health Evaluation.

Powered air-purifying respirators (PAPRs) are respiratory protective devices that actively supply filtered air and maintain positive pressure in the breathing zone, thereby limiting the inward leakage of unfiltered ambient air through gaps. Their principles and technology are already established. Positive-pressure PAPRs have primarily been used by health care workers in high-risk environments. However, their potential as an individual-level infection-control measure applicable to the general public has not been formally positioned for evaluation within the context of pandemic preparedness. Health effects of filtering facepiece respirators: Research and clinical implications of comfort, thermal, skin, psychologic, and workplace effects.

Filtering facepiece respirators (FFR's) such as N95s have become widely used in appropriate settings for personal respiratory protection and are increasingly used beyond workplace settings. Concerns about possible adverse effects have appeared in many publications, particularly since the COVID-19 pandemic led to much more widespread use. This paper synthesizes known effects based upon review of publications in PubMed since 1995, addressing effects other than pulmonary and cardiovascular (reviewed elsewhere). Findings: (1) Subjective discomfort is very frequently reported; this includes general

Evaluation of reported fatality data associated with workers using respiratory protection in the United States (1990-2012).

OSHA revised the 1971 respiratory protection standard in 1998 to add guidance for selecting and maintaining respirators. Fatality reports from 1990 to 2012 were used to characterize historical trends in fatalities associated with respirators. Industry- and time-specific trends were evaluated to determine the effect of the revision to the standard on respirator-related fatalities; 174 respirator-related deaths were reported. The majority of fatalities were associated with using an airline respirator (n = 34) or the absence of using a respirator in required spaces (n = 38). Overall, 79% of fatal

Development of an Ultraviolet-C Irradiation Room in a Public Portuguese Hospital for Safe Re-Utilization of Personal Protective Respirators.

Almost two years have passed since COVID-19 was officially declared a pandemic by the World Health Organization. However, it still holds a tight grasp on the entire human population. Several variants of concern, one after another, have spread throughout the world. The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) omicron variant may become the fastest spreading virus in history. Therefore, it is more than evident that the use of personal protective equipment (PPE) will continue to play a pivotal role during the current pandemic. This work depicts an integrative approach attestin

Psychological problems associated with mission-oriented protective gear.

"Gas mask phobia," a form of claustrophobia, impacts the ability of many service members to perform their mission. Historically, some soldiers have not donned, or quickly removed, their personal protective respirators because of discomfort, ignorance, or shame. Traditionally, little attention has been paid to this as a readiness issue. Exact numbers of service members who are unable to tolerate their masks are unknown. This phobia can be avoided with adequate training and treated with principles of desensitization, but more research is needed in early identification and treatment.

Performance of respirator filters using quality factor in Korea.

A respirator filter of good quality has not only high aerosol collection efficiency but also low air resistance. "Quality factor", which is expressed with aerosol penetration and pressure drop, can be used to rank the performance of respirator filters within the same category. This study focuses on evaluating several respirator filters which are widely used in Korea using quality factor. Two mechanical filters and three filtering facepieces made by different manufacturers were measured aerosol penetrations and pressure drops by an automatic filter tester (CertiTest Model 8110, TSI Inc., St. Pa

Assessment of half-mask elastomeric respirator and powered air-purifying respirator reprocessing for an influenza pandemic.

BACKGROUND: Health care facilities are considering the use of reusable respiratory protective devices (RPDs) to mitigate a potential N95 filtering facepiece respirator shortage caused by an influenza pandemic. US regulators are also considering stockpiling reusable RPDs for pandemic preparedness, but limited data exist on the effectiveness of cleaning and disinfection of these devices. This study defines reprocessing protocols and evaluates their effectiveness against a pandemic influenza strain in a laboratory setting. METHODS: Five half-mask elastomeric respirator models and 3 powered air-p

Firefighter protective clothing and self contained breathing apparatus does not alter balance testing using a standard sensory organization test or motor control test in healthy, rested individuals.

BACKGROUND: There is a high rate of injury associated with firefighting: in 2016, 21% of all fireground injuries were attributed to falls, jumps and slips. Examining factors related to balance, including experience in wearing firefighter gear, may assist in reducing injury related to falls. OBJECTIVES: To assess the effects of wearing firefighter gear on postural balance in firefighters and non-firefighters in a rested condition. METHODS: Each subject attended two sessions. In session 1, informed consent was obtained, a threshold audiogram was collected, and the sensory organization test (SO

Role of helmet ventilation during the 2019 coronavirus disease pandemic.

Coronavirus disease 2019 (COVID-19) has been declared a pandemic by the World Health Organization; it has affected millions of people and caused hundreds of thousands of deaths. Patients with COVID-19 pneumonia may develop acute hypoxia respiratory failure and require noninvasive respiratory support or invasive respiratory management. Healthcare workers have a high risk of contracting COVID-19 while fitting respiratory devices. Recently, European experts have suggested that the use of helmet continuous positive airway pressure should be the first choice for acute hypoxia respiratory failure ca

Helmet noninvasive ventilation compared to facemask noninvasive ventilation and high-flow nasal cannula in acute respiratory failure: a systematic review and meta-analysis.

BACKGROUND: Although small randomised controlled trials (RCTs) and observational studies have examined helmet noninvasive ventilation (NIV), uncertainty remains regarding its role. We conducted a systematic review and meta-analysis to examine the effect of helmet NIV compared to facemask NIV or high-flow nasal cannula (HFNC) in acute respiratory failure. METHODS: We searched multiple databases to identify RCTs and observational studies reporting on at least one of mortality, intubation, intensive care unit (ICU) length of stay, NIV duration, complications or comfort with NIV therapy. We asses

Disrupting the transmission of influenza a: face masks and ultraviolet light as control measures.

In the event of an influenza pandemic, where effective vaccine and antiviral drugs may be lacking, disrupting environmental transmission of the influenza virus will be the only viable strategy to protect the public. We discuss 2 such modalities, respirators (face masks) and ultraviolet (UV) light. Largely overlooked, the potential utility of each is underappreciated. The effectiveness of disposable face masks may be increased by sealing the edges of the mask to the face. Reusable masks should be stockpiled, because the supply of disposable masks will likely prove inadequate. UV light, directed

Psychological problems associated with the wearing of industrial respirators: a review.

This review has dealt with the psychological problems associated with the use of industrial respirators. The need for men and women to wear various self contained breathing apparatus (SCBA) while working in hostile environments such as toxic and oxygen deficient atmospheres is well documented. Extensive research has been carried out in an attempt to improve the design of respirators, and major advances have resulted in the manufacture of SCBA that insure health, safety, and performance. Indeed, recent research reveals that performance decrements associated with the wearing of respirators are d

Personal protective equipment for preventing highly infectious diseases due to exposure to contaminated body fluids in healthcare staff.

BACKGROUND: In epidemics of highly infectious diseases, such as Ebola Virus Disease (EVD) or SARS, healthcare workers (HCW) are at much greater risk of infection than the general population, due to their contact with patients' contaminated body fluids. Contact precautions by means of personal protective equipment (PPE) can reduce the risk. It is unclear which type of PPE protects best, what is the best way to remove PPE, and how to make sure HCWs use PPE as instructed. OBJECTIVES: To evaluate which type or component of full-body PPE and which method of donning or removing (doffing) PPE have t
