Understanding respiratory rate as a predictor of clinical deterioration and mortality.

Respiratory rate (RR) is a fundamental physiological observation and a key component of early warning systems used to identify deterioration in hospitalised adults. Accurate RR measurement and interpretation, alongside other clinical observations and timely escalation of abnormal findings, are essential to ensure safe patient care. However, despite its clinical importance, RR remains one of the least reliably measured vital signs in practice and it is frequently estimated rather than counted accurately. This article explores the evidence on RR as a predictor of clinical deterioration and morta

The effect of temperature reduction on respiratory rate in febrile illnesses.

A raised respiratory rate is a useful sign in the diagnosis of pneumonia in children. It was observed that children with malaria and other febrile illnesses may also present with a raised respiratory rate. To determine the extent to which increased body temperature contributes to the raised respiratory rate observed in these children the effect of change in body temperature on respiratory rate was measured in 186 sick Gambian children with a raised respiratory rate, including those with pneumonia or malaria. A temperature dependent effect on respiratory rate of 3.7 breaths per minute per degre

Clinical Validation of a Wearable Respiratory Rate Device for Neonatal Monitoring.

Respiratory rate monitoring is of paramount importance in neonatal care. Manual counting of expansions and contractions of the abdomen or diaphragm of the neonate is still the widely accepted measure of respiratory rate in most clinical settings. A practical, affordable, easy-to-use technology to continuously measure respiratory rate in neonates is essential to recognize the signs and symptoms of respiratory disorders. Clinical validation of a system for continuous and long term respiratory rate monitoring of neonates, in a wearable form factor with the capability of remote monitoring is prese

Heart rate response to respiratory events with or without leg movements.

STUDY OBJECTIVE: This study compares the heart rate responses to the termination of respiratory events, both with and without associated leg movements. METHODS: Heart rate was measured for 15 R-R intervals before (T-15 to T-1) and after (T+1 to T+15) the termination of respiratory events as a change from the baseline rate, defined as the average of 10 R-R intervals occurring before the termination of each respiratory event (T-15 to T-6). Individual heart rate changes of the 21 patients were then averaged separately for 10 respiratory events with and 10 without associated leg movements. SETTI

Respiratory complaints and high sensitization rate at a rennet-producing plant.

BACKGROUND: Workers in a rennet producing plant reported airway symptoms, mainly rhinitis. Rennet was produced as fluid and powder with proteolytic enzymes as the active component. METHODS: Data on airway symptoms and known allergies were collected and skin prick tests (SPTs) with the rennet types produced in the plant and standard allergens were carried out on 35 plant employees. An identical SPT was carried out on 28 controls without known exposure to rennet. RESULTS: Twenty-one employees (60%) had hay fever-like symptoms, 10 cases mainly at work. Nine had mild to moderate asthma-like symp

Gemcitabine-induced respiratory failure associated with elevated erythrocyte sedimentation rate (ESR).

Gemcitabine is a newer pyrimidine analog used for the treatment of solid tumors. Though generally considered safe, it can cause pulmonary toxicity, which is generally mild and reversible. The purpose of this publication is to document a case of adult respiratory distress syndrome (ARDS) caused by Gemcitabine administration. The patient was noticed to have the extreme elevation of erythrocyte sedimentation rate (ESR) (> 100 mm/h) at the time of diagnosis. We observed excellent response to the corticosteroids. The ESR trended back to normal after the resolution of her symptoms thereby suggesting

Measurement of heart rate and respiratory rate using remote photoplethysmography in paediatrics: a prospective comparative trial protocol - 'rMonitoped1'.

INTRODUCTION: Vital signs such as heart rate (HR) and respiratory rate (RR), crucial for clinical assessment, are often challenging to measure in paediatric populations. Remote photoplethysmography (rPPG), a video-based measurement tool, has demonstrated accuracy in adults. The objective of this study is to compare HR and RR measurements obtained using rPPG with those from standard clinical monitoring in a paediatric population. METHODS AND ANALYSIS: This is a monocentric, prospective study enrolling 600 paediatric participants. Each participant will have standard monitoring electrodes (ECG/i

Comparison of Techniques for Respiratory Rate Extraction from Electrocardiogram and Photoplethysmogram.

BACKGROUND: Respiratory rate (RR) is a key vital sign and one of the most sensitive indicators of physiological conditions, playing a crucial role in the early identification of clinical deterioration. The monitoring of RR using electrocardiography (ECG) and photoplethysmography (PPG) aims to overcome limitations of traditional methods in clinical settings. METHODS: The proposed approach extracts RR from ECG and PPG signals using different morphological and temporal features from publicly available datasets (iAMwell and Capnobase). The algorithm was used to develop and test with a selection o

Controlling heart rate variability for respiratory effects in ambulatory psychophysiological measurements.

Respiratory sinus arrhythmia (RSA) is the heart period variability observed in synchrony with respiration. RSA amplitude is widely used in psychophysiological research to non-invasively index cardiac vagal activity. However, RSA measures are significantly affected by respiratory behavior, even in the absence of changes in cardiac vagal activity. Fifty-to-sixty percent of the variation in RSA can be attributed to respiration rate and tidal volume. This poses a notable challenge for ambulatory RSA measurement where respiratory behavior cannot be experimentally controlled and can show substantial

Heart Rate Variability to Automatically Identify Hyperbaric States Considering Respiratory Component.

This study's primary objective was to identify individuals whose physiological responses deviated from the rest of the study population by automatically monitoring atmospheric pressure levels to which they are exposed and using parameters derived from their heart rate variability (HRV). To achieve this, 28 volunteers were placed in a dry hyperbaric chamber, where they experienced varying pressures from 1 to 5 atmospheres, with five sequential stops lasting five minutes each at different atmospheric pressures. The HRV was dissected into two components: the respiratory component, which is linked

Open-source software for respiratory rate estimation using single-lead electrocardiograms.

Respiratory rate (RR) is a critical vital sign used to assess pulmonary function. Currently, RR estimating instrumentation is specialized and bulky, therefore unsuitable for remote health monitoring. Previously, RR was estimated using proprietary software that extract surface electrocardiogram (ECG) waveform features obtained at several thoracic locations. However, developing a non-proprietary method that uses minimal ECG leads, generally available from mobile cardiac monitors is highly desirable. Here, we introduce an open-source and well-documented Python-based algorithm that estimates RR re

Estimation of the respiratory rate from ballistocardiograms using the Hilbert transform.

BACKGROUND: Measuring the respiratory rate is usually associated with discomfort for the patient due to contact sensors or a high time demand for healthcare personnel manually counting it. METHODS: In this paper, two methods for the continuous extraction of the respiratory rate from unobtrusive ballistocardiography signals are introduced. The Hilbert transform is used to generate an amplitude-invariant phase signal in-line with the respiratory rate. The respiratory rate can then be estimated, first, by using a simple peak detection, and second, by differentiation. RESULTS: By analysis of a s
