[Dyspnea and chronic recurrent respiratory tract infections].

A 48-year-old man presented with recurrent bronchopulmonary infections of many years' standing. The diagnosis of tracheobronchomegaly (Mounier-Kuhn syndrome) was based on CT examination and subsequent bronchoscopy. Since lung transplantation was not considered advisable, a biluminal stent was inserted to prevent bronchial collapse.

Introduction: Evolving needs in respiratory tract infections.

Two issues that have become clinically relevant to the treatment of pneumonia over the past few years are the development of antibiotic resistance among respiratory pathogens and the increasing importance of the atypical respiratory pathogens---Mycoplasma pneumoniae, Chlamydia pneumoniae and Legionella spp. Resistance has become an important issue in Streptococcus pneumoniae, methicillin-resistant Staphylococcus aureus and Gram-negative rods. The ways by which bacteria become resistant to antibiotics include production of antibiotic-modifying enzymes, reduced access to target sites, efflux of

Exercise, immunology and upper respiratory tract infections.

The literature reveals a paradoxical response of the immune and host defense systems to endurance exercise apparent stimulation following long-term regular training and suppression in response to acute exposure to exhaustive endurance exercise. Several epidemiological surveys have confirmed a clinical manifestation of immunosuppression in the form of increased incidence of upper respiratory tract infection symptoms following participation in competitive marathon and ultramarathon running events. Prerace training status and racing intensity have been related to the incidence of this symptomatol

Allergic rhinitis and upper respiratory tract infections.

The existence of a relationship between allergic rhinitis and upper respiratory tract infection (URI) has not yet been verified. In a prospective study, 64 patients with allergic rhinitis and 23 non-allergic individuals were followed for 11.4 months. All participants kept a diary in which they recorded symptoms of URI, nasal discharge, blocked nose, sneezing and sinusitis. The allergic individuals did not have more frequent or more long lasting URI or sinusitis more frequently than non-allergic subjects.

Ambulatory management of lower respiratory tract infections.

Clinical manifestations and radiographic findings are unreliable guides to the selection of antimicrobial therapy for lower respiratory infections. Laboratory evaluation is necessary to identify the etiologic agent. Multiple oral antibiotics are available for outpatient treatment of bronchitis or pneumonia suspected to be of bacterial origin.

[Therapy of chronic recurrent respiratory tract infections].

In 1987, the most frequently identified pathogens in chronic respiratory tract infections in our clinic were Haemophilus influenzae, Pseudomonas aeruginosa, Branhamella catarrhalis, Streptococcus pneumoniae, Staphylococcus aureus and Klebsiella pneumoniae. Recurrent infection is a common phenomenon in patients with chronic respiratory tract infections, including chronic bronchitis, chronic bronchiolitis and bronchiectasis. H. influenzae is the most common pathogen in such patients. Macrolides, tetracyclines and new quinolones were effective to protect against recurrent infection of H. influenz

[Respiratory tract infections--clinical results with ofloxacin].

In an open clinical trial, out-patients with respiratory tract infections were given 200 mg ofloxacin b. i. d. orally. 36 had acute bronchitis and pneumonia was diagnosed in 44. The average duration of therapy was nine days for bronchitis and 12 days for pneumonia. In the sputum of bronchitis patients, Haemophilus influenzae (n = 25), Streptococcus pneumoniae (n = 18), Branhamella catarrhalis (n = 2) and Pasteurella multocida (n = 1) were isolated. 17 H. influenzae and 12 S. pneumoniae were eliminated. All 20 S. pneumoniae strains isolated from patients with pneumonia were eliminated. A cure o

Clindamycin in the upper respiratory tract infections.

Bacterial infections of the upper respiratory airways are common. Some of these infections are caused by anaerobic bacteria and Staphylococcus aureus and may constitute considerable therapeutical problems. Chronic suppurative otitis media may sometimes give rise to serious osteitis of surrounding bone structures, sigmoid sinus thrombosis and intracranial abscesses. The causative microorganisms are mostly anaerobes. Many paranasal sinus infections are also due to anaerobic bacteria. These infections can rapidly cause irreversible damage of the sinus mucosa and eventually spread intracranially.

[Lower respiratory tract infections in immunocompetent adult].

Lower respiratory tract infections represent one of the main causes of mortality in the world. They essentially consist of bronchitis, acute exacerbations of chronic obstructive bronchopulmonary diseases (COPD) and acute pneumonia. If acute bronchitis is mainly of viral origin, acute exacerbations of COPD and pneumonia are mainly due to a trio of bacteria (Streptococcus pneumoniae, Haemophilus influenzae, and Moraxella catarrhalis). Other pathogens as many viruses and atypical bacteria such as Mycoplasma pneumoniae, Legionella, some Enterobacteriaceae and very rarely Pseudomonas aeruginosa are

Vaccinations against respiratory tract infections at Hajj.

The transmission of respiratory tract infections (RTIs) is very high among the Hajj congregation in Mecca, Saudi Arabia. Despite recommendations for vaccinations, pilgrims remain at increased risk of RTIs. In this paper we systematically reviewed available studies assessing the uptake and effectiveness of vaccinations against RTIs among Hajj pilgrims and enumerated important demographic factors, if described, associated with vaccine uptake. Of the 42 included studies, 29 reported on the uptake and effectiveness of influenza vaccine among pilgrims, eight studies reported the uptake of other vac

Inhaled colistin for lower respiratory tract infections.

INTRODUCTION: Lower respiratory tract infections, due to Pseudomonas aeruginosa or Acinetobacter baumannii, are frequently encountered in patients with cystic fibrosis (CF) or in patients developing nosocomial pneumonias. Both of these conditions bear a high mortality risk and aggressive antibiotic therapy is necessary. Inhaled antibiotics might represent an effective therapeutic approach for these diseases as it has demonstrated good bactericidal efficacy and safety in both preclinical and clinical studies. This colistin formulation might be useful particularly in patients with respiratory tr

[Contemporary clinical diagnostics of respiratory tract infections].

The patient population at risk pulmonary infections has increased during the last decade. The spectrum of organisms causing infections has also grown, most of them are viruses Baseline diagnostic assessment include history, clinical examination, radiography and measurements of procalcitonin (PCT), highly sensitive C-reactive protein (hsCRP) and leukocyte count. PCT represents a good biological marker for difficult diagnoses in critically ill patients and is superior to CRP. Chest x-rays seldom give enough information to determine the exact cause of the abnormality, but they can help a doctor t

Azithromycin for acute lower respiratory tract infections.

BACKGROUND: Acute lower respiratory tract infections (LRTI) range from acute bronchitis and acute exacerbations of chronic bronchitis to pneumonia. Approximately five million people die of acute respiratory tract infections annually. Among these, pneumonia represents the most frequent cause of mortality, hospitalization and medical consultation. Azithromycin is a new macrolide antibiotic, structurally modified from erythromycin and noted for its activity against some gram-negative organisms associated with respiratory tract infections, particularly Haemophilus influenzae (H. influenzae). [Immunity and pathophysiology of respiratory tract infections].

The respiratory tract is permanently exposed to infections that may remain localized (bronchitis, pneumonias) or become potentially invasive (bacteremia and meningitis). It can be considered as an immunologic organ the upper part of which, the tracheobronchial tree, has the same secretory epithelium as the naso-oropharynx and shares bronchial associated lymphoid tissue (BALT). In this tissue, secretory IgA are more abundant than IgG. It is colonized by a commensal bacterial flora, including some potentially pathogenic species (Streptococcus pneumoniae, Haemophilus influenzae, Neisseria meningi

Antibiotics, acne, and upper respiratory tract infections.

About two million people per year in the U.S. have acne severe enough to require treatment with antibiotics. Treatment frequently lasts for more than six months, prompting concerns about antibiotic resistance and other possible consequences of long-term antibiotic use, such as increased susceptibility to infections. This Issue Brief summarizes a large study that evaluates the risk for upper respiratory and urinary tract infections among adolescents and young adults treated with antibiotics for acne.
